pH 7.35–7.45, PaCO₂ 35–45 mmHg (4.7–6.0 kPa), HCO₃⁻ 22–26 mmol/L
ROME
PaCO₂ = 1.5·HCO₃⁻ + 8 ± 2
PaCO₂ ≈ 0.7·HCO₃⁻ + 21
AG = Na⁺ − (Cl⁻ + HCO₃⁻) ≈ 8–12
AG + 2.5·(4.0 − alb g/dL) = AG + 0.25·(40 − alb g/L)
(AG − 12)/(24 − HCO₃⁻)
P/F 比
PaO₂/FiO₂
PAO₂ = FiO₂·(760 − 47) − PaCO₂/0.8
2·Na⁺ + glucose + urea (2·Na⁺ + glucose/18 + BUN/2.8)

解き方

D · It is always diluted and given by infusion pump, never as an IV push

Concentrated potassium chloride given as an IV push can cause fatal cardiac arrest, so it is never pushed. It is diluted, mixed well and given by infusion pump at a rate set by protocol, often with cardiac monitoring at higher rates.

答え

D · It is always diluted and given by infusion pump, never as an IV push

D · Keep the IV open with normal saline using new tubing and call the provider

These signs suggest an acute hemolytic reaction. The transfusion is stopped, the line is kept open with normal saline through new tubing, the provider and blood bank are informed, and the bag, tubing and blood and urine samples are sent for testing as per policy. Hemolytic transfusion reaction · MedlinePlus (NIH) (2025)

答え

D · Keep the IV open with normal saline using new tubing and call the provider

Osm = 292 mOsm/kg

海面での肺胞気酸素分圧は PAO₂ = FiO₂·(760 − 47) − PaCO₂/0.8 mmHg で、A–a 勾配は PAO₂ − PaO₂ です。

Osm = 2·134 + 5.8 + 18.1 = 291.9 ≈ 292 mOsm/kg

答え

Osm = 292 mOsm/kg

24 + 1·2.5 = 26.5 mmol/L
24 + 3.5·2.5 = 32.75 mmol/L

CL = 0.05·79 = 3.95 L/h

R = CL·Css/S = 3.95·12/0.8 = 59.25 mg/h

答え

R = 59.25 mg/h

(Cockcroft–Gault)

CrCl = (140 − 41)·67/(72·2.7)·0.85 = 29 mL/min

(Dettli)

Q = 1 − 0.7·(1 − 29/120) = 0.4692

D = 0.4692·250 = 117.3 ≈ 117 mcg

答え

D = 117 mcg

B · Spare tracheostomy tubes, including one a size smaller, and working suction

Emergency equipment such as spare tubes (same size and one smaller), suction, oxygen and tracheal dilators must be at the bedside. This allows rapid management of a blocked or displaced tube.

答え

B · Spare tracheostomy tubes, including one a size smaller, and working suction

D · Without any insulin they can develop diabetic ketoacidosis

People with type 1 diabetes make no insulin, so stopping it causes ketosis and DKA. Basal insulin is usually continued, often with a variable-rate insulin infusion and glucose monitoring.

答え

D · Without any insulin they can develop diabetic ketoacidosis
