1Formules en methoden

Medisch-chirurgisch: cardiovasculair en respiratoir

Methode

Normaalwaarden
pH 7,35–7,45, PaCO₂ 35–45 mmHg (4,7–6,0 kPa), HCO₃⁻ 22–26 mmol/L
ROME
Respiratoir: PaCO₂ tegengesteld aan pH; Metabool: HCO₃⁻ gelijk aan pH
Formule van Winter
PaCO₂ = 1,5·HCO₃⁻ + 8 ± 2
Metabole alkalose
PaCO₂ ≈ 0,7·HCO₃⁻ + 21
Anion gap
AG = Na⁺ − (Cl⁻ + HCO₃⁻) ≈ 8–12
Voor albumine gecorrigeerde gap
AG + 2,5·(4,0 − alb g/dL) = AG + 0,25·(40 − alb g/L)
Deltaratio
(AG − 12)/(24 − HCO₃⁻)
P/F-ratio
PaO₂/FiO₂
Alveolaire gasvergelijking
PAO₂ = FiO₂·(760 − 47) − PaCO₂/0,8
Serumosmolaliteit
2·Na⁺ + glucose + urea (2·Na⁺ + glucose/18 + BUN/2,8)

Methode

Papier (25 mm/s, 10 mm/mV)

Hartfrequentie, onregelmatig ritme
QRS-complexen in 6 s × 10
Normaal PR-interval
0,12–0,20 s
Normale QRS-duur
< 0.12 s
QTc (Bazett)
QT/√RR
QTc (Fridericia)
QT/∛RR
QTc (Framingham)
QT + 154·(1 − RR)
Sinusfrequenties
bradycardie < 60, normaal 60–100, tachycardie > 100 /min
Ontsnappingsfrequenties
junctioneel 40–60, ventriculair 20–40 /min
Snelle ritmes
SVT 150–250 /min (smal), VT > 100 /min (breed)

Methode

3Oplossingen

1.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: B · Take the best of three blows, ideally morning and evening

Waarom dit het antwoord is

The patient stands or sits upright, takes a full breath and blows as hard and fast as possible, three times, recording the best. Regular readings show variability and guide the action plan, which uses the patient's own best value as the reference.

Antwoord

B · Take the best of three blows, ideally morning and evening
2.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: D · Low oxygen saturation is not explained by anxiety; PE must be excluded

Waarom dit het antwoord is

Panic-related hyperventilation causes respiratory alkalosis but normal oxygen saturation. Hypoxaemia with tachycardia in a woman taking oestrogen suggests PE. She needs oxygen, urgent medical assessment and investigation. Paper-bag rebreathing is unsafe and can worsen hypoxaemia.

Antwoord

D · Low oxygen saturation is not explained by anxiety; PE must be excluded
3.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: D · Chronic subdural haematoma

Waarom dit het antwoord is

Older people have brain atrophy that stretches the bridging veins, and anticoagulants increase bleeding. A slow venous bleed can cause gradual confusion, headache or weakness over weeks. A CT scan is needed.

Antwoord

D · Chronic subdural haematoma
4.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: D · An acute Charcot foot, needing urgent offloading

Waarom dit het antwoord is

Acute Charcot neuroarthropathy causes inflammation and destruction of bones and joints in a foot without protective sensation. It can look like cellulitis. Early immobilisation and offloading prevent permanent deformity.

Antwoord

D · An acute Charcot foot, needing urgent offloading
5.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: D · They may increase the risk of perforation

Waarom dit het antwoord is

Stimulating peristalsis can raise pressure on an inflamed appendix and may lead to rupture and peritonitis. The patient is kept nil by mouth while awaiting surgical assessment.

Antwoord

D · They may increase the risk of perforation
6.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: A · Hypoglycaemia if insulin continues without the feed

Waarom dit het antwoord is

When feed stops but insulin continues, blood glucose can fall sharply. The insulin plan is reviewed with the team, glucose is checked frequently, and a glucose infusion may be needed.

Antwoord

A · Hypoglycaemia if insulin continues without the feed
7.Klinische verpleegkunde

Antwoord: D · Lying flat, with the legs raised

Waarom dit het antwoord is

Lying flat with the legs raised supports circulation in anaphylactic shock. Patients with breathing difficulty may prefer to sit up, but should not stand or walk, as this can cause collapse.

Antwoord

D · Lying flat, with the legs raised
8.Klinische verpleegkunde

Antwoord: A · Oxygen is given if the SpO2 is below the target range

Waarom dit het antwoord is

Current guidance recommends oxygen in ACS only for hypoxaemia, as routine oxygen in patients with normal saturations has not been shown to help. Urgent ECG, analgesia and antiplatelet treatment remain priorities.

Antwoord

A · Oxygen is given if the SpO2 is below the target range
9.Laboratoriumwaarden en elektrolyten

Antwoord: C · Prick the side of the fingertip

Waarom dit het antwoord is

The sides of the fingertips are less sensitive than the pads, so they are used for capillary sampling. Rotating sites reduces soreness, and hard squeezing can add tissue fluid that gives inaccurate results.

Antwoord

C · Prick the side of the fingertip
10.Laboratoriumwaarden en elektrolyten

Antwoord: C · Cushing syndrome

Waarom dit het antwoord is

Excess cortisol raises glucose, causes central obesity, a round face and purple striae, and has some mineralocorticoid effect that lowers potassium. Adrenal insufficiency causes low glucose and high potassium.Gecontroleerd met: Hypercortisolism (Cushing Syndrome) · StatPearls (NCBI Bookshelf) (2025); Hypokalemia · StatPearls (NCBI Bookshelf) (2025)

Antwoord

C · Cushing syndrome
11.Interpretatie van arteriële bloedgassen

Antwoord: HCO₃⁻ = 26,7 mmol/L

Verandering in PaCO₂

|67 − 40| = 27 mmHg → 2,7 × 10 mmHg

Verwachte HCO₃⁻

HCO₃⁻ = 24 + 1·2,7 = 26,7 mmol/L

Antwoord

HCO₃⁻ = 26,7 mmol/L
12.Interpretatie van arteriële bloedgassen

Antwoord: A–a-gradiënt = 237 mmHg

Alveolaire zuurstof

PAO₂ = 0,5·713 − 30/0,8 = 356,5 − 37,5 = 319 mmHg

Gradiënt

A–a = 319 − 82 = 237 mmHg

Antwoord

A–a-gradiënt = 237 mmHg
13.Interpretatie van ECG-ritmestrips

Antwoord: eerstegraads AV-blok

Vijf stappen

1. Frequentie: R–R = 27 kleine hokjes, 1500 ÷ 27 ≈ 56 per minuut.
2. Ritme: regelmatig, elke R–R is 1,08 s.
3. P-toppen: positief, één vóór elke QRS, allemaal gelijk.
4. PR-interval: 0,32 s, constant, langer dan 0,20 s.
5. QRS: 0,08 s, smal (onder 0,12 s).

Antwoord: A · Stop the heparin infusion and inform the provider

Waarom dit het antwoord is

Black, tarry stools suggest upper GI bleeding, and a high aPTT shows too much anticoagulation. The nurse stops the heparin, checks vital signs and informs the provider, who may prescribe protamine sulfate. Vitamin K reverses warfarin, not heparin.

Antwoord

A · Stop the heparin infusion and inform the provider
