1Formula dan kaedah

Pemberian keutamaan & delegasi

Kaedah

Kawalan jangkitan & keselamatan pesakit

Kaedah

  2. Tambah langkah berjaga-jaga mengikut laluan: sentuhan (gaun, sarung tangan), titisan (pelitup muka pembedahan), bawaan udara (N95, bilik tekanan negatif).
  3. Pastikan item steril kekal steril: jika ragu-ragu, ia tidak steril; gantikan, jangan sekali-kali dilap.

Perubatan-pembedahan: neurologi, endokrin & renal

Kaedah

1.Pemberian keutamaan & delegasi

Jawapan: C · Dressing soaked with bright red blood and a heart rate rising to 118/min

Mengapa ini jawapannya

Heavy fresh bleeding with a rising heart rate suggests hemorrhage and early shock. The other patients have routine needs that can wait.

Jawapan

C · Dressing soaked with bright red blood and a heart rate rising to 118/min
2.Pemberian keutamaan & delegasi

Jawapan: C · Diabetes insipidus

Mengapa ini jawapannya

Damage to the pituitary or hypothalamus can stop the release of antidiuretic hormone, causing diabetes insipidus. The patient loses water, so urine is dilute and sodium rises; SIADH causes the opposite.

Jawapan

C · Diabetes insipidus
3.Kawalan jangkitan & keselamatan pesakit

Jawapan: D · Secured upright in a stand or holder

Mengapa ini jawapannya

Oxygen cylinders are under high pressure and can be damaged or cause injury if they fall. They are kept upright in a holder or cart, away from heat and flames.

Jawapan

D · Secured upright in a stand or holder
4.Kawalan jangkitan & keselamatan pesakit

Jawapan: B · “I am concerned, I am uncomfortable, this is a safety issue.”

Mengapa ini jawapannya

CUS words (Concerned, Uncomfortable, Safety issue) are agreed language that signals the need to stop and listen. Maximal barrier precautions are part of the central line bundle, so the concern is valid.

Jawapan

B · “I am concerned, I am uncomfortable, this is a safety issue.”
5.Nilai makmal & elektrolit

Jawapan: B · Metformin

Mengapa ini jawapannya

Metformin is cleared by the kidneys, and it is contraindicated when the eGFR is below 30 mL/min/1.73 m² because of the risk of lactic acidosis. Insulin doses may also need adjustment, but insulin is not contraindicated.

Jawapan

B · Metformin
6.Nilai makmal & elektrolit

Jawapan: C · Rifampin speeds up warfarin metabolism

Mengapa ini jawapannya

Rifampin induces liver enzymes that break down warfarin, so warfarin's effect falls and the INR drops, increasing clot risk. The INR is monitored closely and the dose adjusted; the reverse happens when rifampin is stopped.

Jawapan

C · Rifampin speeds up warfarin metabolism
7.Kejururawatan klinikal

Jawapan: D · Respiratory alkalosis from a low PaCO2

Mengapa ini jawapannya

Hyperventilation blows off CO2, raising the pH. Symptoms can include tingling of the lips and fingers and lightheadedness.

Jawapan

D · Respiratory alkalosis from a low PaCO2
8.Kejururawatan klinikal

Jawapan: B · Respect the refusal, check it is informed and document it, and inform the team

Mengapa ini jawapannya

An adult with capacity has the right to refuse treatment, even life-saving treatment. The team should make sure the patient understands the consequences, discuss alternatives such as cell salvage and record the decision.

Jawapan

B · Respect the refusal, check it is informed and document it, and inform the team
9.Perubatan-pembedahan: kardiovaskular & respiratori

Jawapan: D · Phaeochromocytoma

Mengapa ini jawapannya

Phaeochromocytoma is a catecholamine-secreting tumour, usually of the adrenal medulla. Classic features are paroxysmal headache, sweating, palpitations and hypertension. It is diagnosed with plasma or urine metanephrines.

Jawapan

D · Phaeochromocytoma
10.Perubatan-pembedahan: kardiovaskular & respiratori

Jawapan: C · Low oxygen saturation is not explained by anxiety; PE must be excluded

Mengapa ini jawapannya

Panic-related hyperventilation causes respiratory alkalosis but normal oxygen saturation. Hypoxaemia with tachycardia in a woman taking oestrogen suggests PE. She needs oxygen, urgent medical assessment and investigation. Paper-bag rebreathing is unsafe and can worsen hypoxaemia.

Jawapan

C · Low oxygen saturation is not explained by anxiety; PE must be excluded
11.Perubatan-pembedahan: neurologi, endokrin & renal

Jawapan: A · SSRI antidepressants

Mengapa ini jawapannya

SSRIs such as sertraline and citalopram can cause SIADH, particularly in older adults. Carbamazepine, some antipsychotics and some chemotherapy drugs are other causes. Sodium is checked if symptoms appear.

Jawapan

A · SSRI antidepressants
12.Perubatan-pembedahan: neurologi, endokrin & renal

Jawapan: A · The other adrenal gland has been suppressed

Mengapa ini jawapannya

Excess cortisol from the adenoma suppressed ACTH, so the remaining adrenal gland is shrunken and underactive. Steroid replacement is needed until it recovers, which can take months. Stopping too early risks adrenal crisis.

Jawapan

A · The other adrenal gland has been suppressed
13.Perubatan-pembedahan: GI, perioperatif & onkologi

Jawapan: A · If sterility is in doubt, the item is treated as contaminated

Mengapa ini jawapannya

Sterile technique treats any doubtful item as contaminated. A gown is sterile only from chest to waist in front, and wet packaging can let microorganisms through.

Jawapan

A · If sterility is in doubt, the item is treated as contaminated
14.Perubatan-pembedahan: GI, perioperatif & onkologi

Jawapan: C · Deceptive placebos are unethical and do not show pain is "fake"

Mengapa ini jawapannya

A placebo response does not mean pain is not real, and giving placebos without consent is deceptive and damages trust. Pain is assessed with the patient and treated according to the plan.

Jawapan

C · Deceptive placebos are unethical and do not show pain is "fake"
