D · “I will take it at breakfast with my calcium tablet.”

Levothyroxine is best absorbed on an empty stomach and is separated from calcium and iron by several hours. It is usually lifelong, so it is not stopped when the patient feels better.

答え

D · “I will take it at breakfast with my calcium tablet.”

A · Report it to the nurse manager according to policy

Suspected drug diversion must be reported through the proper channel, usually the manager. It protects patients and allows the colleague to get help; staying silent is not acceptable.

答え

A · Report it to the nurse manager according to policy

A · An infection may present as confusion in older adults, so assess further and report

In older adults, infections such as urinary tract infection may present with new confusion, falls or reduced appetite rather than fever. Sudden confusion is assessed and reported, not dismissed as aging.

答え

A · An infection may present as confusion in older adults, so assess further and report

C · Glucagon by injection, or IV dextrose as prescribed

A patient who cannot swallow safely must not receive anything by mouth because of the risk of aspiration. Glucagon (injected or intranasal) or IV dextrose raises the glucose; once awake and able to swallow, the patient is given oral carbohydrate and a snack.

答え

C · Glucagon by injection, or IV dextrose as prescribed

D · Nephrotic syndrome

Nephrotic syndrome is heavy proteinuria with low albumin, edema and high lipids. Low albumin reduces oncotic pressure, causing edema, and the liver increases lipoprotein production. There is also a higher risk of clots and infection.

答え

D · Nephrotic syndrome

C · To exclude significant arterial disease, as compression could cut off blood flow

ABPI compares ankle and arm blood pressures. A low result suggests arterial insufficiency, where compression could cause ischaemia; venous ulcers with adequate arterial supply are treated with compression.

答え

C · To exclude significant arterial disease, as compression could cut off blood flow

A · Its active metabolites build up because the kidneys excrete them

Morphine metabolites, including morphine-6-glucuronide, are cleared by the kidneys and accumulate in renal impairment, causing sedation and respiratory depression. Alternative opioids or lower doses are often chosen.

答え

A · Its active metabolites build up because the kidneys excrete them

B · One shock and well: contact the device clinic; repeated shocks or unwell: call emergency services

A single shock with full recovery usually needs a call to the device team. Several shocks in a short time, or a shock with chest pain, breathlessness or collapse, needs emergency help. Patients should not drive themselves after a shock.

答え

B · One shock and well: contact the device clinic; repeated shocks or unwell: call emergency services

A · Keep taking basal insulin and check glucose and ketones more often

Stress hormones during illness raise glucose and ketones. Stopping insulin can quickly lead to DKA. Patients keep taking basal insulin, test glucose and ketones often, drink fluids and seek help if ketones rise or vomiting persists.

答え

A · Keep taking basal insulin and check glucose and ketones more often

B · Respect the decision and support symptom-focused care

A competent patient can refuse dialysis. The nurse makes sure the choice is informed, respects it and supports conservative kidney management, with symptom control, advance care planning and palliative care input.

答え

B · Respect the decision and support symptom-focused care

(Ginkgo biloba)

(Ginkgo biloba)
