1Formules en methoden

Prioriteren en delegeren

Methode

3Oplossingen

1.Prioriteren en delegeren

Antwoord: D · Relieve pressure on the area by changing position regularly

Waarom dit het antwoord is

Non-blanching redness is a stage 1 pressure injury. Relieving pressure through repositioning and support surfaces is the priority; massage can damage tissue further.

Antwoord

D · Relieve pressure on the area by changing position regularly
2.Prioriteren en delegeren

Antwoord: D · Inserting a tube or suction catheter through the nose

Waarom dit het antwoord is

Clear nasal drainage after a head injury can be cerebrospinal fluid from a basal skull fracture. Nasal tubes could enter the brain and introduce infection, so they are avoided.

Antwoord

D · Inserting a tube or suction catheter through the nose
3.Infectiepreventie en patiëntveiligheid

Antwoord: C · It can burn through the lining of the esophagus within hours

Waarom dit het antwoord is

A button battery lodged in the esophagus can cause severe chemical burns within a few hours, so the child needs emergency assessment and often removal. It should never be treated as harmless.

Antwoord

C · It can burn through the lining of the esophagus within hours
4.Infectiepreventie en patiëntveiligheid

Antwoord: C · “I keep small rugs in the hall so my feet stay warm.”

Waarom dit het antwoord is

Loose rugs are a common cause of trips and falls and should be removed or secured. Night lights, grab bars and non-slip footwear all reduce fall risk.

Antwoord

C · “I keep small rugs in the hall so my feet stay warm.”
5.Laboratoriumwaarden en elektrolyten

Antwoord: D · Every few days, until it is stable

Waarom dit het antwoord is

When warfarin is started or its dose changes, the INR is checked frequently (often every few days) until it is stable in the target range. After that, the interval can be extended, often up to every 4 weeks or longer if very stable.

Antwoord

D · Every few days, until it is stable
6.Laboratoriumwaarden en elektrolyten

Antwoord: B · Severe C. difficile infection

Waarom dit het antwoord is

A white count of 15 × 10³/µL or more or a creatinine above 1,5 mg/dL marks severe C. difficile infection. These patients need prompt treatment, fluid replacement and close monitoring for complications such as toxic megacolon. Contact precautions with soap-and-water hand hygiene are used.

Antwoord

B · Severe C. difficile infection
7.Klinische verpleegkunde

Antwoord: A · Reorientation, glasses and hearing aids in use, and good sleep and hydration

Waarom dit het antwoord is

Multicomponent prevention, including orientation, sensory aids, hydration, nutrition, mobility, sleep and avoiding unnecessary moves or drugs, reduces delirium.

Antwoord

A · Reorientation, glasses and hearing aids in use, and good sleep and hydration
8.Klinische verpleegkunde

Antwoord: B · "Mr Patel's breathing rate is 8 a minute and he's hard to wake."

Waarom dit het antwoord is

A low respiratory rate with reduced consciousness suggests respiratory depression, for example from opioids. It needs immediate assessment and escalation.

Antwoord

B · "Mr Patel's breathing rate is 8 a minute and he's hard to wake."
9.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: B · A stiff ventricle that fills poorly in diastole

Waarom dit het antwoord is

In HFpEF, the left ventricle is stiff and relaxes poorly, so filling pressures rise and cause congestion even though the ejection fraction is normal. It is common in older people with hypertension, diabetes and obesity. HFrEF is mainly a problem of contraction.

Antwoord

B · A stiff ventricle that fills poorly in diastole

Antwoord: A · Skin cancer

Waarom dit het antwoord is

Transplant recipients have a much higher risk of skin cancers, especially squamous cell carcinoma, because of immunosuppression. Sun protection and regular skin checks are important.

Antwoord

A · Skin cancer
12.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: C · Focal impaired awareness seizure

Waarom dit het antwoord is

Focal impaired awareness seizures often come from the temporal lobe. They last about a minute or two, with automatisms such as lip smacking and fumbling, and are followed by confusion. Absence seizures are shorter and have no postictal phase.

Antwoord

C · Focal impaired awareness seizure

Antwoord: A · IV lipid emulsion

Waarom dit het antwoord is

These are signs of local anaesthetic systemic toxicity. The injection is stopped, help is called, the airway is secured and seizures are treated. IV lipid emulsion is the specific treatment, especially if the circulation is affected.

Antwoord

A · IV lipid emulsion
