1Formules en methoden

Infectiepreventie en patiëntveiligheid

Methode

Metrische stappen
1 kg = 1000 g, 1 g = 1000 mg, 1 mg = 1000 mcg, 1 L = 1000 mL
Gewicht en lengte
1 kg = 2,2 lb, 1 lb = 16 oz, 1 in = 2,54 cm, 1 ft = 12 in
Huishoudelijke maten
1 tsp = 5 mL, 1 tbsp = 15 mL, 1 fl oz = 30 mL, 1 cup = 8 fl oz = 240 mL
Temperatuur
°C = (°F − 32)·5/9, °F = °C·9/5 + 32
24-uursklok
PM-uren 1–11: tel er 12 bij op; 12 AM = 00:xx; 12 PM = 12:xx
Dosissnelheid
mcg/min × 60/1000 = mg/h
Laboratoriumfactoren
glucose 1 mmol/L = 18 mg/dL; creatinine 1 mg/dL = 88,4 µmol/L; Hb 1 g/dL = 10 g/L

Methode

Dosisberekeningen

Dosis uit de concentratie
volume = dosis / concentratie
Dosis per kilogram lichaamsgewicht
dosis = mg/kg × lichaamsgewicht
Tabletten
aantal = dosis / sterkte van een tablet
Eenheden
1 g = 1000 mg, 1 mg = 1000 mcg

Methode

Infuussnelheden

Pompsnelheid
mL/h = volume (mL)/time (h)
Druppelsnelheid
drops/min = volume (mL) × drop factor / time (min)
Inlooptijd
time (h) = volume (mL)/rate (mL/h)

Methode

4 mL/kg (eerste 10 kg) + 2 mL/kg (volgende 10 kg) + 1 mL/kg (elke kg boven 20)
Holliday–Segar per dag
100 / 50 / 20 mL/kg voor dezelfde gewichtsklassen
Vochtbalans
intake − output
IJsschilfers
tellen voor de helft van hun volume
Urineproductie
mL/kg/h = mL / (kg × h)
Inlooptijd
tijd (h) = resterend volume (mL) / snelheid (mL/h)
Toegediend volume
mL = snelheid × tijd

3Oplossingen

1.Basisverpleegkunde

Antwoord: B · Measuring how much urine is in the bladder without a catheter

Waarom dit het antwoord is

A bladder scanner uses ultrasound to estimate bladder volume, for example after a patient has voided. It helps detect retention and avoids unnecessary catheterisation.

Antwoord

B · Measuring how much urine is in the bladder without a catheter
2.Basisverpleegkunde

Antwoord: C · At the bedside, with the patient and their wristband

Waarom dit het antwoord is

The final check matches the patient's identity, by asking and from the wristband, with the blood bag and prescription at the bedside. Most wrong-blood events come from checks done away from the patient.

Antwoord

C · At the bedside, with the patient and their wristband
3.Infectiepreventie en patiëntveiligheid

6.Prioriteren en delegeren

Antwoord: B · Rejection of the transplant

Waarom dit het antwoord is

Fever, falling urine output and graft tenderness are signs of possible rejection. The nurse reports promptly, because early treatment can save the kidney.

Antwoord

B · Rejection of the transplant
7.Klinische verpleegkunde

Antwoord: D · Lying flat, with the legs raised

Waarom dit het antwoord is

Lying flat with the legs raised supports circulation in anaphylactic shock. Patients with breathing difficulty may prefer to sit up, but should not stand or walk, as this can cause collapse.

Antwoord

D · Lying flat, with the legs raised
8.Klinische verpleegkunde

Antwoord: A · Hypokaliëmie

Antwoord

A · Hypokaliëmie
9.Laboratoriumwaarden en elektrolyten

Antwoord: D · The pancreas is making very little insulin, as in type 1 diabetes

Waarom dit het antwoord is

C-peptide is cleaved from proinsulin when insulin is made, so it reflects the body's own insulin production. A very low level suggests beta-cell loss, as in type 1 diabetes; in type 2 diabetes it is often normal or high early on.Gecontroleerd met: C-Peptide Test · MedlinePlus (NIH) (2025)

Antwoord

D · The pancreas is making very little insulin, as in type 1 diabetes

11.Farmacologie

Antwoord: B · To reduce the risk of oral thrush (candidiasis) and hoarseness

Waarom dit het antwoord is

Steroid left in the mouth and throat suppresses local immunity and can cause candidiasis and a hoarse voice. Rinsing and spitting, and using a spacer, reduce this risk.

Antwoord

B · To reduce the risk of oral thrush (candidiasis) and hoarseness
12.Farmacologie

Antwoord: B · Bone marrow suppression, because both drugs inhibit folate metabolism

Waarom dit het antwoord is

Trimethoprim and methotrexate both inhibit dihydrofolate reductase, and trimethoprim also reduces methotrexate excretion. The combination can cause severe pancytopenia, so another antibiotic is chosen.

Antwoord

B · Bone marrow suppression, because both drugs inhibit folate metabolism
13.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: C · To make sputum thinner and easier to cough up

Waarom dit het antwoord is

Mucolytics reduce sputum viscosity, which can make expectoration easier and may reduce exacerbations in some people with COPD. They are not bronchodilators or antibiotics. Cough suppressants are generally avoided in productive cough.

Antwoord

C · To make sputum thinner and easier to cough up
14.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: A · A 12-lead ECG

Waarom dit het antwoord is

After return of spontaneous circulation, a 12-lead ECG looks for STEMI, which needs urgent coronary angiography and PCI. Post-resuscitation care also aims for normal oxygen and CO₂ levels, blood pressure support, glucose control and avoiding fever.

Antwoord

A · A 12-lead ECG
15.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: B · It protects the heart from arrhythmias

Waarom dit het antwoord is

Calcium stabilises the cardiac cell membrane and reduces the risk of arrhythmias within minutes. It does not change the potassium level, so other treatments are needed to shift and remove potassium.

Antwoord

B · It protects the heart from arrhythmias
16.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: D · Explain that this is emotional lability after stroke and help the family understand

Waarom dit het antwoord is

Emotional lability (pseudobulbar affect) is common after stroke: crying or laughter does not match the person's mood. Explaining it, staying calm and gently distracting the patient helps. True depression is also common and should be screened for separately.

Antwoord

D · Explain that this is emotional lability after stroke and help the family understand
17.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: C · Alternate staples first, then the rest if the wound holds

Waarom dit het antwoord is

Removing alternate staples first allows the nurse to check that the wound edges stay together. If the wound is secure, the rest are removed, and adhesive strips can be applied for support.

Antwoord

C · Alternate staples first, then the rest if the wound holds
18.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: D · Use long-handled forceps to put it in the lead container; report it

Waarom dit het antwoord is

A dislodged source is picked up with long-handled forceps and placed in the shielded container kept in the room. The radiation safety officer and team are informed, following local policy.

Antwoord

D · Use long-handled forceps to put it in the lead container; report it
19.Verloskundige verpleegkunde

Antwoord: C · Sudden swelling of the face and hands with a headache

Waarom dit het antwoord is

Sudden facial or hand swelling, severe headache, visual changes and upper abdominal pain can signal pre-eclampsia. Mild ankle oedema, white discharge and heartburn are common discomforts.

Antwoord

C · Sudden swelling of the face and hands with a headache
20.Verloskundige verpleegkunde

Antwoord: D · "I will check my glucose as shown and record it."

Waarom dit het antwoord is

Regular glucose monitoring with records guides treatment changes. Skipping meals can cause hypoglycaemia and ketosis, insulin does not cross the placenta in a meaningful amount, and fetal growth is monitored.

Antwoord

D · "I will check my glucose as shown and record it."
21.Kinderverpleegkunde

Antwoord

C · Intramusculaire injecties en rectale temperaturen zo mogelijk vermijden
22.Kinderverpleegkunde

Antwoord: D · Listen calmly, praise her for telling, explain it must be shared, and follow safeguarding

Waarom dit het antwoord is

The nurse listens without showing shock, reassures the child, avoids leading questions, records her words accurately and explains that people who can help need to know. The concern is reported at once through the safeguarding procedure.

Antwoord

D · Listen calmly, praise her for telling, explain it must be shared, and follow safeguarding
23.Psychiatrische verpleegkunde

Antwoord: C · Let the tablet dissolve completely under the tongue without chewing or swallowing it

Waarom dit het antwoord is

Buprenorphine is poorly absorbed if swallowed, so it is dissolved under the tongue. Naloxone is added to discourage injection, because it is poorly absorbed sublingually but active if injected.

Antwoord

C · Let the tablet dissolve completely under the tongue without chewing or swallowing it
24.Psychiatrische verpleegkunde

Elk item in mL

2½ tsp = 2,5·5 = 12,5 mL
1½ kopjes = 1,5·240 = 360 mL
3 fl oz = 3·30 = 90 mL
2 tbsp = 2·15 = 30 mL

Optellen

V = 12,5 + 360 + 90 + 30 = 492,5 mL

Antwoord

V = 492,5 mL
27.Dosisberekeningen

Antwoord: V = 4,6 mL

D = 2·23 = 46 mg

Volume

V = 46/10 = 4,6 mL

Antwoord

V = 4,6 mL
28.Dosisberekeningen

Antwoord: V₁ = 10 mL

Solution strengths: a percentage strength is grams per 100 mL, so 1% = 1 g/100 mL = 10 mg/mL. A ratio strength 1:1000 means 1 g in 1000 mL = 1 mg/mL; 1:10 000 is 0,1 mg/mL. For a dilution the amount of drug stays the same: C₁·V₁ = C₂·V₂.

Volume voorraadoplossing

V₁ = 1·250/25 = 10 mL

Antwoord

V₁ = 10 mL
29.Infuussnelheden

Antwoord: R = 155 mL/h

Infusion rates: a pump is set in mL/h = volume/time in hours. A gravity drip is counted in drops per minute: gtt/min = volume (mL) × drop factor (gtt/mL)/time (min). The drop factor is printed on the tubing package: macrodrip sets deliver 10, 15 or 20 gtt/mL (blood sets often 10 gtt/mL in the US, 15 in the UK) and microdrip sets 60 gtt/mL. Always use the drop factor the question or package gives.

Oorspronkelijke snelheid

1000/8 = 125 mL/h

Nieuwe snelheid

R = 930/6 = 155 mL/h

Antwoord

R = 155 mL/h

Veel ziekenhuizen staan zo’n aanpassing tot 25% toe zonder nieuw voorschrift; controleer het lokale protocol.

30.Infuussnelheden

Antwoord: D = 150 mg/h

Concentratie

250/50 = 5 mg/mL

Dosissnelheid

D = 5·30 = 150 mg/h

Antwoord

D = 150 mg/h
31.Infuusvloeistoffen en vochtbalans

Antwoord: I = 1310 mL

oraal: 120 + 240 + 250 = 610 mL
Infuus: 50·12 = 600 mL
intraveneus antibioticum: 2·50 = 100 mL
I = 1310 mL

Antwoord

I = 1310 mL
32.Infuusvloeistoffen en vochtbalans

Antwoord: V = 580 mL

Verstreken tijd

12:45 → 19:45 = 7 h

Toegediend volume

60·7 = 420 mL

Resterend volume

V = 1000 − 420 = 580 mL

Antwoord

V = 580 mL
33.Klinische beoordelingsscores

Antwoord: M = 2

Motorische score

M = 7 − 2 − 3 = 2

Antwoord

M = 2
34.Klinische beoordelingsscores

Antwoord: laag-matig

Score elke parameter

RR 16/min → 0
SpO₂ 97% → 0
op kamerlucht → 0
SBP 112 mmHg → 0
HR 38/min → 3
alert (A) → 0
T 36,2 °C → 0

Totaal en rode scores

NEWS2 = 0 + 0 + 0 + 0 + 3 + 0 + 0 = 3
Ten minste één parameter scoort 3.

Risicogroep

laag-matig

Antwoord

laag-matig
