Antwoord
A · Dehydration from fluid loss
3.Infectiepreventie en patiëntveiligheid
Antwoord: C · Let it dry fully and do not touch the site again
Waarom dit het antwoord is
Chlorhexidine-alcohol must dry completely to kill organisms effectively. Touching the site after cleaning contaminates it, so the vein is located before prepping or with a sterile glove.
Antwoord
C · Let it dry fully and do not touch the site again
4.Infectiepreventie en patiëntveiligheid
Antwoord: A · Weaknesses in several layers of defense line up at the same time

Antwoord
D · Metformin
10.Laboratoriumwaarden en elektrolyten
Antwoord: B · Bloeding
Antwoord
B · Bloeding
11.Farmacologie
Antwoord: A · A modified-release (extended-release) tablet

Antwoord
A · A modified-release (extended-release) tablet
12.Farmacologie
Antwoord: B · Raising sodium too fast can cause osmotic demyelination in the brain
Waarom dit het antwoord is
In chronic hyponatraemia the brain adapts to low osmolality. Correcting sodium too rapidly shrinks brain cells and can cause osmotic demyelination (central pontine myelinolysis), so sodium is checked often.
Antwoord
B · Raising sodium too fast can cause osmotic demyelination in the brain

Antwoord: A · Potassium and magnesium
Waarom dit het antwoord is
Low potassium and low magnesium both increase ventricular irritability and the risk of VT and VF after an MI. They are checked and corrected, usually keeping potassium in the upper part of the normal range. The patient is also assessed for ischaemia and haemodynamic change.
Antwoord
A · Potassium and magnesium
15.Medisch-chirurgisch: neurologie, endocrinologie en nieren
Antwoord: A · Avoid blowing the nose, straining and bending forward
Waarom dit het antwoord is

Antwoord
D · Their uterus contracts and relaxes more irregularly
20.Verloskundige verpleegkunde
Antwoord: C · Call for urgent help and start fluid resuscitation
Waarom dit het antwoord is
Tachycardia, hypotension and cool peripheries indicate hypovolaemic shock. Help is called, IV access secured, fluids and blood given as prescribed, and the cause of bleeding treated.
Antwoord
C · Call for urgent help and start fluid resuscitation
21.Kinderverpleegkunde
Antwoord: D · Jaundice, because the breakdown of the trapped blood adds to bilirubin
Waarom dit het antwoord is
A cephalohaematoma is a collection of blood under the periosteum. As the red cells break down, bilirubin rises, so these babies have a higher risk of jaundice. The swelling itself usually resolves over weeks without treatment.
Antwoord
D · Jaundice, because the breakdown of the trapped blood adds to bilirubin
22.Kinderverpleegkunde
Antwoord: A · He may feel worried, jealous or left out and needs attention and information
Waarom dit het antwoord is
Siblings of seriously ill children often feel neglected, anxious, guilty or jealous and may show this through behaviour changes. Family-centred care includes giving siblings age-appropriate information, involvement and time with parents.
Antwoord
A · He may feel worried, jealous or left out and needs attention and information
23.Psychiatrische verpleegkunde
Antwoord: D · Understand, retain and weigh the information, or communicate their decision
Waarom dit het antwoord is
The Act's functional test asks whether the person can understand, retain, use or weigh the information, and communicate a decision. Making an unwise decision does not by itself mean a person lacks capacity.
Antwoord
D · Understand, retain and weigh the information, or communicate their decision
24.Psychiatrische verpleegkunde
Antwoord: C · Poor sleep, withdrawal from others and increasing suspiciousness
Waarom dit het antwoord is
Early warning signs of relapse include sleep disturbance, social withdrawal, irritability, suspiciousness and neglect of self-care. Recognising them early allows treatment to be reviewed before a full relapse.
Antwoord
C · Poor sleep, withdrawal from others and increasing suspiciousness
25.Verpleegkundig rekenen en eenhedenomrekeningen
Antwoord: m = 1,9 kg
3 oz/16 = 0,1875 lb
4 + 0,1875 = 4,188 lb
Pond naar kilogram
m = 4,188/2,2 = 1,903 ≈ 1,9 kg
Antwoord
m = 1,9 kg
26.Verpleegkundig rekenen en eenhedenomrekeningen
Antwoord: V = 105 mL
1½ tsp = 1,5·5 = 7,5 mL
Hele kuur
V = 7,5 mL × 2/dag × 7 = 105 mL
Antwoord
V = 105 mL
27.Dosisberekeningen
Antwoord: V = 7 mL
D = 5·14 = 70 mg
Volume
V = 70/10 = 7 mL
Antwoord
V = 7 mL
28.Dosisberekeningen
Antwoord: W = 240 mL
Solution strengths: a percentage strength is grams per 100 mL, so 1% = 1 g/100 mL = 10 mg/mL. A ratio strength 1:1000 means 1 g in 1000 mL = 1 mg/mL; 1:10 000 is 0,1 mg/mL. For a dilution the amount of drug stays the same: C₁·V₁ = C₂·V₂.
Volume voorraadoplossing
V₁ = 1·250/25 = 10 mL
Toe te voegen water
W = 250 − 10 = 240 mL
Antwoord
W = 240 mL
29.Infuussnelheden
Antwoord: R = 140 mL/h
Infusion rates: a pump is set in mL/h = volume/time in hours. A gravity drip is counted in drops per minute: gtt/min = volume (mL) × drop factor (gtt/mL)/time (min). The drop factor is printed on the tubing package: macrodrip sets deliver 10, 15 or 20 gtt/mL (blood sets often 10 gtt/mL in the US, 15 in the UK) and microdrip sets 60 gtt/mL. Always use the drop factor the question or package gives.
Oorspronkelijke snelheid
1500/12 = 125 mL/h
Nieuwe snelheid
R = 280/2 = 140 mL/h
Antwoord
R = 140 mL/h
Veel ziekenhuizen staan zo’n aanpassing tot 25% toe zonder nieuw voorschrift; controleer het lokale protocol.
30.Infuussnelheden
Antwoord: D = 16 mg/h
Concentratie
400/500 = 0,8 mg/mL
Dosissnelheid
D = 0,8·20 = 16 mg/h
Antwoord
D = 16 mg/h
31.Infuusvloeistoffen en vochtbalans
Antwoord: I = 1530 mL
oraal: 100 + 120 + 150 = 370 mL
ijsschilfers: 120/2 = 60 mL
Infuus: 125·8 = 1000 mL
intraveneus antibioticum: 1·100 = 100 mL
I = 1530 mL
Antwoord
I = 1530 mL
32.Infuusvloeistoffen en vochtbalans
Antwoord: t = 14 h
Eerste deel
100·4 = 400 mL
1000 − 400 = 600 mL over
Tweede deel
600/60 = 10 h
Totale tijd
t = 4 + 10 = 14 h
Antwoord
t = 14 h
Dat is ongeveer 14 h 00 min, dus de zak is rond 01:00 leeg.
33.Klinische beoordelingsscores
Antwoord: SI = 0,45
Bloeddrukwaarden: polsdruk PP = SBP − DBP.
Shockindex
SI = 54/120 = 0,45
Antwoord
SI = 0,45
Binnen of onder de gebruikelijke 0,5–0,7.
34.Klinische beoordelingsscores
Antwoord: hoog risico
Morse Fall Scale: zes items. IV-therapie of zoutslot: nee 0, ja 20.
