- ASleep sitting up in a chair
- BAvoid drinking water before bed
- CNever swim or bathe alone
- DAvoid all forms of exercise
A nurse has worked three long shifts in a row and feels exhausted. The manager asks the nurse to stay for another shift. What is the most appropriate response?
- AAgree, but skip the independent medication checks to save energy on the shift
- BExplain that fatigue would make care unsafe and decline the extra shift
- CAgree and drink extra coffee to stay alert
- DAgree without telling anyone about the fatigue
A patient is getting up for the first time after several days in bed. What should the nurse do first?
- AHave the patient sit on the edge of the bed for a short time before standing
- BHelp the patient stand up quickly, so that the dizziness has less time to start
- CGive the patient a large drink of water while standing
- DAsk the patient to walk to the bathroom alone
A dehydrated patient is receiving IV fluids. Which finding best shows the treatment is working?
- AUrine output rises and the heart rate falls towards normal
- BThe urine becomes darker and more concentrated
- CThe patient feels more thirsty than before the fluids started
- DThe heart rate rises and the skin becomes cooler

Staphylococcus aureus

C · Never swim or bathe alone
People with uncontrolled seizures are advised not to swim alone, to shower rather than bathe and to follow local laws on driving. Most normal activities can continue with sensible precautions.
C · Never swim or bathe alone
B · Explain that fatigue would make care unsafe and decline the extra shift
Long hours and fatigue increase errors and injuries. Nurses have a professional responsibility to judge whether they are fit to work safely and to say so clearly.
B · Explain that fatigue would make care unsafe and decline the extra shift
A · Have the patient sit on the edge of the bed for a short time before standing
After bed rest blood pressure may drop on standing. Sitting on the edge of the bed first, with the feet on the floor, lets the body adjust and lowers the risk of fainting and falls.
A · Have the patient sit on the edge of the bed for a short time before standing
A · Urine output rises and the heart rate falls towards normal
With effective fluid replacement, perfusion improves: urine output increases, the heart rate settles and the blood pressure stabilises. Watching for fluid overload is also important.
A · Urine output rises and the heart rate falls towards normal
