D · It suggests tissues are short of oxygen and using anaerobic metabolism
When tissues are poorly perfused, cells switch to anaerobic metabolism and produce lactate. A raised lactate is a marker of tissue hypoperfusion and severity in sepsis and shock. Repeated measurements show whether treatment is working.
답
D · It suggests tissues are short of oxygen and using anaerobic metabolism
C · High-concentration oxygen via a non-rebreather mask
Carbon monoxide binds haemoglobin far more strongly than oxygen, causing tissue hypoxia, while pulse oximetry can look normal. High-concentration oxygen speeds CO removal. Carboxyhaemoglobin is measured on a blood gas, and others in the home may also be at risk.
답
C · High-concentration oxygen via a non-rebreather mask
C · Potassium
Spironolactone is an aldosterone antagonist. It retains potassium, so hyperkalaemia can develop, especially with kidney disease or ACE inhibitors. Breast enlargement is another known side effect.
답
C · Potassium
D · Rapid correction can cause brain swelling
In chronic hypernatraemia, brain cells adapt by holding extra solutes. If the sodium falls too quickly, water rushes into the cells and causes cerebral oedema and seizures. Correction is gradual, with regular sodium checks.
답
D · Rapid correction can cause brain swelling
C · It increases gut movement against the blockage
Metoclopramide stimulates gastric and intestinal motility, which can worsen pain and risk perforation in complete mechanical obstruction. Other antiemetics are chosen.
답
C · It increases gut movement against the blockage
B · Anal fissure
An anal fissure causes severe pain during and after defecation with bright red blood. Treatment includes stool softeners, fibre and topical treatment to relax the sphincter.
답
B · Anal fissure
답: C · Urea (blood urea nitrogen)
Blood proteins digested in the upper GI tract are absorbed and converted to urea, so urea rises out of proportion to creatinine. Urea is part of the Glasgow-Blatchford bleeding score.
답
C · Urea (blood urea nitrogen)
D · Placed on ice and sent to the laboratory at once
Ammonia can rise in a standing sample as red cells and proteins break down. The sample is usually placed on ice and analyzed promptly to avoid a falsely high result.
답
D · Placed on ice and sent to the laboratory at once
답: A · 위산 손실.
Alkalosis · StatPearls (NCBI Bookshelf) (2024)
답
A · 위산 손실.
PaCO₂ 42 mmHg:
HCO₃⁻ 30 mmol/L:

답: A–a 기울기 = 49.98 mmHg
해수면에서 폐포 산소 분압은 PAO₂ = FiO₂·(760 − 47) − PaCO₂/0.8 mmHg이고, A–a 기울기는 PAO₂ − PaO₂입니다.

(Wenckebach)
