- 糖尿病の緊急事態では、DKA(ケトン、アシドーシス)と HHS(非常に高い浸透圧、脱水)を区別し、まず輸液を行います。

D · “I will come and check the pump and the IV site now.”
An alarm may mean an occlusion, air in the line or infiltration. The nurse checks the pump and site; the assistant should not adjust the pump or silence alarms.
D · “I will come and check the pump and the IV site now.”
C · Keep the legs level or slightly lowered, not raised
In arterial disease, gravity helps blood reach the feet, so legs are not elevated. Elevation and compression suit venous disease, and crossing the legs reduces flow further.
C · Keep the legs level or slightly lowered, not raised
B · To reduce skin bacteria and bloodstream infections linked to central lines
Daily chlorhexidine bathing lowers the number of organisms on the skin, reducing central line-associated bloodstream infections and the spread of resistant bacteria in ICUs.
B · To reduce skin bacteria and bloodstream infections linked to central lines
C · An acute bacterial infection
In acute bacterial infection, the bone marrow releases neutrophils before they mature, raising the proportion of bands. Allergies and parasites raise eosinophils, and viral infections more often raise lymphocytes.
C · An acute bacterial infection
Blood Differential · MedlinePlus (NIH) (2024)
A · A very high creatine kinase (CK)
These signs suggest neuroleptic malignant syndrome, a medical emergency. Muscle rigidity releases CK, often to very high levels, and the white count is often raised. Myoglobin can damage the kidneys. The antipsychotic is stopped and supportive care is started.
A · A very high creatine kinase (CK)
A · To reduce the risk of oral thrush
Inhaled steroid deposited in the mouth and throat can cause oral candidiasis and a hoarse voice. Rinsing and using a spacer reduce this.
A · To reduce the risk of oral thrush
A · Pre-renal acute kidney injury from poor perfusion, treated by restoring fluid volume
Fluid loss reduces kidney perfusion, causing pre-renal AKI. Prompt fluid replacement can restore function before the kidney tissue itself is damaged.
A · Pre-renal acute kidney injury from poor perfusion, treated by restoring fluid volume
D · Low oxygen saturation is not explained by anxiety; PE must be excluded
Panic-related hyperventilation causes respiratory alkalosis but normal oxygen saturation. Hypoxaemia with tachycardia in a woman taking oestrogen suggests PE. She needs oxygen, urgent medical assessment and investigation. Paper-bag rebreathing is unsafe and can worsen hypoxaemia.
D · Low oxygen saturation is not explained by anxiety; PE must be excluded
A · Cloudy fluid, many neutrophils, low glucose and high protein
Bacterial meningitis usually gives turbid CSF with raised neutrophils and protein and low glucose. Viral meningitis typically shows clear fluid with lymphocytes and normal glucose.
A · Cloudy fluid, many neutrophils, low glucose and high protein
C · The dawn phenomenon from early-morning hormones
In the dawn phenomenon, cortisol and growth hormone rise in the early morning and push glucose up without a preceding hypo. A night-time hypo followed by rebound hyperglycaemia would suggest the Somogyi effect. The prescriber can adjust the insulin regimen.
C · The dawn phenomenon from early-morning hormones
D · Do not use the tube until its position is confirmed by policy
A change in external length means the tube may have moved. Pushing it back blindly is unsafe. It is not used until placement is confirmed by pH or X-ray, or it is replaced.
D · Do not use the tube until its position is confirmed by policy
