A nurse teaches a patient with COPD pursed-lip breathing. Which statement shows a need for further teaching?
After a head injury, a patient has clear fluid dripping from the nose. Which action should the nurse avoid?
D · No more than half the internal diameter of the tube
A catheter no larger than half the internal diameter of the tube lets air enter around it, reducing negative pressure in the lungs, atelectasis and hypoxaemia. Catheter size is often calculated from the tube size.
B · Acute respiratory acidosis
A low pH with raised PaCO₂ is respiratory acidosis. A normal bicarbonate (about 22–26 mmol/L) shows the kidneys have not yet compensated, so the problem is acute, as in opioid toxicity, exhaustion or an acute COPD exacerbation. Treatment targets ventilation.
D · Hydrocortisone
Starting thyroid replacement first increases metabolism and cortisol clearance and can trigger an adrenal crisis in a patient with low cortisol. Glucocorticoid replacement is started before levothyroxine.
A · An urgent review by a clinician skilled in acute illness
NEWS2 scores respiratory rate, SpO₂, oxygen use, BP, pulse, consciousness or new confusion, and temperature. A score of 5–6 needs an urgent response, and 7 or more an emergency response.
A · Wash the site with soap and running water, without scrubbing
The site is washed with soap and running water; it may be allowed to bleed gently, but it is not scrubbed or sucked. The injury is reported immediately so that post-exposure assessment, including hepatitis B immunity and treatment if needed, can start quickly.
B · Only the patient should press the button
If the patient becomes too sedated, they stop pressing the button, which protects them. When others press it, this safety mechanism is lost and overdose can occur.
B · 5% glucose (D5W)
The glucose in 5% glucose is metabolised, leaving free water that dilutes the plasma sodium further. Postoperative patients often have raised ADH, so large volumes of hypotonic fluid can cause dangerous hyponatraemia.
A · Magnesium
Loop diuretics block the Na-K-2Cl transporter in the loop of Henle, where much magnesium and calcium are also reabsorbed. They therefore commonly cause hypokalemia and hypomagnesemia, and they can raise uric acid and bicarbonate. Magnesium Blood Test · MedlinePlus (NIH) (2023)
Acute Kidney Injury · StatPearls (NCBI Bookshelf) (2026)
HCO₃⁻ = 22.4 mmol/L
B · “I will breathe in through my mouth and out quickly through my nose.”
Pursed-lip breathing means breathing in through the nose and out slowly through pursed lips, making exhalation longer. This keeps airways open and reduces air trapping.
C · Inserting a tube or suction catheter through the nose
Clear nasal drainage after a head injury can be cerebrospinal fluid from a basal skull fracture. Nasal tubes could enter the brain and introduce infection, so they are avoided.
