1Formules en methoden

Prioriteren en delegeren

Methode

Antwoord: C · To remove items that could be used to harm self or others

Waarom dit het antwoord is

Belongings are searched with the patient, according to policy, for items such as sharp objects, cords, drugs and lighters. These are stored safely to reduce the risk of self-harm or violence.

Antwoord

C · To remove items that could be used to harm self or others
4.Infectiepreventie en patiëntveiligheid

Antwoord: D · “I think he needs to be assessed now. Can you come within the next 15 minutes?”

Waarom dit het antwoord is

In SBAR, the situation names the problem, the background gives the history, the assessment shares the nurse’s findings and judgement, and the recommendation states a clear request with a time frame.

Antwoord

D · “I think he needs to be assessed now. Can you come within the next 15 minutes?”
5.Laboratoriumwaarden en elektrolyten

Antwoord: A · A common stress response to surgery that is monitored

Waarom dit het antwoord is

A mild leukocytosis is common after surgery because of tissue injury and the stress response. It is monitored along with temperature and wound assessment. A rising count after several days, or fever, suggests infection.

Antwoord

A · A common stress response to surgery that is monitored

7.Klinische verpleegkunde

Antwoord: C · Removing hair, if needed, with clippers rather than a razor

Waarom dit het antwoord is

Razors cause micro-abrasions that increase infection risk, so hair is removed only if necessary with clippers just before surgery. Keeping patients warm and good skin preparation also help.

Antwoord

C · Removing hair, if needed, with clippers rather than a razor
8.Klinische verpleegkunde

Antwoord: D · Symptoms can return later in a biphasic reaction

Waarom dit het antwoord is

Some patients have a second wave of symptoms hours after the first, called a biphasic reaction. Observation, an adrenaline auto-injector where indicated and allergy referral are part of safe discharge.

Antwoord

D · Symptoms can return later in a biphasic reaction
9.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: D · To prevent peripheral neuropathy

Waarom dit het antwoord is

Isoniazid increases pyridoxine loss, which can cause peripheral neuropathy, especially in people with diabetes, alcohol dependence, malnutrition or HIV. Pyridoxine prevents this. Isoniazid can also cause hepatitis. Ethambutol affects vision; pyrazinamide can cause gout.

Antwoord

D · To prevent peripheral neuropathy
10.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: A · Withhold the next doses and report urgently for liver tests

Waarom dit het antwoord is

Isoniazid, rifampicin and pyrazinamide can all cause hepatotoxicity. Jaundice, dark urine and nausea suggest drug-induced hepatitis, so the drugs are stopped and liver function is checked urgently. Orange urine from rifampicin does not cause yellow eyes. Alcohol increases the risk.

Antwoord

A · Withhold the next doses and report urgently for liver tests
11.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: A · A weight gain of 2 kg (4,4 lb) in two days with ankle swelling

Waarom dit het antwoord is

Rapid weight gain reflects fluid retention. Oedema, breathlessness, crackles in the lungs and a raised blood pressure are other signs. Daily weights are a simple and reliable way to track fluid balance.

Antwoord

A · A weight gain of 2 kg (4,4 lb) in two days with ankle swelling
12.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: D · A patient with Guillain–Barré syndrome whose vital capacity is falling

Waarom dit het antwoord is

A falling vital capacity and weak cough in Guillain–Barré syndrome warn of respiratory failure, which may need ventilation. The other patients have expected or less urgent problems.

Antwoord

D · A patient with Guillain–Barré syndrome whose vital capacity is falling
13.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: A · Fistulas between the bowel and other organs

Waarom dit het antwoord is

Transmural inflammation in Crohn's disease can create fistulas, abscesses and strictures. Ulcerative colitis is mucosal, and removing the colon and rectum is curative.

Antwoord

A · Fistulas between the bowel and other organs
14.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: A · Refeeding syndrome

Waarom dit het antwoord is

Refeeding syndrome causes low phosphate, potassium and magnesium with fluid retention, heart failure and muscle weakness. The team slows the feed, replaces electrolytes, gives thiamine and monitors closely.

Antwoord

A · Refeeding syndrome
