1Formules en methoden

Infectiepreventie en patiëntveiligheid

Methode

Metrische stappen
1 kg = 1000 g, 1 g = 1000 mg, 1 mg = 1000 mcg, 1 L = 1000 mL
Gewicht en lengte
1 kg = 2,2 lb, 1 lb = 16 oz, 1 in = 2,54 cm, 1 ft = 12 in
Huishoudelijke maten
1 tsp = 5 mL, 1 tbsp = 15 mL, 1 fl oz = 30 mL, 1 cup = 8 fl oz = 240 mL
Temperatuur
°C = (°F − 32)·5/9, °F = °C·9/5 + 32
24-uursklok
PM-uren 1–11: tel er 12 bij op; 12 AM = 00:xx; 12 PM = 12:xx
Dosissnelheid
mcg/min × 60/1000 = mg/h
Laboratoriumfactoren
glucose 1 mmol/L = 18 mg/dL; creatinine 1 mg/dL = 88,4 µmol/L; Hb 1 g/dL = 10 g/L

Methode

Dosisberekeningen

Dosis uit de concentratie
volume = dosis / concentratie
Dosis per kilogram lichaamsgewicht
dosis = mg/kg × lichaamsgewicht
Tabletten
aantal = dosis / sterkte van een tablet
Eenheden
1 g = 1000 mg, 1 mg = 1000 mcg

Methode

Infuussnelheden

Pompsnelheid
mL/h = volume (mL)/time (h)
Druppelsnelheid
drops/min = volume (mL) × drop factor / time (min)
Inlooptijd
time (h) = volume (mL)/rate (mL/h)

Methode

4 mL/kg (eerste 10 kg) + 2 mL/kg (volgende 10 kg) + 1 mL/kg (elke kg boven 20)
Holliday–Segar per dag
100 / 50 / 20 mL/kg voor dezelfde gewichtsklassen
Vochtbalans
intake − output
IJsschilfers
tellen voor de helft van hun volume
Urineproductie
mL/kg/h = mL / (kg × h)
Inlooptijd
tijd (h) = resterend volume (mL) / snelheid (mL/h)
Toegediend volume
mL = snelheid × tijd

Calling medicine candy encourages children to look for it and swallow it, which can lead to poisoning. Parents are taught to call medicine medicine and keep it locked away.

Antwoord

B · Do not call medicine candy, because the child may take it when no one is watching
4.Infectiepreventie en patiëntveiligheid

Antwoord: A · Stop the infusion and check the patient’s blood glucose

Waarom dit het antwoord is

A concentration error means the dose delivered was wrong. The nurse stops the infusion, assesses the patient including blood glucose, treats as needed, notifies the prescriber and reports the event.

Antwoord

A · Stop the infusion and check the patient’s blood glucose
5.Prioriteren en delegeren

Antwoord: A · Last drink two days ago; now shaking, sweating, heart rate 124/min and seeing insects

Waarom dit het antwoord is

Hallucinations, tremor, sweating and tachycardia two to three days after the last drink suggest severe withdrawal and possible delirium tremens, which can be fatal. This patient needs prompt assessment and treatment.

Antwoord

A · Last drink two days ago; now shaking, sweating, heart rate 124/min and seeing insects
6.Prioriteren en delegeren

Antwoord: C · Flush the eye with plenty of water or saline straight away

Waarom dit het antwoord is

Chemical eye injuries are treated by immediate, continuous irrigation with large amounts of water or saline. Delay, rubbing or a patch allows more damage.

Antwoord

C · Flush the eye with plenty of water or saline straight away
7.Klinische verpleegkunde

Antwoord: C · Removing hair, if needed, with clippers rather than a razor

Waarom dit het antwoord is

Razors cause micro-abrasions that increase infection risk, so hair is removed only if necessary with clippers just before surgery. Keeping patients warm and good skin preparation also help.

Antwoord

C · Removing hair, if needed, with clippers rather than a razor
8.Klinische verpleegkunde

Antwoord: B · Acute kidney injury from rhabdomyolysis

Waarom dit het antwoord is

Prolonged pressure on muscles causes rhabdomyolysis, releasing myoglobin and potassium. Myoglobin can block kidney tubules; treatment includes IV fluids and close monitoring of urine output and electrolytes.

Antwoord

B · Acute kidney injury from rhabdomyolysis
9.Laboratoriumwaarden en elektrolyten

Antwoord: D · Low TSH with a high free T4

Waarom dit het antwoord is

In Graves disease, antibodies stimulate the thyroid to make too much hormone. The high free T4 suppresses TSH from the pituitary through negative feedback, so TSH is low. A high TSH with a low T4 fits primary hypothyroidism.Gecontroleerd met: TSH test · MedlinePlus (NIH) (2026); Thyroxine (T4) Test · MedlinePlus (NIH) (2024); Graves disease · MedlinePlus (NIH) (2026)

Antwoord

D · Low TSH with a high free T4
10.Laboratoriumwaarden en elektrolyten

Antwoord: A · Give another 15 g of fast carbohydrate and recheck

Waarom dit het antwoord is

The "15-15 rule" is repeated until the glucose is at least 70 mg/dL (3,9 mmol/L); UK guidance uses 4,0 mmol/L and 15–20 g of carbohydrate. Then a snack or meal with complex carbohydrate and protein is given if the next meal is not soon, to prevent recurrence.

Antwoord

A · Give another 15 g of fast carbohydrate and recheck
11.Farmacologie

Antwoord: D · It stimulates muscarinic receptors and contracts the bladder muscle

Waarom dit het antwoord is

Bethanechol mimics acetylcholine at muscarinic receptors, contracting the detrusor muscle. It must not be used if there is mechanical obstruction to outflow.

Antwoord

D · It stimulates muscarinic receptors and contracts the bladder muscle
12.Farmacologie

Antwoord: B · Glucagon

Waarom dit het antwoord is

Beta blockers can make anaphylaxis more severe and resistant to adrenaline. Glucagon acts through its own receptor to increase heart rate and contractility, bypassing the blocked beta receptors.

Antwoord

B · Glucagon
13.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: C · Below 0,5 mL/kg/h, suggesting poor kidney perfusion

Waarom dit het antwoord is

A urine output below about 0,5 mL/kg/h (35 mL/h for 70 kg) for several hours is oliguria and may indicate hypovolaemia, shock or acute kidney injury. It should be reported along with the other observations, and the catheter checked for blockage.

Antwoord

C · Below 0,5 mL/kg/h, suggesting poor kidney perfusion
14.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: D · Pneumothorax

Waarom dit het antwoord is

Pneumothorax is a common complication of thoracentesis, caused by the needle puncturing the lung or air entering the chest. The nurse assesses the patient, gives oxygen and informs the team, who will arrange a chest X-ray. An effusion re-accumulates over days, and infection appears later.

Antwoord

D · Pneumothorax
15.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: D · Chronic subdural haematoma

Waarom dit het antwoord is

Older people have brain atrophy that stretches the bridging veins, and anticoagulants increase bleeding. A slow venous bleed can cause gradual confusion, headache or weakness over weeks. A CT scan is needed.

Antwoord

D · Chronic subdural haematoma
16.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: D · Overdrainage of CSF, so act per protocol and inform the team

Waarom dit het antwoord is

When the head rises above the drain level, CSF drains faster. Overdrainage can cause headache, collapse of the ventricles or a subdural bleed. The nurse follows the protocol (often clamping briefly and re-levelling) and informs the neurosurgical team.

Antwoord

D · Overdrainage of CSF, so act per protocol and inform the team
17.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: D · Tell the surgeon, as the patient may not be able to consent now

Waarom dit het antwoord is

Opioids and sedatives can impair understanding, so consent may not be valid. The surgeon is informed; consent is obtained when the patient has capacity, or other legal processes are followed in an emergency.

Antwoord

D · Tell the surgeon, as the patient may not be able to consent now
18.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: B · Cardiac tamponade from a pericardial effusion

Waarom dit het antwoord is

Malignant pericardial effusion can compress the heart, causing hypotension, raised JVP and muffled heart sounds. It is an emergency needing urgent echocardiography and drainage (pericardiocentesis).

Antwoord

B · Cardiac tamponade from a pericardial effusion
19.Verloskundige verpleegkunde

Antwoord: C · Full flexion, with the chin on the chest

Waarom dit het antwoord is

When the head is fully flexed, the suboccipitobregmatic diameter of about 9,5 cm presents. Extension, as in brow and face presentations, presents larger diameters and can obstruct labour.

Antwoord

C · Full flexion, with the chin on the chest
20.Verloskundige verpleegkunde

21.Kinderverpleegkunde

Antwoord: B · Pull the pinna gently down and back

Waarom dit het antwoord is

In children under about 3 years the ear canal is short and angled upward, so the pinna is pulled down and back. In older children and adults it is pulled up and back.

Antwoord

B · Pull the pinna gently down and back
22.Kinderverpleegkunde

Antwoord: A · It works best before any exposure to the virus, and young adolescents respond strongly

Waarom dit het antwoord is

HPV is passed on through sexual contact, so vaccination is most effective before any exposure. Younger adolescents also produce a strong antibody response. The vaccine protects against HPV types that cause cervical and other cancers and genital warts.

Antwoord

A · It works best before any exposure to the virus, and young adolescents respond strongly
23.Psychiatrische verpleegkunde

Antwoord: B · Explore the experience in her cultural context before calling it a symptom

Waarom dit het antwoord is

A belief or experience that is accepted in the person's culture is not classed as a delusion or hallucination in itself. The nurse explores its meaning, how it is shared by others and whether it causes distress.

Antwoord

B · Explore the experience in her cultural context before calling it a symptom
24.Psychiatrische verpleegkunde

Antwoord: D · "Please don't stop your lithium on your own. Let's arrange for you to plan this with your prescriber before you conceive."

Waarom dit het antwoord is

Lithium in early pregnancy is linked to cardiac malformations, but stopping it abruptly greatly raises the risk of relapse. Pre-conception planning with the prescriber balances these risks.

Antwoord

D · "Please don't stop your lithium on your own. Let's arrange for you to plan this with your prescriber before you conceive."
25.Verpleegkundig rekenen en eenhedenomrekeningen

Antwoord: 09:55

Elk item in mL

8 fl oz = 8·30 = 240 mL
1½ tbsp = 1,5·15 = 22,5 mL
2½ tsp = 2,5·5 = 12,5 mL

Optellen

V = 240 + 22,5 + 12,5 = 275 mL

Antwoord

V = 275 mL
27.Dosisberekeningen

Antwoord: V = 2,8 mL

D = 4·7 = 28 mg

Volume

V = 28/10 = 2,8 mL

Antwoord

V = 2,8 mL
28.Dosisberekeningen

Antwoord: D = 185,2 mg

Body-surface-area dosing: many cytotoxic and some paediatric doses are prescribed per square metre of body surface area: dose (mg) = dose per m² × BSA (m²). The Mosteller formula estimates BSA = √(height (cm) × weight (kg)/3600).

Lichaamsoppervlak

BSA = √(154·44/3600) = √1,882 = 1,372 m²

Dosis

D = 135·1,372 = 185,2 mg

Antwoord

D = 185,2 mg
29.Infuussnelheden

Antwoord: R = 17 gtt/min

Tijd in minuten

10·60 = 600 min

Druppelsnelheid

R = 1000·10/600 = 16,67 ≈ 17 gtt/min

Antwoord

R = 17 gtt/min

Concentratie

1000/100 = 10 mg/mL

Dosissnelheid

D = 10·5 = 50 mg/h

Antwoord

D = 50 mg/h
31.Infuusvloeistoffen en vochtbalans

Antwoord: B = −40 mL

Totale intake

oraal: 240 + 120 = 360 mL
ijsschilfers: 240/2 = 120 mL
Infuus: 75·12 = 900 mL
intraveneus antibioticum: 1·50 = 50 mL
totale intake = 1430 mL

Totale output

475 + 275 + 400 + 120 + 200 = 1470 mL

Balans

B = 1430 − 1470 = −40 mL

Antwoord

B = −40 mL

Resterend volume

500 − 250 = 250 mL

Tijd

t = 250/80 = 3,125 ≈ 3,1 h

Antwoord

t = 3,1 h
