A patient's potassium is unexpectedly 5.8 mEq/L. The sample was taken after the tourniquet was on for several minutes while the patient pumped their fist. What is the best interpretation?
A health care worker's hepatitis B tests show: HBsAg negative, anti-HBs positive, anti-HBc positive. What does this mean?
Compared with a colostomy, a patient with an ileostomy is at greater risk of:
After a head injury, a patient passes very large volumes of dilute urine and the serum sodium is rising. What is most likely?
A · The result may be falsely high and should be repeated with better technique
Repeated fist clenching and a long tourniquet time release potassium from muscle and can cause hemolysis, both of which give a falsely high potassium. The patient is assessed and the test is repeated with correct technique; true hyperkalemia must not be missed. Potassium Blood Test · MedlinePlus (NIH) (2024); Potassium test · MedlinePlus (NIH) (2025)
A · Past infection that has resolved, with immunity
The vaccine contains only surface antigen, so vaccination produces anti-HBs alone. A positive anti-HBc shows past or current infection; with HBsAg negative and anti-HBs positive, the infection has cleared and the person is immune. Hepatitis Testing · MedlinePlus (NIH)
HCO₃⁻ = 28.55 mmol/L
PaCO₂ = 44.8 mmHg

B · Dehydration and electrolyte loss from liquid output
Ileostomy output is liquid because it bypasses the colon, which normally absorbs water and salts. Patients must drink enough and watch for signs of dehydration, especially in hot weather or illness.
A · Diabetes insipidus
Cranial diabetes insipidus is a lack of ADH, so the kidneys cannot concentrate urine. Water loss raises sodium; strict fluid balance and replacement are needed.
