1Formules en methoden

Infectiepreventie en patiëntveiligheid

Methode

Metrische stappen
1 kg = 1000 g, 1 g = 1000 mg, 1 mg = 1000 mcg, 1 L = 1000 mL
Gewicht en lengte
1 kg = 2,2 lb, 1 lb = 16 oz, 1 in = 2,54 cm, 1 ft = 12 in
Huishoudelijke maten
1 tsp = 5 mL, 1 tbsp = 15 mL, 1 fl oz = 30 mL, 1 cup = 8 fl oz = 240 mL
Temperatuur
°C = (°F − 32)·5/9, °F = °C·9/5 + 32
24-uursklok
PM-uren 1–11: tel er 12 bij op; 12 AM = 00:xx; 12 PM = 12:xx
Dosissnelheid
mcg/min × 60/1000 = mg/h
Laboratoriumfactoren
glucose 1 mmol/L = 18 mg/dL; creatinine 1 mg/dL = 88,4 µmol/L; Hb 1 g/dL = 10 g/L

Methode

Dosisberekeningen

Dosis uit de concentratie
volume = dosis / concentratie
Dosis per kilogram lichaamsgewicht
dosis = mg/kg × lichaamsgewicht
Tabletten
aantal = dosis / sterkte van een tablet
Eenheden
1 g = 1000 mg, 1 mg = 1000 mcg

Methode

Infuussnelheden

Pompsnelheid
mL/h = volume (mL)/time (h)
Druppelsnelheid
drops/min = volume (mL) × drop factor / time (min)
Inlooptijd
time (h) = volume (mL)/rate (mL/h)

Methode

4 mL/kg (eerste 10 kg) + 2 mL/kg (volgende 10 kg) + 1 mL/kg (elke kg boven 20)
Holliday–Segar per dag
100 / 50 / 20 mL/kg voor dezelfde gewichtsklassen
Vochtbalans
intake − output
IJsschilfers
tellen voor de helft van hun volume
Urineproductie
mL/kg/h = mL / (kg × h)
Inlooptijd
tijd (h) = resterend volume (mL) / snelheid (mL/h)
Toegediend volume
mL = snelheid × tijd

Antwoord

B · The respiratory rate and level of sedation
3.Infectiepreventie en patiëntveiligheid

Antwoord: B · Working suction and oxygen equipment

Waarom dit het antwoord is

Suction and oxygen are kept ready to manage secretions and support breathing during or after a seizure. Nothing is put in the mouth, and restraints are not used because they can cause injury.

Antwoord

B · Working suction and oxygen equipment
4.Infectiepreventie en patiëntveiligheid

Antwoord: B · A wrong-patient sample, so the test is repeated and the event reported

Waarom dit het antwoord is

A different blood group strongly suggests the sample came from another patient. A new sample is taken with careful bedside identification and labeling, and the event is reported as a serious near miss.

Antwoord

B · A wrong-patient sample, so the test is repeated and the event reported
5.Prioriteren en delegeren

Antwoord: B · Raise the head of the bed, give oxygen and call for urgent help

Waarom dit het antwoord is

Sudden breathlessness and chest pain after surgery suggest a pulmonary embolism. The nurse supports breathing with positioning and oxygen and escalates at once; walking or leg massage could make things worse.

Antwoord

B · Raise the head of the bed, give oxygen and call for urgent help
6.Prioriteren en delegeren

Antwoord: A · Respiratory muscle fatigue with possible impending respiratory failure

Waarom dit het antwoord is

Early in an asthma attack, fast breathing lowers the PaCO₂. A rising, “normal” value in a tiring patient means they can no longer move enough air, which is a warning sign of respiratory failure.

Antwoord

A · Respiratory muscle fatigue with possible impending respiratory failure
7.Klinische verpleegkunde

Antwoord: A · Attacks of symptoms, such as visual loss or numbness, that improve between relapses

Waarom dit het antwoord is

MS causes demyelination in the brain and spinal cord. Many patients first have attacks such as optic neuritis or sensory symptoms, with partial or full recovery between relapses.

Antwoord

A · Attacks of symptoms, such as visual loss or numbness, that improve between relapses
8.Klinische verpleegkunde

Antwoord: D · Repeat the blood test promptly, as haemolysis can falsely raise potassium

Waarom dit het antwoord is

Haemolysis in the tube releases intracellular potassium, giving a falsely high reading. The result still needs prompt repeating and review, because true hyperkalaemia cannot be excluded.

Antwoord

D · Repeat the blood test promptly, as haemolysis can falsely raise potassium
9.Laboratoriumwaarden en elektrolyten

Antwoord: A · Hemoglobin and blood pressure

Waarom dit het antwoord is

Epoetin stimulates red cell production. Hemoglobin is monitored, because raising it too high increases the risk of hypertension, stroke and thrombosis. Blood pressure is monitored, and iron stores are checked, since iron is needed for a good response.Gecontroleerd met: Erythropoietin Stimulating Agents · StatPearls (NCBI Bookshelf) (2024)

Antwoord

A · Hemoglobin and blood pressure
10.Laboratoriumwaarden en elektrolyten

Antwoord: C · Low albumin raises the free (active) fraction of phenytoin

Waarom dit het antwoord is

Phenytoin is highly protein bound, and only the free drug is active. When albumin is low, more phenytoin is free, so toxicity can occur with a "normal" total level. A free phenytoin level or a corrected level helps.Gecontroleerd met: Phenytoin · StatPearls (NCBI Bookshelf) (2023); Antiepileptic Drug Monitoring · StatPearls (NCBI Bookshelf) (2025)

Antwoord

C · Low albumin raises the free (active) fraction of phenytoin
11.Farmacologie

Antwoord: D · Vasopressors cannot keep perfusion up if the circulation is under-filled

Waarom dit het antwoord is

Vasoconstriction without adequate volume reduces organ blood flow and can cause ischaemia. Fluid resuscitation restores preload, and vasopressors are then used for persistent hypotension.

Antwoord

D · Vasopressors cannot keep perfusion up if the circulation is under-filled
12.Farmacologie

Antwoord: A · To restore breathing without causing severe withdrawal and pain

Waarom dit het antwoord is

In opioid-dependent patients, a large dose of naloxone can cause abrupt withdrawal, severe pain and agitation. Small titrated doses aim to restore adequate breathing while keeping some analgesia.

Antwoord

A · To restore breathing without causing severe withdrawal and pain

Antwoord: B · Check vital signs, report urgently and anticipate a blood count

Waarom dit het antwoord is

Melaena with postural dizziness suggests upper gastrointestinal bleeding, a risk with dual antiplatelet therapy. The nurse assesses for shock, informs the team urgently and expects bloods, a group and save (type and screen) and possibly endoscopy. Stopping antiplatelets after a stent risks stent thrombosis, so the decision belongs to the medical and cardiology team.

Antwoord

B · Check vital signs, report urgently and anticipate a blood count
15.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: B · Een benzodiazepine zoals intraveneus lorazepam of buccaal midazolam

Antwoord

B · Een benzodiazepine zoals intraveneus lorazepam of buccaal midazolam
16.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: D · Contact the prescriber or pharmacist about another route

Waarom dit het antwoord is

Omitting Parkinson's medication can cause severe rigidity, swallowing failure and, rarely, a life-threatening withdrawal syndrome. Alternatives such as giving it by nasogastric tube or a dopamine agonist patch should be arranged urgently.

Antwoord

D · Contact the prescriber or pharmacist about another route
17.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: B · Soon after an acute soft tissue injury, to reduce swelling

Waarom dit het antwoord is

Cold reduces swelling, bleeding and nerve conduction in acute injuries. It is wrapped and applied for short periods and avoided on areas with poor circulation or sensation. Heat is better for muscle spasm and stiffness.

Antwoord

B · Soon after an acute soft tissue injury, to reduce swelling
18.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: B · Inform the anaesthetist, as surgery may be delayed

Waarom dit het antwoord is

Solid food is usually stopped 6 hours before anaesthesia because of aspiration risk. The anaesthetist decides whether to delay or change the plan; the nurse reports accurately and does not hide the information.

Antwoord

B · Inform the anaesthetist, as surgery may be delayed
19.Verloskundige verpleegkunde

Antwoord: D · Every 4 minutes, lasting 60 seconds

Waarom dit het antwoord is

Frequency is measured from the start of one contraction to the start of the next (4 minutes), and duration from the start to the end of the same contraction (60 seconds). The 3 minutes between contractions is the resting interval.

Antwoord

D · Every 4 minutes, lasting 60 seconds
20.Verloskundige verpleegkunde

Antwoord: A · Massage the fundus and look for the source

Waarom dit het antwoord is

The first priority is to stop the bleeding: massage a boggy fundus, check for tears, empty the bladder and give uterotonics per protocol. Weighing pads gives a more accurate blood loss but comes after life-saving actions.

Antwoord

A · Massage the fundus and look for the source
21.Kinderverpleegkunde

Antwoord: B · On the side, curled up with knees drawn up and head bent forward

Waarom dit het antwoord is

Curling the child on the side (or sitting and bending forward) flexes the spine and widens the spaces between lumbar vertebrae for the needle. A nurse holds the child gently and monitors breathing, as tight flexion can compromise the airway in infants.

Antwoord

B · On the side, curled up with knees drawn up and head bent forward
22.Kinderverpleegkunde

24.Psychiatrische verpleegkunde

Antwoord: D · Assess and treat her physical injuries, then offer forensic examination with her consent

Waarom dit het antwoord is

Medical care for injuries comes first. Forensic examination is offered, explained and carried out only with consent, and the nurse supports the survivor's control over each step.

Antwoord

D · Assess and treat her physical injuries, then offer forensic examination with her consent
25.Verpleegkundig rekenen en eenhedenomrekeningen

Antwoord: m = 2,61 kg

12 oz/16 = 0,75 lb
5 + 0,75 = 5,75 lb

Pond naar kilogram

m = 5,75/2,2 = 2,614 ≈ 2,61 kg

Antwoord

m = 2,61 kg
26.Verpleegkundig rekenen en eenhedenomrekeningen

Antwoord: t = 405 min

20:40 → 24:00 = 200 min

Na middernacht

00:00 → 03:25 = 205 min

Totaal

t = 200 + 205 = 405 min

Antwoord

t = 405 min
27.Dosisberekeningen

Antwoord: V = 1 mL

D = 2,5·20 = 50 mg

Volume

V = 50/50 = 1 mL

Antwoord

V = 1 mL
28.Dosisberekeningen

Antwoord: V = 10 mL

20·20 = 400 mg

Volume

V = (400/200)·5 = 10 mL

Antwoord

V = 10 mL
29.Infuussnelheden

Antwoord: V = 720 mL

Infusion rates: a pump is set in mL/h = volume/time in hours. A gravity drip is counted in drops per minute: gtt/min = volume (mL) × drop factor (gtt/mL)/time (min). The drop factor is printed on the tubing package: macrodrip sets deliver 10, 15 or 20 gtt/mL (blood sets often 10 gtt/mL in the US, 15 in the UK) and microdrip sets 60 gtt/mL. Always use the drop factor the question or package gives.

Snelheid in mL/h

40·60/20 = 120 mL/h

Volume

V = 120·6 = 720 mL

Antwoord

V = 720 mL
30.Infuussnelheden

Antwoord: R = 60 mL/h

Dose-based infusions: concentration = amount of drug in the bag/bag volume. Pump rate (mL/h) = dose per hour/concentration, and dose per hour = mL/h × concentration. Convert per-minute doses to per-hour (× 60) and keep the mass units the same (1 g = 1000 mg, 1 mg = 1000 mcg).

Concentratie

1000 mg/500 mL = 2 mg/mL

Dosis per uur

2·60 = 120 mg/h

Pompsnelheid

R = 120/2 = 60 mL/h

Antwoord

R = 60 mL/h
31.Infuusvloeistoffen en vochtbalans

Antwoord: I = 1630 mL

oraal: 180 + 350 + 100 = 630 mL
Infuus: 100·8 = 800 mL
intraveneus antibioticum: 2·100 = 200 mL
I = 1630 mL

Antwoord

I = 1630 mL
32.Infuusvloeistoffen en vochtbalans

Antwoord: t = 6 h

Resterend volume

500 − 50 = 450 mL

Tijd

t = 450/75 = 6 h

Antwoord

t = 6 h

Score elke parameter

RR 16/min → 0
SpO₂ 98% → 0
met zuurstof → 2
SBP 106 mmHg → 1
HR 98/min → 1
alert (A) → 0
T 36,8 °C → 0

Optellen

NEWS2 = 0 + 0 + 2 + 1 + 1 + 0 + 0 = 4

Antwoord

NEWS2 = 4
34.Klinische beoordelingsscores
