A nurse teaches a patient about storing insulin at home. Which statement shows a need for further teaching?
Which finding after a laparoscopic cholecystectomy is expected and needs only reassurance and comfort measures?
A · To kill slow-growing bacteria and prevent drug resistance
Mycobacterium tuberculosis grows slowly and has resistant mutants in any large population. A combination (for example rifampicin, isoniazid, pyrazinamide and ethambutol at first) kills susceptible and resistant bacteria and prevents resistance. Stopping early risks relapse and multidrug-resistant TB.
C · Sit the patient up, give oxygen and call for urgent airway help
Post-extubation stridor suggests laryngeal oedema and may progress to complete obstruction. The patient is sat up, given humidified oxygen and assessed by an airway expert urgently. Nebulised adrenaline and steroids may be used, and reintubation may be needed.
C · Ask questions that can be answered with yes, no or a gesture
In expressive aphasia, comprehension is largely preserved. Short, closed questions, extra time, gestures and picture or word boards help. Shouting does not help, and constantly finishing sentences can frustrate the patient.
A · Normal protein and enough calories, with a late-evening snack
Protein restriction worsens malnutrition and muscle loss and is no longer routinely advised. Adequate protein and energy, small frequent meals and a late-evening snack are recommended.
B · Ascending cholangitis
Fever, jaundice and right upper quadrant pain suggest infection of an obstructed bile duct. It can rapidly cause sepsis and needs blood cultures, IV antibiotics, fluids and urgent biliary drainage, usually by ERCP.
D · To reduce the risk of aspirating stomach contents
Under general anaesthesia the airway reflexes are lost, so food or fluid in the stomach could be regurgitated and inhaled. Fasting reduces the risk of aspiration pneumonitis.
C · Acute angle-closure glaucoma
Acute angle-closure glaucoma causes a rapid rise in eye pressure, with pain, visual disturbance, haloes and vomiting. It is an emergency that can cause permanent sight loss without urgent treatment.
A · Every few days, until it is stable
When warfarin is started or its dose changes, the INR is checked frequently (often every few days) until it is stable in the target range. After that, the interval can be extended, often up to every 4 weeks or longer if very stable.
B · Hold the isoniazid and report the findings to the prescriber at once
Isoniazid can cause hepatitis, which may be severe. Symptoms such as nausea, loss of appetite and dark urine with a markedly raised ALT mean the drug should be held and the prescriber informed. Patients are taught to avoid alcohol and report these symptoms early.
PaCO₂ = 44.1 mmHg
A · “I will keep my spare insulin in the freezer so it lasts longer.”
Freezing damages insulin, and frozen insulin should not be used. Unopened insulin is kept in the fridge, and heat and expired products are avoided.
A · Right shoulder pain from the gas used during surgery
Gas used to inflate the abdomen irritates the diaphragm and causes referred shoulder pain, which settles in a day or two. Jaundice, fever with worsening pain and signs of shock suggest complications.
