Which patient is the best choice for the room closest to the nurses’ station?
D · To make both ventricles contract together and more efficiently
In heart failure with a wide QRS (often left bundle branch block), the ventricles contract out of step. Pacing both ventricles restores coordinated contraction, improving symptoms and survival. Many devices also include a defibrillator.
A · Sit the patient up, give oxygen and call for urgent airway help
Post-extubation stridor suggests laryngeal oedema and may progress to complete obstruction. The patient is sat up, given humidified oxygen and assessed by an airway expert urgently. Nebulised adrenaline and steroids may be used, and reintubation may be needed.
B · To stop blood clots from blocking the catheter
After TURP, the raw prostate bed bleeds, and clots can block the catheter and cause retention and bladder spasms. Continuous irrigation through a three-way catheter keeps the drainage flowing.
B · Stop the IV fluids, sit the patient up and escalate
Older patients and those with heart or kidney disease can develop fluid overload. The nurse stops fluids, sits the patient up, gives oxygen if needed and asks for medical review, often for diuretics.
A · Do not stop them suddenly
Long-term steroids suppress the adrenal glands, so stopping suddenly can cause adrenal crisis. Patients should carry a steroid alert card or medical ID and follow sick-day guidance.
C · 0.9% sodium chloride
Only 0.9% sodium chloride is used with red cells. Dextrose solutions can cause hemolysis, and lactated Ringer's contains calcium, which can cause clotting in the citrated blood. Drugs are never added to blood products.
A · IV hydration, a uric acid-lowering drug and frequent electrolytes
Prevention includes aggressive IV hydration, drugs such as allopurinol or rasburicase to lower uric acid, and frequent monitoring of potassium, phosphate, calcium, uric acid and creatinine. Tumor lysis usually begins within 1–3 days of starting treatment. Uric Acid Test · MedlinePlus (NIH) (2026)
HCO₃⁻ = 34.5 mmol/L
PaCO₂ = 53.2 mmHg
B · A confused older adult at high risk of falls
A confused patient at high risk of falls benefits from being close to staff, so they can be observed and helped quickly. The other patients are stable and independent.
