- 糖尿病の緊急事態では、DKA(ケトン、アシドーシス)と HHS(非常に高い浸透圧、脱水)を区別し、まず輸液を行います。

A · “Check his glucose and give the insulin according to the sliding scale.”
Giving insulin is medication administration and needs a licensed nurse. Measuring glucose, hygiene and supervised walking are suitable tasks with clear reporting instructions.
A · “Check his glucose and give the insulin according to the sliding scale.”
D · Whether the patient has any metal implants or devices, such as a pacemaker
The MRI magnet can move or heat metal objects and can stop some implanted devices from working. Patients are screened for implants, pacemakers, metal fragments and metal items such as jewelry before entering the scanner room.
D · Whether the patient has any metal implants or devices, such as a pacemaker
D · At-risk behavior, managed by coaching and fixing the system pressures behind it
At-risk behavior is a choice whose risk is not recognized or is believed to be justified, often because shortcuts have become normal. It is managed by coaching and removing the pressures behind it, while reckless behavior may need disciplinary action.
D · At-risk behavior, managed by coaching and fixing the system pressures behind it
C · The blood-to-anticoagulant ratio must be right for accurate clotting times
Citrate tubes contain a set volume of anticoagulant. If the tube is underfilled, the extra citrate falsely prolongs the PT and aPTT, so the laboratory may reject the sample.
C · The blood-to-anticoagulant ratio must be right for accurate clotting times
B · Sodium 119 mEq/L in a patient with new confusion and vomiting
Severe hyponatremia with confusion and vomiting suggests cerebral edema and can progress to seizures and coma. The other results are mildly abnormal and expected for those conditions, so they are less urgent. Low blood sodium - Medical Encyclopedia · MedlinePlus (NIH) (2025); Sodium Blood Test · MedlinePlus (NIH) (2024)
B · Sodium 119 mEq/L in a patient with new confusion and vomiting
A · Patients can be seriously injured climbing over or becoming trapped in rails
Bed rails can cause entrapment or a fall from a greater height. Their use is based on an individual risk assessment; alternatives include a low bed, crash mats and closer observation.
A · Patients can be seriously injured climbing over or becoming trapped in rails
C · As soon as IV or IO access is obtained, then every 3–5 minutes
In non-shockable rhythms, adrenaline is given as soon as possible and repeated every 3–5 minutes while CPR continues. In shockable rhythms it is given after the third shock in UK/European guidelines (after the second shock in AHA guidelines).
C · As soon as IV or IO access is obtained, then every 3–5 minutes
C · Signs of bleeding, such as haematuria and a rising pulse
Bleeding is the main complication of kidney biopsy. The patient usually has bed rest for some hours, with frequent BP and pulse checks and inspection of urine for haematuria. Back pain or a falling BP is reported at once.
C · Signs of bleeding, such as haematuria and a rising pulse
B · Treat it as very high, check ketones and escalate
"HI" means the glucose is above the meter's upper range. In a drowsy patient this may be DKA or HHS. A laboratory glucose, ketones, observations and urgent medical review are needed.
B · Treat it as very high, check ketones and escalate
A · It causes dry mouth and raises the risk of decay and jaw damage
Head and neck radiotherapy damages salivary glands, causing xerostomia and dental decay, and can cause osteoradionecrosis of the jaw after extractions. Dental treatment is done first, with careful oral hygiene and fluoride.
A · It causes dry mouth and raises the risk of decay and jaw damage
D · Treat it as a possible burn, report it and complete an incident form
A blister under the return electrode suggests a diathermy burn. The nurse assesses and dresses it, informs the team, and reports it as an incident so that the cause can be investigated.
D · Treat it as a possible burn, report it and complete an incident form
