1Formules en methoden

Infectiepreventie en patiëntveiligheid

Methode

Metrische stappen
1 kg = 1000 g, 1 g = 1000 mg, 1 mg = 1000 mcg, 1 L = 1000 mL
Gewicht en lengte
1 kg = 2,2 lb, 1 lb = 16 oz, 1 in = 2,54 cm, 1 ft = 12 in
Huishoudelijke maten
1 tsp = 5 mL, 1 tbsp = 15 mL, 1 fl oz = 30 mL, 1 cup = 8 fl oz = 240 mL
Temperatuur
°C = (°F − 32)·5/9, °F = °C·9/5 + 32
24-uursklok
PM-uren 1–11: tel er 12 bij op; 12 AM = 00:xx; 12 PM = 12:xx
Dosissnelheid
mcg/min × 60/1000 = mg/h
Laboratoriumfactoren
glucose 1 mmol/L = 18 mg/dL; creatinine 1 mg/dL = 88,4 µmol/L; Hb 1 g/dL = 10 g/L

Methode

Dosisberekeningen

Dosis uit de concentratie
volume = dosis / concentratie
Dosis per kilogram lichaamsgewicht
dosis = mg/kg × lichaamsgewicht
Tabletten
aantal = dosis / sterkte van een tablet
Eenheden
1 g = 1000 mg, 1 mg = 1000 mcg

Methode

Infuussnelheden

Pompsnelheid
mL/h = volume (mL)/time (h)
Druppelsnelheid
drops/min = volume (mL) × drop factor / time (min)
Inlooptijd
time (h) = volume (mL)/rate (mL/h)

Methode

4 mL/kg (eerste 10 kg) + 2 mL/kg (volgende 10 kg) + 1 mL/kg (elke kg boven 20)
Holliday–Segar per dag
100 / 50 / 20 mL/kg voor dezelfde gewichtsklassen
Vochtbalans
intake − output
IJsschilfers
tellen voor de helft van hun volume
Urineproductie
mL/kg/h = mL / (kg × h)
Inlooptijd
tijd (h) = resterend volume (mL) / snelheid (mL/h)
Toegediend volume
mL = snelheid × tijd

3Oplossingen

1.Basisverpleegkunde

Antwoord: B · Slowly, low and close to the skin, while supporting the skin

Waarom dit het antwoord is

Peeling the tape back low and slow while holding the skin reduces the pulling force on fragile skin. Adhesive removers can also help.

Antwoord

B · Slowly, low and close to the skin, while supporting the skin
2.Basisverpleegkunde

Antwoord: D · "I'll cut the opening much bigger than my stoma so it doesn't rub."

Waarom dit het antwoord is

An opening that is too large leaves skin exposed to output, causing soreness and leaks. Checking the skin, re-measuring and drying the skin are correct.

Antwoord

D · "I'll cut the opening much bigger than my stoma so it doesn't rub."
3.Infectiepreventie en patiëntveiligheid

Antwoord: B · At the bedside, in front of the patient, right after collection

Waarom dit het antwoord is

Labeling tubes at the bedside, using two patient identifiers and in the patient’s presence, prevents wrong-patient specimens. Pre-labeled or later-labeled tubes are easily mixed up.

Antwoord

B · At the bedside, in front of the patient, right after collection
4.Infectiepreventie en patiëntveiligheid

Antwoord: D · “I think he needs to be assessed now. Can you come within the next 15 minutes?”

Waarom dit het antwoord is

In SBAR, the situation names the problem, the background gives the history, the assessment shares the nurse’s findings and judgement, and the recommendation states a clear request with a time frame.

Antwoord

D · “I think he needs to be assessed now. Can you come within the next 15 minutes?”
5.Prioriteren en delegeren

Antwoord: C · “Stop and call me at once if she feels dizzy or short of breath.”

Waarom dit het antwoord is

Clear delegation includes what to watch for and when to report. Dizziness or breathlessness may mean low blood pressure, bleeding or a pulmonary embolism, so the nurse must know at once.

Antwoord

C · “Stop and call me at once if she feels dizzy or short of breath.”
6.Prioriteren en delegeren

Antwoord: B · Seizure precautions and close neurological checks

Waarom dit het antwoord is

Severe hyponatremia can cause seizures and falls. The nurse keeps the patient safe and reports changes; free water is restricted rather than encouraged, as prescribed.

Antwoord

B · Seizure precautions and close neurological checks
7.Klinische verpleegkunde

Antwoord: C · Delays can quickly worsen rigidity, immobility and swallowing

Waarom dit het antwoord is

Levodopa has a short effect, so missed or late doses can lead to severe stiffness, falls, swallowing problems and, rarely, a dangerous hyperpyrexia-like state. Parkinson's medicines are treated as time-critical.

Antwoord

C · Delays can quickly worsen rigidity, immobility and swallowing
8.Klinische verpleegkunde

Antwoord: C · Standard oximeters cannot tell carboxyhaemoglobin from oxyhaemoglobin

Waarom dit het antwoord is

Standard SpO2 can appear normal despite severe tissue hypoxia. Carboxyhaemoglobin is measured on a blood gas, and high-concentration oxygen is given.

Antwoord

C · Standard oximeters cannot tell carboxyhaemoglobin from oxyhaemoglobin
9.Laboratoriumwaarden en elektrolyten

Antwoord: C · Gently inverting the tubes instead of shaking them

Waarom dit het antwoord is

Hemolysis falsely raises potassium and affects other results. It is reduced by gentle inversion of tubes, avoiding very small needles, avoiding strong suction or forcing blood through a needle, and keeping tourniquet time short.

Antwoord

C · Gently inverting the tubes instead of shaking them
10.Laboratoriumwaarden en elektrolyten

Antwoord: B · A low erythropoietin level

Waarom dit het antwoord is

In polycythemia vera, a marrow mutation (usually JAK2) drives red cell production, so erythropoietin is low. In secondary polycythemia from hypoxia (COPD, high altitude, smoking), erythropoietin is high. Often white cells and platelets are also raised in polycythemia vera.Gecontroleerd met: Erythropoietin test · MedlinePlus (NIH) (2025); Polycythemia vera · MedlinePlus (NIH) (2025)

Antwoord

B · A low erythropoietin level
11.Farmacologie

12.Farmacologie

Antwoord: C · Rebound hypertension from abrupt clonidine withdrawal

Waarom dit het antwoord is

Clonidine is a central alpha-2 agonist. Stopping it suddenly lets sympathetic activity surge, causing rebound hypertension, so it is tapered gradually.

Antwoord

C · Rebound hypertension from abrupt clonidine withdrawal
13.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: B · Some warning signs, such as a fast heartbeat, may be masked

Waarom dit het antwoord is

Beta-blockers can blunt adrenergic symptoms of hypoglycaemia such as palpitations and tremor. Sweating usually remains. Patients should check their glucose if they feel unwell and know their other warning signs.

Antwoord

B · Some warning signs, such as a fast heartbeat, may be masked
14.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: D · A large difference in blood pressure between the arms

Waarom dit het antwoord is

A difference of more than about 20 mmHg in systolic pressure between the arms, a pulse deficit or a new aortic regurgitation murmur suggests dissection. Sweating, tachycardia and rest pain also occur in MI. Thrombolysis and anticoagulants could be disastrous in dissection, so the distinction matters.

Antwoord

D · A large difference in blood pressure between the arms
15.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: A · Loss of movement and pain sense, with position sense kept

Waarom dit het antwoord is

Anterior cord syndrome usually follows damage to the anterior spinal artery. Motor, pain and temperature pathways are lost below the injury, but the dorsal columns (fine touch, vibration and proprioception) at the back are spared.

Antwoord

A · Loss of movement and pain sense, with position sense kept
16.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: B · Suspect a clot in the catheter and follow the protocol

Waarom dit het antwoord is

Clots blocking the catheter make the bladder distend, causing pain and spasms. The nurse checks for kinks, adjusts the irrigation rate per protocol, performs a bladder washout if trained and escalates if not cleared.

Antwoord

B · Suspect a clot in the catheter and follow the protocol
17.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: B · To detect dehydration, fluid overload or kidney problems early

Waarom dit het antwoord is

Fluid balance charts record IV and oral intake against urine, drains, vomit and NG losses. Trends help detect hypovolaemia, overload and acute kidney injury.

Antwoord

B · To detect dehydration, fluid overload or kidney problems early
18.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: C · Withhold strong compression and discuss urgent vascular referral

Waarom dit het antwoord is

An ABPI of 0,5 suggests severe arterial disease, and strong compression could cause limb ischaemia. The nurse raises the concern and the patient is referred to vascular specialists.

Antwoord

C · Withhold strong compression and discuss urgent vascular referral
19.Verloskundige verpleegkunde

Antwoord: A · It is hormone-free and very effective

Waarom dit het antwoord is

The copper IUD contains no hormones, does not affect milk supply and is highly effective for many years. It does not protect against sexually transmitted infections.

Antwoord

A · It is hormone-free and very effective
20.Verloskundige verpleegkunde

21.Kinderverpleegkunde

Antwoord: A · A bulging fontanelle, a high-pitched cry, poor feeding and irritability

Waarom dit het antwoord is

Young infants with meningitis may have non-specific signs: fever or low temperature, poor feeding, lethargy or irritability, a high-pitched cry and a bulging fontanelle. Neck stiffness is often absent at this age.

Antwoord

A · A bulging fontanelle, a high-pitched cry, poor feeding and irritability
22.Kinderverpleegkunde

Antwoord: C · Phosphate

Waarom dit het antwoord is

When feeding restarts after starvation, insulin drives phosphate, potassium and magnesium into cells. Low phosphate can cause heart failure, arrhythmias and confusion, so electrolytes are checked often and feeds are increased gradually.

Antwoord

C · Phosphate
23.Psychiatrische verpleegkunde

Antwoord: C · Stay with him in a calm, quiet place and reassure him that the effects are from the drug and will pass

Waarom dit het antwoord is

For a frightening hallucinogen experience, a calm setting, a supportive person and simple reassurance usually help. The nurse also monitors vital signs and watches for injury or other substances.

Antwoord

C · Stay with him in a calm, quiet place and reassure him that the effects are from the drug and will pass
24.Psychiatrische verpleegkunde

Antwoord: A · Assign a female nurse where possible and explain her care plan to her

Waarom dit het antwoord is

Trauma-informed care respects the patient's need for safety and control. Where possible, adjusting staffing reduces re-traumatisation and helps build trust.

Antwoord

A · Assign a female nurse where possible and explain her care plan to her
25.Verpleegkundig rekenen en eenhedenomrekeningen

Antwoord: T = 40,9 °C

105,6 − 32 = 73,6

Vermenigvuldig met 5/9

T = 73,6·5/9 = 40,89 ≈ 40,9 °C

Antwoord

T = 40,9 °C
26.Verpleegkundig rekenen en eenhedenomrekeningen

Antwoord: V = 105 mL

1 tsp = 1·5 = 5 mL

Hele kuur

V = 5 mL × 3/dag × 7 = 105 mL

Antwoord

V = 105 mL
27.Dosisberekeningen

Antwoord: V = 9,2 mL

D = 4·23 = 92 mg

Volume

V = 92/10 = 9,2 mL

Antwoord

V = 9,2 mL
28.Dosisberekeningen

Antwoord: V = 8,4 mL

Dose calculation: the amount to give equals what is prescribed divided by what you have, times the quantity it comes in: amount = (desired dose/stock strength) × stock volume. Always keep the units the same (convert g to mg, mg to mcg first) and check that the answer is sensible.

Gewicht in kg

30,8/2,2 = 14 kg

Dosis

15·14 = 210 mg

Volume

V = (210/125)·5 = 8,4 mL

Antwoord

V = 8,4 mL
29.Infuussnelheden

Antwoord: V = 1200 mL

Infusion rates: a pump is set in mL/h = volume/time in hours. A gravity drip is counted in drops per minute: gtt/min = volume (mL) × drop factor (gtt/mL)/time (min). The drop factor is printed on the tubing package: macrodrip sets deliver 10, 15 or 20 gtt/mL (blood sets often 10 gtt/mL in the US, 15 in the UK) and microdrip sets 60 gtt/mL. Always use the drop factor the question or package gives.

Snelheid in mL/h

50·60/20 = 150 mL/h

Volume

V = 150·8 = 1200 mL

Antwoord

V = 1200 mL
30.Infuussnelheden

Antwoord: R = 100 gtt/min

Dose-based infusions: concentration = amount of drug in the bag/bag volume. Pump rate (mL/h) = dose per hour/concentration, and dose per hour = mL/h × concentration. Convert per-minute doses to per-hour (× 60) and keep the mass units the same (1 g = 1000 mg, 1 mg = 1000 mcg).

Concentratie

200/100 = 2 mg/mL

Volume per uur

200/2 = 100 mL/h

Druppelsnelheid

R = 100·60/60 = 100 gtt/min

Antwoord

R = 100 gtt/min
31.Infuusvloeistoffen en vochtbalans

Antwoord: I = 2150 mL

oraal: 150 + 200 + 150 = 500 mL
ijsschilfers: 100/2 = 50 mL
Infuus: 125·12 = 1500 mL
intraveneus antibioticum: 2·50 = 100 mL
I = 2150 mL

Antwoord

I = 2150 mL
32.Infuusvloeistoffen en vochtbalans

Antwoord: t = 2,7 h

Resterend volume

500 − 100 = 400 mL

Tijd

t = 400/150 = 2,667 ≈ 2,7 h

Antwoord

t = 2,7 h
