A patient at 36 weeks of pregnancy has sudden severe abdominal pain, a hard, board-like uterus and dark vaginal bleeding. What should the nurse suspect?
A · Keep teeth and gums healthy with regular dental check-ups
Oral bacteria often enter the blood and can settle on prosthetic valves. Good oral hygiene and regular dental care are key. Patients should also report unexplained fever and avoid body piercing or tattoos where possible. Antibiotic prophylaxis policies differ between countries.
A · Whether the patient is taking the medicines as prescribed
Before labelling hypertension as resistant, adherence, measurement technique, white-coat effect and interacting substances (NSAIDs, decongestants, alcohol, liquorice) should be checked. Side effects, cost and complex regimens are common reasons for missed doses.
B · Pause these medicines until eating and drinking again
During dehydrating illness, ACE inhibitors, diuretics and metformin can worsen AKI or its complications. Sick-day guidance usually advises pausing them until the person is eating and drinking normally, and NSAIDs are avoided.
C · The kidneys clear less insulin, so it lasts longer
The kidneys clear a large share of insulin. As kidney function falls, insulin stays in the blood longer and some oral agents build up, so hypos increase. Insulin doses often need to be reduced.
D · Drink plenty of clear fluids while taking the preparation
Bowel preparation causes profuse diarrhoea, which can lead to dehydration and electrolyte disturbance. Patients follow a low-residue diet, then clear fluids, and drink plenty of extra clear fluid.
C · Arterial insufficiency
Rest pain eased by dependency is typical of severe peripheral arterial disease. Elevating the leg worsens pain, and compression is avoided. Vascular assessment is needed.
A · Postural hypotension
A fall in systolic pressure of 20 mmHg or more (here 26 mmHg) on standing, especially with symptoms, indicates postural hypotension. Causes include medicines, dehydration and autonomic problems.
C · Glucagon by injection, or IV dextrose as prescribed
A patient who cannot swallow safely must not receive anything by mouth because of the risk of aspiration. Glucagon (injected or intranasal) or IV dextrose raises the glucose; once awake and able to swallow, the patient is given oral carbohydrate and a snack. Hypoglycemia · MedlinePlus (NIH); Low blood sugar · MedlinePlus (NIH) (2026)
A · Thalassemia trait
Thalassemia trait causes microcytosis from reduced globin production, often with a normal or high red cell count and normal ferritin. Iron deficiency has low ferritin. Iron supplements do not help thalassemia trait and can cause iron overload. Thalassemia - Medical Encyclopedia · MedlinePlus (NIH) (2026); Red Blood Cell (RBC) Indices · MedlinePlus (NIH) (2024)
PaCO₂ = 20 mmHg
B · Placental abruption, which is an emergency
Painful bleeding with a rigid, tender uterus suggests placental abruption, where the placenta separates early. Mother and baby are at risk, so urgent escalation and monitoring are needed.
