A patient has a tonic-clonic seizure in bed. What should the nurse do first?
A patient with MRSA in a wound is on contact precautions. Which instruction to the nursing assistant is correct?
D · Troponin may take a few hours to rise after muscle damage
Troponin starts to rise within hours of myocardial injury, so a very early result can be normal. Protocols use serial measurements at set intervals, with the ECG and clinical picture, to diagnose or exclude MI. A rising pattern supports acute injury.
B · Call for expert help and give oxygen to both the face and the stoma
In the first days after a tracheostomy, the tract is immature and the tube can easily be misplaced into the tissues. Emergency guidance is to call for an airway expert, apply oxygen to face and stoma and assess breathing. Reinsertion is done only by trained staff.
A · Hypoactive delirium
Hypoactive delirium presents with drowsiness, withdrawal and slowness rather than agitation, so it is often missed. It has a worse prognosis than the hyperactive form. A family report of a sudden change is important, and a screening tool such as the 4AT or the CAM (Confusion Assessment Method) can be used.
C · A narrowing such as an oesophageal tumour
Progressive dysphagia, solids first then liquids, with weight loss, is typical of a mechanical obstruction such as oesophageal cancer or a stricture. It needs an urgent referral for endoscopy.
B · A catheter-related deep vein thrombosis
PICCs increase the risk of upper limb DVT. Arm swelling, pain and dilated superficial veins are reported for ultrasound assessment and possible anticoagulation; the line is not used until reviewed.
D · Fever, rigors and loin pain
Pyelonephritis is an upper urinary tract infection and causes systemic signs such as fever and rigors, with flank pain. Cystitis usually causes local symptoms such as dysuria and frequency.
C · The sodium can rise too quickly
Tolvaptan blocks ADH receptors in the kidney, causing free water loss. Sodium can rise too fast, risking osmotic demyelination, so treatment is started in hospital with frequent sodium checks. Fluid restriction is usually avoided at the start.
B · Stress hyperglycemia from acute illness
Stress hormones and inflammation raise glucose in acute illness, even without diabetes. A normal HbA1c shows that glucose was normal over the previous months. Stress hyperglycemia is still treated, usually with insulin, because it worsens outcomes. Hemoglobin A1C (HbA1c) Test · MedlinePlus (NIH) (2025)
Osm = 287 mOsm/kg
海面での肺胞気酸素分圧は PAO₂ = FiO₂·(760 − 47) − PaCO₂/0.8 mmHg で、A–a 勾配は PAO₂ − PaO₂ です。
C · Protect the head and turn the patient on their side when possible
During a seizure the nurse protects the patient from injury and turns them to the side to keep the airway clear. Nothing is put in the mouth, movements are not restrained and nothing is given by mouth.
A · Wear a gown and gloves for contact with the patient or the room, and use dedicated equipment
MRSA spreads through direct contact and contaminated surfaces. Gown and gloves, hand hygiene and dedicated equipment prevent transmission; a respirator is not needed for contact precautions.
