Why is the level of sedation a useful early warning in a patient on opioids?
- ASedation means the opioid dose is too low
- BSedation shows that the pain has completely gone and the dose is right
- CIncreasing sedation usually comes before respiratory depression
- DSedation only occurs after the breathing has stopped

B · Discard it in front of a second licensed (registered) nurse, who witnesses and documents it
Wasting controlled substances requires a witness, usually another licensed nurse, and both document the waste. This prevents and detects drug diversion.
B · Discard it in front of a second licensed (registered) nurse, who witnesses and documents it
C · “We keep the gun loaded in the nightstand, and the children know not to touch it.”
Firearms in homes with children should be stored unloaded and locked, with ammunition stored separately and locked. Telling children not to touch a gun does not reliably prevent injuries.
C · “We keep the gun loaded in the nightstand, and the children know not to touch it.”
B · Ask simple yes/no questions and allow plenty of time
Short questions that can be answered with yes, no or a gesture, picture boards and patience let the patient communicate. Hearing is not the problem, so shouting does not help.
B · Ask simple yes/no questions and allow plenty of time
C · Increasing sedation usually comes before respiratory depression
Patients usually become drowsy before their breathing slows dangerously. Checking sedation with a simple scale alongside respiratory rate allows early action.
C · Increasing sedation usually comes before respiratory depression
