1Formules en methoden

Infectiepreventie en patiëntveiligheid

Methode

Metrische stappen
1 kg = 1000 g, 1 g = 1000 mg, 1 mg = 1000 mcg, 1 L = 1000 mL
Gewicht en lengte
1 kg = 2,2 lb, 1 lb = 16 oz, 1 in = 2,54 cm, 1 ft = 12 in
Huishoudelijke maten
1 tsp = 5 mL, 1 tbsp = 15 mL, 1 fl oz = 30 mL, 1 cup = 8 fl oz = 240 mL
Temperatuur
°C = (°F − 32)·5/9, °F = °C·9/5 + 32
24-uursklok
PM-uren 1–11: tel er 12 bij op; 12 AM = 00:xx; 12 PM = 12:xx
Dosissnelheid
mcg/min × 60/1000 = mg/h
Laboratoriumfactoren
glucose 1 mmol/L = 18 mg/dL; creatinine 1 mg/dL = 88,4 µmol/L; Hb 1 g/dL = 10 g/L

Methode

Dosisberekeningen

Dosis uit de concentratie
volume = dosis / concentratie
Dosis per kilogram lichaamsgewicht
dosis = mg/kg × lichaamsgewicht
Tabletten
aantal = dosis / sterkte van een tablet
Eenheden
1 g = 1000 mg, 1 mg = 1000 mcg

Methode

Infuussnelheden

Pompsnelheid
mL/h = volume (mL)/time (h)
Druppelsnelheid
drops/min = volume (mL) × drop factor / time (min)
Inlooptijd
time (h) = volume (mL)/rate (mL/h)

Methode

4 mL/kg (eerste 10 kg) + 2 mL/kg (volgende 10 kg) + 1 mL/kg (elke kg boven 20)
Holliday–Segar per dag
100 / 50 / 20 mL/kg voor dezelfde gewichtsklassen
Vochtbalans
intake − output
IJsschilfers
tellen voor de helft van hun volume
Urineproductie
mL/kg/h = mL / (kg × h)
Inlooptijd
tijd (h) = resterend volume (mL) / snelheid (mL/h)
Toegediend volume
mL = snelheid × tijd

Klinische beoordelingsscores

5 tekens × 0–2 = 0–10; 7–10 stelt gerust, 4–6 matig afwijkend, 0–3 laag
Bradenschaal
5 × (1–4) + 1 × (1–3) = 6–23; ≤ 18 risico
Morse Fall Scale
vallen 25, diagnose 15, hulpmiddel 15/30, IV 20, looppatroon 10/20, mentale toestand 15
NEWS2-risico
0–4 laag; enkele 3 laag-matig; 5–6 matig; ≥ 7 hoog
Gemiddelde arteriële druk
MAP = (SBP + 2·DBP)/3 = DBP + PP/3
Polsdruk en shockindex
PP = SBP − DBP, SI = HR/SBP
Body mass index
BMI = kg/m² = 703·lb/in²

Methode

  5. Gebruik voor afgeleide waarden MAP = (SBP + 2·DBP)/3, PP = SBP − DBP, SI = HR/SBP en BMI = kg/m².

3Oplossingen

1.Basisverpleegkunde

Antwoord: A · The right supervision

Waarom dit het antwoord is

The five rights of delegation are the right task, circumstance, person, direction or communication, and supervision or evaluation. The registered nurse stays accountable for the decision to delegate.

Antwoord

A · The right supervision
2.Basisverpleegkunde

Antwoord: B · Impaired skin integrity related to immobility, as shown by a sacral wound

Waarom dit het antwoord is

A PES statement gives the nursing problem, the related cause and the defining signs. Pneumonia is a medical diagnosis, turning is an intervention, and a cause cannot repeat the problem.

Antwoord

B · Impaired skin integrity related to immobility, as shown by a sacral wound
3.Infectiepreventie en patiëntveiligheid

Antwoord: C · Do not call medicine candy, because the child may take it when no one is watching

Waarom dit het antwoord is

Calling medicine candy encourages children to look for it and swallow it, which can lead to poisoning. Parents are taught to call medicine medicine and keep it locked away.

Antwoord

C · Do not call medicine candy, because the child may take it when no one is watching
4.Infectiepreventie en patiëntveiligheid

Antwoord: D · Lead poisoning from old paint dust or chips

Waarom dit het antwoord is

Lead-based paint in older housing is the main source of lead exposure in young children, who put dust and chips in their mouths. Lead can cause anemia, behavior changes and developmental problems, and a blood lead level confirms it.

Antwoord

D · Lead poisoning from old paint dust or chips
5.Prioriteren en delegeren

Antwoord: D · Hold the warfarin dose and inform the provider

Waarom dit het antwoord is

An INR well above target raises the risk of serious bleeding. The nurse holds the dose, informs the provider and watches for bleeding. Without bleeding, holding warfarin is usually enough at this level; vitamin K is mainly used for very high INRs or bleeding.

Antwoord

D · Hold the warfarin dose and inform the provider
6.Prioriteren en delegeren

Antwoord: C · Doing suctioning, turning and bathing all at the same time

Waarom dit het antwoord is

Clustering several stimulating activities can cause a sustained rise in intracranial pressure. Spacing care, keeping the neck straight and a calm environment help keep it down.

Antwoord

C · Doing suctioning, turning and bathing all at the same time
7.Klinische verpleegkunde

Antwoord: A · Airway, breathing, circulation and level of consciousness

Waarom dit het antwoord is

Early postoperative assessment checks the airway, breathing, oxygen saturation, circulation, consciousness, pain, wound and drains. This detects anaesthetic and surgical complications early.

Antwoord

A · Airway, breathing, circulation and level of consciousness
8.Klinische verpleegkunde

Vóór middernacht

17:05 → 24:00 = 415 min

Na middernacht

00:00 → 06:30 = 390 min

Totaal

415 + 390 = 805 min
t = 805/60 = 13,42 h

Antwoord

t = 13,42 h
27.Dosisberekeningen

Antwoord: V = 7,2 mL

D = 15·24 = 360 mg

Volume

V = 360/50 = 7,2 mL

Antwoord

V = 7,2 mL
28.Dosisberekeningen

Antwoord: V = 0,82 mL

Dezelfde eenheden

2 mg/mL = 2000 mcg/mL

Volume

V = 1640/2000 = 0,82 mL

Antwoord

V = 0,82 mL
29.Infuussnelheden

Antwoord: R = 14 gtt/min

Tijd in minuten

3·60 = 180 min

Druppelsnelheid

R = 250·10/180 = 13,89 ≈ 14 gtt/min

Antwoord

R = 14 gtt/min

30.Infuussnelheden

Antwoord: D = 266,7 mcg/min

Dose-based infusions: concentration = amount of drug in the bag/bag volume. Pump rate (mL/h) = dose per hour/concentration, and dose per hour = mL/h × concentration. Convert per-minute doses to per-hour (× 60) and keep the mass units the same (1 g = 1000 mg, 1 mg = 1000 mcg).

Concentratie

400000 mcg/500 mL = 800 mcg/mL

Dosis per uur

20·800 = 16000 mcg/h

Dosis per minuut

D = 16000/60 = 266,7 mcg/min

Antwoord

D = 266,7 mcg/min
31.Infuusvloeistoffen en vochtbalans

Antwoord: I = 1220 mL

oraal: 120 + 200 = 320 mL
Infuus: 100·8 = 800 mL
intraveneus antibioticum: 2·50 = 100 mL
I = 1220 mL

Antwoord

I = 1220 mL
32.Infuusvloeistoffen en vochtbalans

Antwoord: V = 400 mL

Verstreken tijd

13:30 → 21:30 = 8 h

Toegediend volume

75·8 = 600 mL

Resterend volume

V = 1000 − 600 = 400 mL

Antwoord

V = 400 mL
33.Klinische beoordelingsscores

Antwoord: NEWS2 = 7

Score elke parameter

RR 26/min → 3
SpO₂ 93% → 2
met zuurstof → 2
SBP 130 mmHg → 0
HR 88/min → 0
alert (A) → 0
T 36,3 °C → 0

Optellen

NEWS2 = 3 + 2 + 2 + 0 + 0 + 0 + 0 = 7

Antwoord

NEWS2 = 7
