1Formules en methoden

Infectiepreventie en patiëntveiligheid

Methode

Metrische stappen
1 kg = 1000 g, 1 g = 1000 mg, 1 mg = 1000 mcg, 1 L = 1000 mL
Gewicht en lengte
1 kg = 2,2 lb, 1 lb = 16 oz, 1 in = 2,54 cm, 1 ft = 12 in
Huishoudelijke maten
1 tsp = 5 mL, 1 tbsp = 15 mL, 1 fl oz = 30 mL, 1 cup = 8 fl oz = 240 mL
Temperatuur
°C = (°F − 32)·5/9, °F = °C·9/5 + 32
24-uursklok
PM-uren 1–11: tel er 12 bij op; 12 AM = 00:xx; 12 PM = 12:xx
Dosissnelheid
mcg/min × 60/1000 = mg/h
Laboratoriumfactoren
glucose 1 mmol/L = 18 mg/dL; creatinine 1 mg/dL = 88,4 µmol/L; Hb 1 g/dL = 10 g/L

Methode

Dosisberekeningen

Dosis uit de concentratie
volume = dosis / concentratie
Dosis per kilogram lichaamsgewicht
dosis = mg/kg × lichaamsgewicht
Tabletten
aantal = dosis / sterkte van een tablet
Eenheden
1 g = 1000 mg, 1 mg = 1000 mcg

Methode

Infuussnelheden

Pompsnelheid
mL/h = volume (mL)/time (h)
Druppelsnelheid
drops/min = volume (mL) × drop factor / time (min)
Inlooptijd
time (h) = volume (mL)/rate (mL/h)

Methode

4 mL/kg (eerste 10 kg) + 2 mL/kg (volgende 10 kg) + 1 mL/kg (elke kg boven 20)
Holliday–Segar per dag
100 / 50 / 20 mL/kg voor dezelfde gewichtsklassen
Vochtbalans
intake − output
IJsschilfers
tellen voor de helft van hun volume
Urineproductie
mL/kg/h = mL / (kg × h)
Inlooptijd
tijd (h) = resterend volume (mL) / snelheid (mL/h)
Toegediend volume
mL = snelheid × tijd

3Oplossingen

1.Basisverpleegkunde

Antwoord: A · Air flows into the room, so airborne particles do not escape into the corridor

Waarom dit het antwoord is

Negative pressure draws air into the room and out through filtered exhaust. It is used for airborne infections such as tuberculosis; positive pressure is used to protect patients with very weak immunity.

Antwoord

A · Air flows into the room, so airborne particles do not escape into the corridor
2.Basisverpleegkunde

Antwoord: C · A postoperative patient with SpO₂ 89% on air and a respiratory rate of 28

Waarom dit het antwoord is

Low saturation with a high respiratory rate shows a breathing problem that could worsen quickly. The other needs are real but not urgent.

Antwoord

C · A postoperative patient with SpO₂ 89% on air and a respiratory rate of 28
3.Infectiepreventie en patiëntveiligheid

Antwoord: C · Limit time close to the patient and handle body fluids per radiation policy

Waarom dit het antwoord is

Radioactive iodine is excreted in urine, sweat and saliva, so the patient is usually in a private room with their own bathroom. Staff limit time near them, use distance and follow policy for body fluids and waste.

Antwoord

C · Limit time close to the patient and handle body fluids per radiation policy
4.Infectiepreventie en patiëntveiligheid

Antwoord: D · Keep everyone inside, close windows and doors and shut off outside air intake

Waarom dit het antwoord is

Sheltering in place keeps people inside, away from the hazard. Windows and doors are closed and ventilation systems that bring in outside air are shut off until the all-clear.

Antwoord

D · Keep everyone inside, close windows and doors and shut off outside air intake
5.Prioriteren en delegeren

Antwoord: A · “I add extra salt to my food so it tastes better.”

Waarom dit het antwoord is

Salt causes fluid retention and can worsen heart failure. Daily weights, reporting rapid weight gain and taking diuretics in the morning are all correct.

Antwoord

A · “I add extra salt to my food so it tastes better.”
6.Prioriteren en delegeren

Antwoord: D · That the child is passing urine

Waarom dit het antwoord is

Potassium is not added to IV fluids until the child is passing urine, because poor kidney function can let potassium build up to dangerous levels.

D · That the child is passing urine
7.Klinische verpleegkunde

Antwoord: A · Hyperosmolaire hyperglykemische toestand

Antwoord

A · Hyperosmolaire hyperglykemische toestand
8.Klinische verpleegkunde

Antwoord: B · Bijniercrisis

Antwoord

B · Bijniercrisis
9.Laboratoriumwaarden en elektrolyten

Antwoord: A · Keep taking insulin and check blood or urine ketones

Waarom dit het antwoord is

During illness, stress hormones raise glucose, so insulin is continued even when eating less. Glucose and ketones are checked often, fluids are taken, and the provider is contacted if ketones are moderate or high or vomiting prevents fluids.Gecontroleerd met: Diabetes - when you are sick · MedlinePlus (NIH) (2026)

Antwoord

A · Keep taking insulin and check blood or urine ketones
10.Laboratoriumwaarden en elektrolyten

Antwoord: C · Low albumin raises the free (active) fraction of phenytoin

Waarom dit het antwoord is

Phenytoin is highly protein bound, and only the free drug is active. When albumin is low, more phenytoin is free, so toxicity can occur with a "normal" total level. A free phenytoin level or a corrected level helps.Gecontroleerd met: Phenytoin · StatPearls (NCBI Bookshelf) (2023); Antiepileptic Drug Monitoring · StatPearls (NCBI Bookshelf) (2025)

Antwoord

C · Low albumin raises the free (active) fraction of phenytoin
11.Farmacologie

Antwoord: C · Take it with vitamin C, such as orange juice, on an empty stomach

Waarom dit het antwoord is

Vitamin C keeps iron in its more absorbable ferrous form. Milk, antacids, tea and some antibiotics reduce iron absorption. If the stomach becomes upset, it may be taken with food, accepting lower absorption.

Antwoord

C · Take it with vitamin C, such as orange juice, on an empty stomach
12.Farmacologie

Antwoord: D · Doses should be given on time at evenly spaced intervals

Waarom dit het antwoord is

For beta-lactams, efficacy depends on the time the drug level stays above the minimum inhibitory concentration. Evenly spaced, on-time doses, or extended infusions, support this.

Antwoord

D · Doses should be given on time at evenly spaced intervals
13.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: C · One shock and well: contact the device clinic; repeated shocks or unwell: call emergency services

Waarom dit het antwoord is

A single shock with full recovery usually needs a call to the device team. Several shocks in a short time, or a shock with chest pain, breathlessness or collapse, needs emergency help. Patients should not drive themselves after a shock.

Antwoord

C · One shock and well: contact the device clinic; repeated shocks or unwell: call emergency services
14.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: B · Stop or clamp the drainage and inform the doctor

Waarom dit het antwoord is

Rapid removal of a large effusion can cause re-expansion pulmonary oedema. Coughing, chest tightness or pain are warning signs. Drainage is stopped and the doctor is informed, and the volume is usually drained in a controlled way.

Antwoord

B · Stop or clamp the drainage and inform the doctor
15.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: A · Alcohol can cause hypos, even the next day, so eat carbs with it

Waarom dit het antwoord is

Alcohol reduces the liver's release of glucose, so it increases the risk of hypoglycaemia, which may be delayed for many hours. Patients should eat carbohydrate, check glucose before bed and tell friends that a hypo can look like drunkenness.

Antwoord

A · Alcohol can cause hypos, even the next day, so eat carbs with it

Antwoord

B · Uitdroging met laag kalium en natrium
17.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: C · To stop the inner bumper becoming buried in the stomach wall

Waarom dit het antwoord is

Buried bumper syndrome occurs when the internal fixator embeds in the gastric wall. Following local guidance, the tube is gently advanced, rotated and returned once healed, and the external fixator is kept slightly loose.

Antwoord

C · To stop the inner bumper becoming buried in the stomach wall
18.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: D · Vitamin B12 deficiency and bile-acid diarrhoea

Waarom dit het antwoord is

The terminal ileum absorbs vitamin B12 and bile acids. After resection, B12 replacement may be needed, and unabsorbed bile acids reaching the colon cause diarrhoea.

Antwoord

D · Vitamin B12 deficiency and bile-acid diarrhoea
19.Verloskundige verpleegkunde

Antwoord: C · Redness, warmth and discharge

Waarom dit het antwoord is

Redness spreading from the incision, warmth, swelling, purulent discharge, gaping edges and fever suggest infection. Numbness near the scar is common and not a sign of infection.

Antwoord

C · Redness, warmth and discharge
20.Verloskundige verpleegkunde

Antwoord: C · Uterine inversion

Waarom dit het antwoord is

An absent fundus on palpation with a mass in the vagina and shock suggest uterine inversion. Help is called immediately, oxytocin is stopped and the uterus is replaced as soon as possible by the medical team.

Antwoord

C · Uterine inversion
21.Kinderverpleegkunde

Antwoord: A · The child's agreement to care, sought alongside the parents' consent

Waarom dit het antwoord is

Assent is the child's own agreement, in language suited to their age, alongside the legal consent given by parents. Seeking assent respects the child and builds cooperation and trust.

Antwoord

A · The child's agreement to care, sought alongside the parents' consent
22.Kinderverpleegkunde

Antwoord: B · Allow them to stay if they wish, with a staff member to support and explain

Waarom dit het antwoord is

Many families want to be present, and guidelines support offering this with a dedicated staff member to explain and support them. Family presence can help with grief and does not generally interfere with resuscitation.

Antwoord

B · Allow them to stay if they wish, with a staff member to support and explain
23.Psychiatrische verpleegkunde

Antwoord: A · Keep to his usual routines where possible and prepare him for any changes

Waarom dit het antwoord is

Predictability reduces distress. Nurses keep routines, use clear and concrete language, reduce sensory stimulation, use visual supports and work closely with the parents, who know the child best.

Antwoord

A · Keep to his usual routines where possible and prepare him for any changes
24.Psychiatrische verpleegkunde

Antwoord: D · Explain that these "baby blues" are common, offer support and advise her to seek help if they last more than about 2 weeks

Waarom dit het antwoord is

Baby blues are common in the first days after birth and usually resolve within 2 weeks. Symptoms that last longer or are severe may indicate postpartum depression and need assessment.

Antwoord

D · Explain that these "baby blues" are common, offer support and advise her to seek help if they last more than about 2 weeks
25.Verpleegkundig rekenen en eenhedenomrekeningen

Antwoord: m = 2,02 kg

7 oz/16 = 0,4375 lb
4 + 0,4375 = 4,438 lb

Pond naar kilogram

m = 4,438/2,2 = 2,017 ≈ 2,02 kg

Antwoord

m = 2,02 kg
26.Verpleegkundig rekenen en eenhedenomrekeningen

Antwoord: t = 3,42 h

20:50 → 24:00 = 190 min

Na middernacht

00:00 → 00:15 = 15 min

Totaal

190 + 15 = 205 min
t = 205/60 = 3,417 ≈ 3,42 h

Antwoord

t = 3,42 h
27.Dosisberekeningen

Antwoord: D = 90 mg

Dosis

2·45 = 90 mg

Antwoord

D = 90 mg
28.Dosisberekeningen

Antwoord: V₁ = 10 mL

Solution strengths: a percentage strength is grams per 100 mL, so 1% = 1 g/100 mL = 10 mg/mL. A ratio strength 1:1000 means 1 g in 1000 mL = 1 mg/mL; 1:10 000 is 0,1 mg/mL. For a dilution the amount of drug stays the same: C₁·V₁ = C₂·V₂.

Volume voorraadoplossing

V₁ = 0,5·1000/50 = 10 mL

Antwoord

V₁ = 10 mL
29.Infuussnelheden

Antwoord: R = 123,3 mL/h

Infusion rates: a pump is set in mL/h = volume/time in hours. A gravity drip is counted in drops per minute: gtt/min = volume (mL) × drop factor (gtt/mL)/time (min). The drop factor is printed on the tubing package: macrodrip sets deliver 10, 15 or 20 gtt/mL (blood sets often 10 gtt/mL in the US, 15 in the UK) and microdrip sets 60 gtt/mL. Always use the drop factor the question or package gives.

Oorspronkelijke snelheid

1000/10 = 100 mL/h

Nieuwe snelheid

R = 370/3 = 123,3 mL/h

Antwoord

R = 123,3 mL/h

Veel ziekenhuizen staan zo’n aanpassing tot 25% toe zonder nieuw voorschrift; controleer het lokale protocol.

30.Infuussnelheden

Antwoord: t = 15 h

Snelheid in mL/h

20·60/20 = 60 mL/h

Resterende tijd

t = 900/60 = 15 h

Antwoord

t = 15 h
31.Infuusvloeistoffen en vochtbalans

Antwoord: I = 1300 mL

oraal: 150 + 200 + 150 = 500 mL
Infuus: 50·12 = 600 mL
intraveneus antibioticum: 2·100 = 200 mL
I = 1300 mL

Antwoord

I = 1300 mL
32.Infuusvloeistoffen en vochtbalans

Antwoord: t = 18 h

Eerste deel

100·2 = 200 mL
1000 − 200 = 800 mL over

Tweede deel

800/50 = 16 h

Totale tijd

t = 2 + 16 = 18 h

Antwoord

t = 18 h

Dat is ongeveer 18 h 00 min, dus de zak is rond 03:30 leeg.

33.Klinische beoordelingsscores

Antwoord: MFS = 115

Morse Fall Scale: zes items. Eerdere val: nee 0, ja 25. Secundaire diagnose: nee 0, ja 15.
