Which newborn finding needs prompt reporting?
A nursing assistant has twice not reported abnormal vital signs to the RN. What is the RN’s best action?
B · Rapid high doses can damage hearing
High plasma levels of loop diuretics can cause tinnitus and hearing loss (ototoxicity), especially with fast injection, renal impairment or other ototoxic drugs such as aminoglycosides. Large IV doses are therefore given slowly.
B · A large difference in blood pressure between the arms
A difference of more than about 20 mmHg in systolic pressure between the arms, a pulse deficit or a new aortic regurgitation murmur suggests dissection. Sweating, tachycardia and rest pain also occur in MI. Thrombolysis and anticoagulants could be disastrous in dissection, so the distinction matters.
D · Ramipril, spironolactone and trimethoprim
ACE inhibitors and spironolactone reduce aldosterone effect, and trimethoprim blocks potassium secretion in the distal tubule. Together, in CKD, they can cause dangerous hyperkalaemia. Loop diuretics, salbutamol and insulin lower potassium.
C · A moist, not wet, wound bed helps cells move and heal faster
A moist environment supports cell migration, autolytic debridement and less pain. Too much moisture causes maceration, so dressings are chosen to balance exudate.
D · Low sodium and high potassium
Without aldosterone, the kidneys lose sodium and retain potassium. Patients may also have low blood pressure, low glucose and increased skin pigmentation.
B · The appendix has perforated, causing peritonitis
Perforation may briefly relieve the pain as pressure in the appendix falls, followed by generalised peritonitis. This needs urgent surgical review, IV fluids and antibiotics.
C · Biotin can distort some immunoassays, such as troponin or TSH
Many immunoassays use biotin-streptavidin binding, and high blood biotin levels can give falsely high or low results, for example a falsely low troponin or abnormal thyroid tests. The laboratory and prescriber should know about biotin use. Troponin Test · MedlinePlus (NIH) (2023); Biotin Interference with Troponin Lab Tests - Assays Subject to Biotin Interference · U.S. FDA (2022)
B · Hold the isoniazid and report the findings to the prescriber at once
Isoniazid can cause hepatitis, which may be severe. Symptoms such as nausea, loss of appetite and dark urine with a markedly raised ALT mean the drug should be held and the prescriber informed. Patients are taught to avoid alcohol and report these symptoms early.
HCO₃⁻ = 21.4 mmol/L
AGc = 25 mmol/L
C · Jaundice that appears within the first 24 hours
Jaundice in the first 24 hours is not physiological and may point to hemolysis or infection. Mild jaundice after the first day, milia and acrocyanosis are common, expected findings.
D · Talk to the assistant privately, restate what must be reported and follow up closely
The RN gives direct, private feedback, restates the reporting limits and increases supervision. If the problem continues, it is escalated to the manager according to policy.
