A patient with chronic kidney disease stage 4 has a high phosphate, a low-normal calcium and a very high parathyroid hormone (PTH). What does this show?

A patient taking warfarin has an INR of 5.2 and no signs of bleeding. What should the nurse do?
A patient on a cardiac monitor suddenly becomes unresponsive, and the monitor shows ventricular fibrillation. What is the priority once help has been called?
D · Cold hands with poor peripheral circulation
Poor perfusion (cold, shock, vasoconstriction), movement, dark nail varnish or bright ambient light can make readings unreliable. The nurse checks the waveform, warms the hand or uses an ear probe, and compares the reading with the clinical picture.
D · Stay for observation, then go home with an adrenaline auto-injector and teaching
Biphasic reactions can occur hours after the first reaction resolves, so patients are observed for a period determined by severity and local guidance. Before discharge they receive adrenaline auto-injectors with training, a written plan and referral to an allergy service.
C · To stop levodopa being converted to dopamine outside the brain
Carbidopa blocks the enzyme that converts levodopa to dopamine in the periphery. More levodopa reaches the brain, so a lower dose works, and side effects such as nausea and low blood pressure are reduced.
C · Carbamazepine can cause hyponatraemia through an SIADH-like effect
Carbamazepine (and oxcarbazepine) can increase the effect of ADH, leading to water retention and hyponatraemia. Severe hyponatraemia causes confusion and seizures. The drug and sodium need urgent medical review.
B · Stopping smoking
Smoking worsens Crohn's disease, increasing flares, complications and the need for surgery. Stopping smoking is one of the most effective changes a patient can make.
C · To stop blood clots from blocking the catheter
Irrigation dilutes blood and flushes out clots so the catheter does not block. Output must be measured carefully and urine output calculated by subtracting the irrigation volume.
D · Diabetes insipidus
Cranial diabetes insipidus is a lack of ADH, so the kidneys cannot concentrate urine. Water loss raises sodium; strict fluid balance and replacement are needed.
Anemia · MedlinePlus (NIH) (2026)
C · Secondary hyperparathyroidism from CKD
In CKD, phosphate retention and reduced activation of vitamin D lower calcium, which stimulates the parathyroid glands. The result is high PTH with high phosphate, causing bone disease. Phosphate binders, diet and active vitamin D are used. Parathyroid Hormone (PTH) Test · MedlinePlus (NIH) (2023)
答え: A–a 勾配 = 9.98 mmHg
海面での肺胞気酸素分圧は PAO₂ = FiO₂·(760 − 47) − PaCO₂/0.8 mmHg で、A–a 勾配は PAO₂ − PaO₂ です。
B · Hold the warfarin dose and inform the provider
An INR well above target raises the risk of serious bleeding. The nurse holds the dose, informs the provider and watches for bleeding. Without bleeding, holding warfarin is usually enough at this level; vitamin K is mainly used for very high INRs or bleeding.
B · Defibrillate as soon as the defibrillator is available, with CPR until then
Ventricular fibrillation is a shockable cardiac arrest rhythm. Early defibrillation, with high-quality CPR until the shock is given, is the most important treatment.
