An older patient who has been constipated for days now has small amounts of liquid stool leaking. What should the nurse suspect?
After a head injury, a patient passes large amounts of very dilute urine, is thirsty and has a rising serum sodium. Which condition is most likely?
A · It can still work, because oxygen collects in the nasopharynx
Studies show that mouth breathing still draws in oxygen from the nasopharynx, so nasal cannulae can work. The effect is judged by the SpO₂, and the device is changed if targets are not reached. Cannulae are never placed in the mouth.
D · Use another reliable method, as rifampicin reduces the pill's effect
Rifampicin induces liver enzymes that break down oestrogen and progestogen, so hormonal contraception may fail. A non-hormonal or unaffected method, such as a copper IUD, is advised during and for a time after treatment. The interaction also affects warfarin and many other drugs.
C · To stop blood clots from blocking the catheter
After TURP, the raw prostate bed bleeds, and clots can block the catheter and cause retention and bladder spasms. Continuous irrigation through a three-way catheter keeps the drainage flowing.
D · Rapid correction can cause brain swelling
In chronic hypernatraemia, brain cells adapt by holding extra solutes. If the sodium falls too quickly, water rushes into the cells and causes cerebral oedema and seizures. Correction is gradual, with regular sodium checks.
D · So that carriers can be decolonised to reduce infection
MRSA carriers have a higher risk of surgical site infection. Screening allows decolonisation with nasal ointment and skin washes, and the right antibiotic prophylaxis.
A · Consider an alternative; if an NSAID is needed, add a PPI
Older age and a previous ulcer greatly increase NSAID-related bleeding. Safer options are considered first; if an NSAID is needed, the lowest effective dose for the shortest time is used with gastroprotection such as a PPI.
C · Sitting fully upright, and staying upright for a time after eating
Sitting upright during and after meals reduces the risk of aspiration. The patient should also be supervised and given the consistency recommended by the speech and language therapist.
D · Low potassium increases the risk of digoxin toxicity
Digoxin binds to the sodium–potassium ATPase at the site potassium normally uses. When potassium is low, digoxin binds more, raising the risk of toxicity and arrhythmias.
B · The dye can interfere with dipstick color readings
Phenazopyridine is a urinary analgesic dye that turns urine orange-red. It can interfere with color-based dipstick tests, so the laboratory should be told. The color is harmless, but it can stain contact lenses and clothing. Urine - abnormal color · MedlinePlus (NIH) (2025)
D · Cortisol is being produced without normal feedback control
Dexamethasone normally suppresses ACTH and lowers morning cortisol. Failure to suppress suggests autonomous cortisol excess (Cushing syndrome). Further tests then find the source, such as a pituitary or adrenal tumor.
A · Fecal impaction with liquid stool passing around it
In fecal impaction, liquid stool from higher in the bowel leaks around the hard mass. Giving an antidiarrheal would make things worse; the nurse assesses and reports.
B · Diabetes insipidus
Damage to the pituitary or hypothalamus can stop the release of antidiuretic hormone, causing diabetes insipidus. The patient loses water, so urine is dilute and sodium rises; SIADH causes the opposite.
