1Formules en methoden

Infectiepreventie en patiëntveiligheid

Methode

Metrische stappen
1 kg = 1000 g, 1 g = 1000 mg, 1 mg = 1000 mcg, 1 L = 1000 mL
Gewicht en lengte
1 kg = 2,2 lb, 1 lb = 16 oz, 1 in = 2,54 cm, 1 ft = 12 in
Huishoudelijke maten
1 tsp = 5 mL, 1 tbsp = 15 mL, 1 fl oz = 30 mL, 1 cup = 8 fl oz = 240 mL
Temperatuur
°C = (°F − 32)·5/9, °F = °C·9/5 + 32
24-uursklok
PM-uren 1–11: tel er 12 bij op; 12 AM = 00:xx; 12 PM = 12:xx
Dosissnelheid
mcg/min × 60/1000 = mg/h
Laboratoriumfactoren
glucose 1 mmol/L = 18 mg/dL; creatinine 1 mg/dL = 88,4 µmol/L; Hb 1 g/dL = 10 g/L

Methode

Dosisberekeningen

Dosis uit de concentratie
volume = dosis / concentratie
Dosis per kilogram lichaamsgewicht
dosis = mg/kg × lichaamsgewicht
Tabletten
aantal = dosis / sterkte van een tablet
Eenheden
1 g = 1000 mg, 1 mg = 1000 mcg

Methode

Infuussnelheden

Pompsnelheid
mL/h = volume (mL)/time (h)
Druppelsnelheid
drops/min = volume (mL) × drop factor / time (min)
Inlooptijd
time (h) = volume (mL)/rate (mL/h)

Methode

4 mL/kg (eerste 10 kg) + 2 mL/kg (volgende 10 kg) + 1 mL/kg (elke kg boven 20)
Holliday–Segar per dag
100 / 50 / 20 mL/kg voor dezelfde gewichtsklassen
Vochtbalans
intake − output
IJsschilfers
tellen voor de helft van hun volume
Urineproductie
mL/kg/h = mL / (kg × h)
Inlooptijd
tijd (h) = resterend volume (mL) / snelheid (mL/h)
Toegediend volume
mL = snelheid × tijd

3Oplossingen

1.Basisverpleegkunde

Antwoord: D · Hemostase, ontsteking, proliferatie, maturatie

Antwoord

D · Hemostase, ontsteking, proliferatie, maturatie
2.Basisverpleegkunde

Antwoord: A · Three deep-cough specimens collected 8 to 24 hours apart, at least one early in the morning

Waarom dit het antwoord is

Several deep-cough sputum samples improve the chance of finding acid-fast bacilli, and early-morning samples contain the most secretions. Saliva is not sputum, and antiseptic mouthwash can affect the result.

Antwoord

A · Three deep-cough specimens collected 8 to 24 hours apart, at least one early in the morning
4.Infectiepreventie en patiëntveiligheid

Antwoord: C · Hold the drug and contact the prescriber

Waarom dit het antwoord is

Amoxicillin is a penicillin, so it could cause a serious allergic reaction. The nurse holds the drug, clarifies the allergy and contacts the prescriber for a safe alternative.

Antwoord

C · Hold the drug and contact the prescriber
5.Prioriteren en delegeren

Antwoord: A · Encourage deep breathing, coughing and use of the incentive spirometer

Waarom dit het antwoord is

Reduced basal breath sounds and a mild temperature early after surgery often mean atelectasis. Deep breathing, coughing, incentive spirometry and early mobilization help re-expand the lungs.

Antwoord

A · Encourage deep breathing, coughing and use of the incentive spirometer
6.Prioriteren en delegeren

Antwoord: A · Giving a scheduled subcutaneous anticoagulant injection to a stable patient

Waarom dit het antwoord is

LPNs/LVNs usually give scheduled injections to stable patients. A fresh postoperative assessment and teaching plans need the RN, and routine vital signs are better given to the assistant so the LPN/LVN can use their skills.

Antwoord

A · Giving a scheduled subcutaneous anticoagulant injection to a stable patient
7.Klinische verpleegkunde

Antwoord: A · Avoid large meals late in the evening and raise the head of the bed

Waarom dit het antwoord is

Smaller meals, not eating close to bedtime, raising the head of the bed, weight loss and stopping smoking reduce reflux. Lying flat after eating makes it worse.

Antwoord

A · Avoid large meals late in the evening and raise the head of the bed
8.Klinische verpleegkunde

Antwoord: D · That the catheter and tubing are not kinked or blocked

Waarom dit het antwoord is

A sudden fall in catheter output may be due to a kinked, clamped or blocked catheter. If it is draining freely, the nurse assesses fluid status and vital signs and escalates possible hypovolaemia or AKI.

Antwoord

D · That the catheter and tubing are not kinked or blocked
9.Laboratoriumwaarden en elektrolyten

Antwoord: A · Take it with food and a full glass of water, and do not crush extended-release tablets

Waarom dit het antwoord is

Oral potassium can irritate the esophagus and stomach, so it is taken with food and plenty of fluid, and extended-release forms are swallowed whole. Most salt substitutes contain potassium chloride and can add to the risk of hyperkalemia.

Antwoord

A · Take it with food and a full glass of water, and do not crush extended-release tablets
10.Laboratoriumwaarden en elektrolyten

Antwoord: B · Hold the clozapine and report the result at once

Waarom dit het antwoord is

An ANC below 500/µL is severe neutropenia, and with fever and sore throat it suggests a serious infection. Clozapine is held, the provider is told immediately, and infection is assessed and treated. A nurse never changes a dose without a prescription.

Antwoord

B · Hold the clozapine and report the result at once
11.Farmacologie

Antwoord: A · Activated partial thromboplastin time (aPTT)

Waarom dit het antwoord is

Unfractionated heparin is often monitored with the aPTT, and the infusion rate is adjusted according to a protocol. Low-molecular-weight heparins usually do not need routine aPTT checks.

Antwoord

A · Activated partial thromboplastin time (aPTT)
12.Farmacologie

Antwoord: C · Rebound hypertension from abrupt clonidine withdrawal

Waarom dit het antwoord is

Clonidine is a central alpha-2 agonist. Stopping it suddenly lets sympathetic activity surge, causing rebound hypertension, so it is tapered gradually.

Antwoord

C · Rebound hypertension from abrupt clonidine withdrawal
13.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: C · Vasospastic (Prinzmetal) angina

Waarom dit het antwoord is

Vasospastic angina is caused by temporary coronary artery spasm, often at rest or at night, with transient ST elevation. It responds to calcium channel blockers and nitrates. Triggers include smoking, cold and cocaine.

Antwoord

C · Vasospastic (Prinzmetal) angina
14.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: A · Continue CPR and give adrenaline (epinephrine), as the rhythm is non-shockable

Waarom dit het antwoord is

Pulseless electrical activity is a non-shockable rhythm. Treatment is high-quality CPR, adrenaline as soon as possible and repeated, and a search for reversible causes (the 4 Hs and 4 Ts, such as hypoxia, hypovolaemia, tension pneumothorax and tamponade). Shocks are only for VF and pulseless VT.

Antwoord

A · Continue CPR and give adrenaline (epinephrine), as the rhythm is non-shockable
15.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: B · After about 6–8 weeks

Waarom dit het antwoord is

Levothyroxine has a half-life of about a week, so it takes several weeks to reach a new steady state. TSH is usually rechecked about 6–8 weeks after a dose change.

Antwoord

B · After about 6–8 weeks
16.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: D · Emptying the drainage bag and recording the volume

Waarom dit het antwoord is

Emptying and recording urine volume is routine and can be delegated to trained staff. Decisions about removal, assessment of a possible blockage and first-time teaching need the registered nurse.

Antwoord

D · Emptying the drainage bag and recording the volume
17.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: C · Palliative radiotherapy to the area

Waarom dit het antwoord is

A single fraction or short course of radiotherapy relieves pain from bone metastases in many patients. Analgesia, including NSAIDs where suitable, and bone-targeted drugs are also used.

Antwoord

C · Palliative radiotherapy to the area
18.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: B · Radiation oesophagitis; analgesia, soft food and nutrition support

Waarom dit het antwoord is

Radiotherapy to the chest can inflame the oesophagus, causing painful swallowing. Analgesia (sometimes before meals), soft moist food, nutritional supplements, dietitian input and monitoring weight are important.

Antwoord

B · Radiation oesophagitis; analgesia, soft food and nutrition support
19.Verloskundige verpleegkunde

Antwoord: A · A port-wine stain does not fade with time

Waarom dit het antwoord is

Stork bites on the neck or eyelids are flat pink capillary marks that usually fade. A port-wine stain is a permanent vascular malformation; on the face it may be linked with Sturge–Weber syndrome.

Antwoord

A · A port-wine stain does not fade with time
20.Verloskundige verpleegkunde

Antwoord: C · Hirschsprung disease

Waarom dit het antwoord is

Delayed meconium, abdominal distension and bilious vomiting suggest bowel obstruction, such as Hirschsprung disease. Bilious vomiting in a newborn is always an emergency.

Antwoord

C · Hirschsprung disease
21.Kinderverpleegkunde

Antwoord: B · Blowing bubbles or looking at a pop-up book

Waarom dit het antwoord is

Distraction that suits the child's age, such as bubbles, books, toys or videos, reduces pain and distress. Blowing bubbles also encourages slow, deep breathing. Parents are encouraged to stay and help.

Antwoord

B · Blowing bubbles or looking at a pop-up book
22.Kinderverpleegkunde

Antwoord: B · Surfactant given into the trachea

Waarom dit het antwoord is

Respiratory distress syndrome in preterm babies is caused by surfactant deficiency. Exogenous surfactant given via the airway, with respiratory support such as CPAP, improves lung compliance. Antibiotics are often started too, because infection can look similar.

Antwoord

B · Surfactant given into the trachea
23.Psychiatrische verpleegkunde

Antwoord: D · Tangentialiteit

Antwoord

D · Tangentialiteit
24.Psychiatrische verpleegkunde

Antwoord: D · A neurocognitive disorder such as Alzheimer's disease

Waarom dit het antwoord is

Gradual, progressive memory loss with impaired daily functioning and normal alertness suggests dementia, most commonly Alzheimer's disease. Delirium is acute and fluctuating with impaired attention.

Antwoord

D · A neurocognitive disorder such as Alzheimer's disease
25.Verpleegkundig rekenen en eenhedenomrekeningen

Antwoord: h = 67,3 in

h = 171/2,54 = 67,32 ≈ 67,3 in

Antwoord

h = 67,3 in
26.Verpleegkundig rekenen en eenhedenomrekeningen

Antwoord: t = 130 min

23:10 → 24:00 = 50 min

Na middernacht

00:00 → 01:20 = 80 min

Totaal

t = 50 + 80 = 130 min

Antwoord

t = 130 min
27.Dosisberekeningen

Antwoord: D = 240 mg

Dosis

20·12 = 240 mg

Antwoord

D = 240 mg
28.Dosisberekeningen

Antwoord: V = 0,95 mL

Dezelfde eenheden

2 mg/mL = 2000 mcg/mL

Volume

V = 1900/2000 = 0,95 mL

Antwoord

V = 0,95 mL
29.Infuussnelheden

Antwoord: V = 420 mL

Infusion rates: a pump is set in mL/h = volume/time in hours. A gravity drip is counted in drops per minute: gtt/min = volume (mL) × drop factor (gtt/mL)/time (min). The drop factor is printed on the tubing package: macrodrip sets deliver 10, 15 or 20 gtt/mL (blood sets often 10 gtt/mL in the US, 15 in the UK) and microdrip sets 60 gtt/mL. Always use the drop factor the question or package gives.

Snelheid in mL/h

35·60/20 = 105 mL/h

Volume

V = 105·4 = 420 mL

Antwoord

V = 420 mL
30.Infuussnelheden

Antwoord: t = 23,4 h

Snelheid in mL/h

32·60/60 = 32 mL/h

Resterende tijd

t = 750/32 = 23,44 ≈ 23,4 h

Antwoord

t = 23,4 h
31.Infuusvloeistoffen en vochtbalans

Antwoord: I = 1250 mL

oraal: 150 + 120 + 240 = 510 mL
ijsschilfers: 100/2 = 50 mL
Infuus: 80·8 = 640 mL
intraveneus antibioticum: 1·50 = 50 mL
I = 1250 mL

Antwoord

I = 1250 mL
32.Infuusvloeistoffen en vochtbalans

Antwoord: t = 4,8 h

Eerste deel

120·3 = 360 mL
500 − 360 = 140 mL over

Tweede deel

140/80 = 1,75 h

Totale tijd

t = 3 + 1,75 = 4,75 ≈ 4,8 h

Antwoord

t = 4,8 h

Score elke subschaal

sensorische waarneming: zeer beperkt → 2
vochtigheid: zeer vochtig → 2
activiteit: loopt regelmatig → 4
mobiliteit: licht beperkt → 3
voeding: uitstekend → 4
wrijving en schuifkrachten: geen zichtbaar probleem → 3

Optellen

Totaal Braden = 2 + 2 + 4 + 3 + 4 + 3 = 18

Antwoord

B = 18
34.Klinische beoordelingsscores

Antwoord: laag-matig

Score elke parameter

RR 14/min → 0
SpO₂ 99% → 0
op kamerlucht → 0
SBP 124 mmHg → 0
HR 40/min → 3
alert (A) → 0
T 37,2 °C → 0

Totaal en rode scores

NEWS2 = 0 + 0 + 0 + 0 + 3 + 0 + 0 = 3
Ten minste één parameter scoort 3.

Risicogroep

laag-matig

Antwoord

laag-matig
