Which patient with diabetes should the nurse see first?
A victim heavily contaminated with an industrial chemical arrives at the emergency department with stable vital signs. What should happen first?
D · Slowly and deeply while pressing the canister, then hold the breath
With a metered-dose inhaler, the patient breathes out, starts a slow deep breath, presses the canister at the start of it and then holds the breath for about 10 seconds. Coordination is difficult, which is why spacers are often used.
B · Stop suctioning, give oxygen and monitor the patient
Suctioning can cause hypoxaemia and vagal stimulation, leading to bradycardia and arrhythmias. The nurse stops, gives oxygen and observes. Each pass should be short (usually no more than about 10–15 seconds), with pre-oxygenation if needed.
C · Keep taking basal insulin and check glucose and ketones more often
Stress hormones during illness raise glucose and ketones. Stopping insulin can quickly lead to DKA. Patients keep taking basal insulin, test glucose and ketones often, drink fluids and seek help if ketones rise or vomiting persists.
B · Loosen any tight dressing, keep the arm at heart level and escalate
These are early signs of compartment syndrome, where raised pressure in a muscle compartment reduces blood flow. Constrictive dressings are released, the limb is kept at heart level and urgent surgical review is needed, as fasciotomy may be required.
A · Giving prompt, effective analgesia
A sickle cell crisis needs rapid analgesia, often opioids, within a short target time, plus hydration, oxygen if hypoxic, warmth and monitoring for complications. Cold can trigger more sickling.
C · To rule out septic arthritis, which can rapidly destroy the joint
Septic arthritis can destroy cartilage within days. Aspiration allows microscopy and culture, and helps distinguish infection from crystal arthritis such as gout.
C · Blood culture bottles
Blood cultures are drawn first to reduce contamination. A standard order of draw then follows (citrate, serum, heparin, EDTA, fluoride), which prevents additives being carried from one tube into the next and distorting results.
C · Vitamin B12 deficiency
Long-term metformin reduces vitamin B12 absorption in the gut. B12 deficiency causes macrocytic anemia and peripheral neuropathy, which can be mistaken for diabetic neuropathy. Periodic B12 testing is recommended for long-term users. Vitamin B Test · MedlinePlus (NIH) (2026)
AGc = 13.25 mmol/L
A · Glucose 48 mg/dL (2.7 mmol/L), sweating and confused
Severe hypoglycemia with confusion can quickly lead to seizures and loss of consciousness. The other patients have problems that matter but are not immediately dangerous.
C · Decontamination before the victim enters the treatment area
A stable, contaminated victim is decontaminated before entering the department, to protect staff, other patients and the victim. Life-saving care may come first only if the victim is unstable.
