A patient with severe neutropenia needs a room. Which roommate is the safest choice?
A patient in acute mania paces constantly, has lost weight and will not sit down to eat. Which intervention is most helpful?
D · To avoid delivering the shock during the T wave
A shock delivered on the T wave (the vulnerable period) can trigger ventricular fibrillation. Synchronisation times the shock to the R wave. In ventricular fibrillation there is no organised QRS, so defibrillation is unsynchronised.
B · A sustained waveform on capnography
A sustained exhaled carbon dioxide waveform confirms tracheal placement. Misting and chest movement can be misleading, and skin colour changes late. A chest X-ray shows the depth of the tube but does not prove it is in the trachea.
D · No antibiotics, as long as there are no symptoms
Nearly all long-term catheters become colonised. Treating asymptomatic bacteriuria does not improve outcomes and promotes resistance. Antibiotics are reserved for symptomatic infection.
D · Replace the catheter and take a sample from the new one
Long-term catheters develop a biofilm that contaminates samples. Guidance usually advises replacing the catheter, collecting a sample from the new one and then starting antibiotics. Bag samples are not used.
D · New diarrhoea or abdominal pain
Checkpoint inhibitors can cause immune-mediated colitis, pneumonitis, hepatitis, skin reactions and endocrine problems. Early reporting allows grading and treatment, often with corticosteroids.
C · Rebound hypoglycaemia
Stopping PN suddenly can cause hypoglycaemia because insulin levels stay high for a while. Glucose is monitored, and a glucose infusion may be given according to local policy.
D · Cool it under cool running water for about 20 minutes
Cooling with running water for around 20 minutes limits tissue damage and pain. Ice can cause further injury, and the patient should be kept warm while the burn is cooled.
D · Blood culture bottles
Blood cultures are drawn first to reduce contamination. A standard order of draw then follows (citrate, serum, heparin, EDTA, fluoride), which prevents additives being carried from one tube into the next and distorting results.
C · "I'll take aspirin if I get a headache"
Aspirin irreversibly inhibits platelet function and raises the bleeding risk further in thrombocytopenia, so it and other NSAIDs are avoided unless prescribed. An electric razor, gentle nose blowing and a soft toothbrush reduce minor bleeding. Bleeding and Bruising (Thrombocytopenia) and Cancer Treatment · National Cancer Institute (NIH) (2022); Aspirin · MedlinePlus (NIH) (2025)
Osm = 295 mOsm/kg
海面での肺胞気酸素分圧は PAO₂ = FiO₂·(760 − 47) − PaCO₂/0.8 mmHg で、A–a 勾配は PAO₂ − PaO₂ です。
AGc = 20.75 mmol/L
D · A patient after a knee replacement with no signs of infection
A neutropenic patient cannot fight infection well, so the roommate must not have a transmissible infection. A clean postoperative patient is the safest choice; ideally the neutropenic patient has a single room.
C · Offer high-calorie finger foods and drinks the patient can take while moving
Patients in mania may be too active to sit and eat. Portable high-calorie foods and fluids help meet physiological needs, which are a priority.
