1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

3Soluzioni

1.Priorità e delega

Risposta: D · Jaw thrust, keeping the head and neck in line

Perché è questa la risposta

A diving injury may damage the cervical spine. A jaw thrust opens the airway with little neck movement; if it fails, the airway still takes priority.

Risposta

D · Jaw thrust, keeping the head and neck in line
2.Priorità e delega

Risposta: C · Acute arterial occlusion, which is an emergency

Perché è questa la risposta

Sudden pain, pallor, coldness and loss of pulse suggest an embolus blocking an artery. The limb can be lost within hours, so the nurse escalates immediately.

Risposta

C · Acute arterial occlusion, which is an emergency
3.Controllo delle infezioni e sicurezza del paziente

Risposta: C · Set it up just before the procedure, because a waiting field may become contaminated

Perché è questa la risposta

The longer a sterile field is exposed, the more likely it is to be contaminated, and an unattended field cannot be considered sterile. It is prepared just before use.

Risposta

C · Set it up just before the procedure, because a waiting field may become contaminated
4.Controllo delle infezioni e sicurezza del paziente

Risposta: C · Leave the area, keep others out and alert the trained response team

Perché è questa la risposta

An unknown chemical spill may be toxic or flammable. The nurse moves people away, prevents entry and calls the designated hazardous materials team, who check the Safety Data Sheet and use the right equipment.

Risposta

C · Leave the area, keep others out and alert the trained response team
5.Valori di laboratorio ed elettroliti

Risposta: D · In the first 15 minutes

Perché è questa la risposta

Acute hemolytic and anaphylactic reactions usually appear early, so the transfusion is started slowly and the nurse stays with the patient for about the first 15 minutes, checking vital signs before, during and after according to policy.

Risposta

D · In the first 15 minutes
6.Valori di laboratorio ed elettroliti

Risposta: C · Acute kidney injury, because it rose by at least 0,3 mg/dL within 48 hours

Perché è questa la risposta

KDIGO defines AKI as a creatinine rise of at least 0,3 mg/dL (26,5 µmol/L) within 48 hours, a rise to 1,5 times baseline within 7 days, or urine output below 0,5 mL/kg/h for 6 hours. A result inside the reference range can still be a significant rise.

Risposta

C · Acute kidney injury, because it rose by at least 0,3 mg/dL within 48 hours
7.Infermieristica clinica

Risposta: D · Pregnant women and immunocompromised people who have not had chickenpox

Perché è questa la risposta

Someone who has never had chickenpox can catch chickenpox from contact with shingles blisters. Non-immune pregnant and immunocompromised people are at particular risk, so lesions are covered and precautions followed.

Risposta

D · Pregnant women and immunocompromised people who have not had chickenpox
8.Infermieristica clinica

Risposta: C · Subarachnoid haemorrhage

Perché è questa la risposta

A sudden thunderclap headache suggests bleeding into the subarachnoid space, often from a ruptured aneurysm. It needs an urgent CT scan and neurosurgical input.

Risposta

C · Subarachnoid haemorrhage
9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: C · Rapid high doses can damage hearing

Perché è questa la risposta

High plasma levels of loop diuretics can cause tinnitus and hearing loss (ototoxicity), especially with fast injection, renal impairment or other ototoxic drugs such as aminoglycosides. Large IV doses are therefore given slowly.

Risposta

C · Rapid high doses can damage hearing
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: B · Hypercalcaemia

Perché è questa la risposta

Squamous cell lung cancer can secrete PTH-related protein, and bone metastases can release calcium. Hypercalcaemia causes confusion, thirst, polyuria, constipation, abdominal pain and arrhythmias ("bones, stones, groans and psychiatric overtones"). Treatment includes IV fluids and bisphosphonates.

Risposta

B · Hypercalcaemia
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: C · Wait to give the insulin until the meal arrives

Perché è questa la risposta

Rapid-acting insulin begins to work within about 15 minutes, so giving it without food risks hypoglycaemia. It is given just before or with the meal. Basal insulin is not affected by meal delays.

Risposta

C · Wait to give the insulin until the meal arrives
12.Area medico-chirurgica: neurologica, endocrina e renale

13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: C · Comunemente circa 48 ore

Risposta

C · Comunemente circa 48 ore
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: A · Inform the surgical and anaesthetic teams, who will check the INR

Perché è questa la risposta

Recent warfarin may leave the INR too high for safe surgery or a spinal anaesthetic. The team needs to know at once; they check the INR and decide whether to delay or reverse.

Risposta

A · Inform the surgical and anaesthetic teams, who will check the INR
