Straight after birth, what is the first priority for a healthy-looking newborn?
An adult choking on food is coughing forcefully and can speak a little. What should the nurse do?
B · Nurse them in separate single rooms and keep them apart
People with CF are colonised with organisms such as Pseudomonas aeruginosa and Burkholderia cepacia complex that can pass between patients and worsen outcomes. They are cared for in single rooms and kept apart, without sharing equipment or sessions.
B · Beta-blockers can mask the tremor and racing heart of a hypo
Beta-blockers blunt adrenergic warning symptoms such as palpitations and tremor. Sweating is often preserved. Patients should check their glucose when they feel unwell, rather than relying on symptoms alone.
C · It increases gut movement against the blockage
Metoclopramide stimulates gastric and intestinal motility, which can worsen pain and risk perforation in complete mechanical obstruction. Other antiemetics are chosen.
C · Acute mesenteric ischaemia
An embolus from AF can block the mesenteric artery, causing bowel ischaemia. Early signs are severe pain with few findings; raised lactate and peritonitis appear as the bowel dies. It needs urgent CT and surgery.
C · Face the patient, speak clearly and reduce background noise
Facing the patient in good light, speaking clearly at a normal pace, checking hearing aids and reducing noise improves communication. Written information can also help.
C · A patient who has just become breathless after starting an IV antibiotic
Breathlessness soon after a new IV antibiotic suggests possible anaphylaxis, an airway and breathing emergency. The infusion should be stopped and the patient assessed immediately.
B · A low ionized calcium
Stored blood contains citrate, which binds calcium. During massive transfusion, citrate can lower ionized calcium, causing tetany, hypotension and arrhythmias. Ionized calcium is monitored and replaced as prescribed.
A · Dry and warm the baby while checking that it is breathing
Breathing and keeping warm come first. Drying stimulates breathing and reduces heat loss; skin-to-skin contact helps. Weighing and routine injections can wait.
C · Encourage the patient to keep coughing and stay with them
A partial obstruction with a forceful cough is best cleared by the patient’s own coughing. The nurse stays and is ready to act if the cough becomes weak or the patient cannot speak or breathe.
