A drowsy patient who has just returned from surgery starts to vomit. What should the nurse do first?
Six hours after surgery, a patient has not passed urine, feels the urge to go and has a firm swelling above the pubic bone. What is the most likely problem?
A · It slows AV node conduction and increases contractility
Digoxin inhibits the Na⁺/K⁺-ATPase, raising intracellular calcium and contractility, and increases vagal tone, slowing AV conduction and the ventricular rate. It controls symptoms but does not reduce stroke risk, so anticoagulation is still needed in AF.
A · A patient with heart failure
Pioglitazone can cause fluid retention and oedema, which can worsen heart failure. It is contraindicated in patients with heart failure. Weight gain and an increased fracture risk are other effects.
D · Less pain and a shorter hospital stay
Smaller incisions usually mean less pain, faster mobilisation, fewer wound complications and shorter stays. There are still risks, including injury to organs and shoulder-tip pain from the gas.
B · "I'll let him lie flat so he sleeps better while the feed runs."
The head should stay raised by at least 30–45° during feeding and for a while afterwards. Flushing, checking the tube length and stopping for breathing problems are all correct.
B · Delays can quickly worsen rigidity, immobility and swallowing
Levodopa has a short effect, so missed or late doses can lead to severe stiffness, falls, swallowing problems and, rarely, a dangerous hyperpyrexia-like state. Parkinson's medicines are treated as time-critical.
D · Loss of circulation to the hand as swelling builds under tight burned skin
Circumferential full-thickness burns form a tight eschar that acts like a tourniquet as oedema develops. Neurovascular checks are needed, and escharotomy may be required.
D · Biotin can distort some immunoassays, such as troponin or TSH
Many immunoassays use biotin-streptavidin binding, and high blood biotin levels can give falsely high or low results, for example a falsely low troponin or abnormal thyroid tests. The laboratory and prescriber should know about biotin use.
D · Large-volume IV fluid replacement
In HHS the fluid deficit is often very large, so IV fluid replacement is the first priority; it also lowers the glucose. Insulin follows as prescribed, and potassium, sodium and mental status are monitored closely.
PaCO₂ = 23 mmHg

C · Turn the patient onto their side
A drowsy patient who vomits can aspirate. Turning them to the side lets vomit drain from the mouth; suction and a prescribed antiemetic follow.
B · Urinary retention
An urge to void with a palpable bladder points to retention, not reduced urine production. The nurse confirms with a bladder scan if available and follows the prescribed plan, such as catheterization.
