Why is a shock for cardioversion synchronised with the QRS complex?
After six weeks of treatment for tuberculosis, a patient has nausea, dark urine and yellow eyes. What should the nurse do?
Which symptoms are typical of hypokalaemia?
A nurse teaches a carer to give buccal midazolam for prolonged seizures. Which statement shows a need for further teaching?
Why is the time a surgical limb tourniquet is inflated carefully recorded?
After a coughing fit, the external mark on a patient's nasogastric tube has moved 10 cm. Feed is due. What should the nurse do?
What is the most common cause of hypernatraemia in frail older inpatients?
A nurse teaches a patient with a new ileostomy. Which statement shows a need for further teaching?
The nurse teaches a patient who is collecting a 24-hour urine sample at home. Which statement shows a need for further teaching?
PAO₂ = FiO₂·(760 − 47) − PaCO₂/0.8 を使って A–a 勾配 を求めてください。
A nurse teaches a patient about sublingual nitroglycerin tablets. Which statement shows a need for further teaching?
A trauma patient has an open leg fracture, absent breath sounds on one side, a trachea shifted to the other side and a falling blood pressure. Which problem is the priority?
A · To avoid delivering the shock during the T wave
A shock delivered on the T wave (the vulnerable period) can trigger ventricular fibrillation. Synchronisation times the shock to the R wave. In ventricular fibrillation there is no organised QRS, so defibrillation is unsynchronised.
B · Withhold the next doses and report urgently for liver tests
Isoniazid, rifampicin and pyrazinamide can all cause hepatotoxicity. Jaundice, dark urine and nausea suggest drug-induced hepatitis, so the drugs are stopped and liver function is checked urgently. Orange urine from rifampicin does not cause yellow eyes. Alcohol increases the risk.
B · Muscle weakness, cramps and palpitations
Low potassium causes muscle weakness, cramps, constipation and arrhythmias. ECG changes include flattened T waves and U waves. Tingling and twitching suggest low calcium.
D · "I will put something between his teeth so he does not bite his tongue."
Nothing should ever be put between the teeth during a seizure. Buccal midazolam is given between the gum and cheek, so the mouth does not need to be forced open. Timing the seizure and following the emergency care plan are correct.
B · Long inflation can cause nerve and muscle damage
A tourniquet creates a bloodless field but causes limb ischaemia. Inflation time and pressure are recorded and the surgeon is told at set intervals, as long inflation can damage nerves and muscle.
C · Do not use the tube until its position is confirmed by policy
A change in external length means the tube may have moved. Pushing it back blindly is unsafe. It is not used until placement is confirmed by pH or X-ray, or it is replaced.
C · Not drinking enough water
Hypernatraemia usually reflects water deficit, often in patients who are confused, unable to drink independently or losing fluid through fever or diarrhoea. Supporting fluid intake prevents it.
D · "My output will be firm and formed like before."
Ileostomy output is liquid to porridge-like because it bypasses the colon. The other statements describe correct care: good hydration, emptying the bag before it is too heavy and recognising a healthy stoma.
Albumin Blood Test · MedlinePlus (NIH) (2024)
C · "If I forget to save one void, I'll just carry on to the end"
A 24-hour collection is valid only if every void in the period is saved; if a void is lost, the nurse or clinic is told and the collection is usually restarted. Keeping the container cool as instructed, adding the final void at the end time and asking about restrictions are correct. Urine 24-hour volume · MedlinePlus (NIH) (2025)
Osm = 299 mOsm/kg
海面での肺胞気酸素分圧は PAO₂ = FiO₂·(760 − 47) − PaCO₂/0.8 mmHg で、A–a 勾配は PAO₂ − PaO₂ です。
答え: A–a 勾配 = 42.37 mmHg

A · “I will swallow the tablet with a glass of water.”
Sublingual tablets dissolve under the tongue and are absorbed quickly there. Sitting down reduces falls from low blood pressure, the tablets are protected from light and moisture, and headache is a common side effect.
C · The suspected tension pneumothorax
A shifted trachea, absent breath sounds and hypotension suggest tension pneumothorax, which threatens breathing and circulation within minutes. The fracture is important, but it comes after the immediate life threat.
