Which telephone call should a clinic nurse return first?
A patient with diabetes takes propranolol. Which sign of hypoglycemia is most likely to still appear?
A · Distributive shock
In distributive shock (septic, anaphylactic, neurogenic), vessels dilate and leak, so blood is maldistributed and pressure falls. Obstructive shock includes PE, tamponade and tension pneumothorax; cardiogenic shock is pump failure; hypovolaemic shock is volume loss.
B · Loss of movement and pain sense, with position sense kept
Anterior cord syndrome usually follows damage to the anterior spinal artery. Motor, pain and temperature pathways are lost below the injury, but the dorsal columns (fine touch, vibration and proprioception) at the back are spared.
D · Thyroid function tests
Amiodarone is rich in iodine and can cause both hypothyroidism and hyperthyroidism. Thyroid function is checked before and during treatment. The symptoms here suggest hypothyroidism.
D · To confirm that the vaccine produced immunity
Some people do not develop protective anti-HBs antibodies after vaccination. Testing identifies non-responders, who may need further doses and advice after exposures.
B · With the head of the bed raised to at least 30 degrees
Keeping the head raised to at least 30–45° during feeding and for a period afterwards lowers the risk of aspiration pneumonia.
D · Paralytic ileus
Paralytic ileus is a temporary loss of gut movement, common after abdominal surgery. Bowel sounds are absent, unlike mechanical obstruction where they are often high-pitched. Management includes nil by mouth, NG decompression, fluids and correcting electrolytes.
A · Iron deficiency without anemia yet
Iron stores (ferritin) are depleted before the hemoglobin falls. A low ferritin with a normal hemoglobin indicates iron deficiency without anemia, and it is usually treated, along with the cause of blood loss. Ferritin Blood Test · MedlinePlus (NIH) (2025); Iron deficiency anemia - Medical Encyclopedia · MedlinePlus (NIH) (2025)
A · The supplements are reducing levothyroxine absorption
Calcium, iron and antacids bind levothyroxine in the gut and reduce absorption, so the TSH rises. Levothyroxine is taken on an empty stomach, usually 30–60 minutes before breakfast, separated from these supplements by about 4 hours.
AGc = 23.75 mmol/L
C · A parent of a 6-week-old with a temperature of 38.3 °C
A fever in an infant under about 3 months old needs urgent assessment, because serious bacterial infection is more likely and signs can be subtle. The other calls are routine.
A · Sweating
Beta-blockers can hide tachycardia, palpitations and tremor during hypoglycemia. Sweating is mainly cholinergic, so it usually remains, and patients are taught to check glucose when they sweat.
