1Formules en methoden

Prioriteren en delegeren

Methode

Antwoord: D · Limit time close to the patient and handle body fluids per radiation policy

Waarom dit het antwoord is

Radioactive iodine is excreted in urine, sweat and saliva, so the patient is usually in a private room with their own bathroom. Staff limit time near them, use distance and follow policy for body fluids and waste.

Antwoord

D · Limit time close to the patient and handle body fluids per radiation policy
4.Infectiepreventie en patiëntveiligheid

Antwoord: C · Ease the patient to the floor, protect the head and turn them on their side

Waarom dit het antwoord is

The nurse guides the patient gently to the floor, cushions the head, moves objects away and turns the patient on their side to protect the airway. The nurse stays, times the seizure and calls for help.

Antwoord

C · Ease the patient to the floor, protect the head and turn them on their side
5.Laboratoriumwaarden en elektrolyten

Antwoord: B · Vitamin K (phytonadione)

Waarom dit het antwoord is

Warfarin blocks vitamin K–dependent clotting factors, and vitamin K restores their production over hours. For serious bleeding, a prothrombin complex concentrate may also be given to replace factors at once. The warfarin is held.Gecontroleerd met: Label: WARFARIN SODIUM tablet · DailyMed (NLM/FDA) (2012)

Antwoord

B · Vitamin K (phytonadione)

Antwoord

B · Een natrium van 126 mEq/L
7.Klinische verpleegkunde

Antwoord: D · Take it on an empty stomach, usually before breakfast

Waarom dit het antwoord is

Levothyroxine is absorbed best on an empty stomach and is usually taken at the same time each morning. It is normally lifelong, with doses adjusted by thyroid blood tests.

Antwoord

D · Take it on an empty stomach, usually before breakfast
8.Klinische verpleegkunde

Antwoord: D · Delirium starts suddenly and fluctuates, while dementia develops gradually

Waarom dit het antwoord is

Delirium develops over hours to days, with fluctuating attention and consciousness, and is often reversible when the cause is treated. Dementia is a gradual, progressive decline. Patients with dementia are at higher risk of delirium.

Antwoord

D · Delirium starts suddenly and fluctuates, while dementia develops gradually
9.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: D · A swallowing assessment and sitting upright for meals

Waarom dit het antwoord is

Patients with dysphagia need a formal swallowing assessment, food and fluid textures that are safe for them, sitting upright during and after meals and close supervision. Good oral hygiene also reduces the bacterial load that can be aspirated.

Antwoord

D · A swallowing assessment and sitting upright for meals
10.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: D · Vasodilators can cause a severe fall in BP with a fixed outflow obstruction

Waarom dit het antwoord is

In severe aortic stenosis, cardiac output is fixed by the narrow valve. Vasodilators lower preload and afterload, and the heart cannot compensate, so the blood pressure can fall sharply and cause syncope. They are used with care and close monitoring.

Antwoord

D · Vasodilators can cause a severe fall in BP with a fixed outflow obstruction
11.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: D · Ask questions that can be answered with yes, no or a gesture

Waarom dit het antwoord is

In expressive aphasia, comprehension is largely preserved. Short, closed questions, extra time, gestures and picture or word boards help. Shouting does not help, and constantly finishing sentences can frustrate the patient.

Antwoord

D · Ask questions that can be answered with yes, no or a gesture
12.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: B · Weighing the patient before they go to dialysis

Waarom dit het antwoord is

Weighing a patient is a routine task suitable for a trained assistant. Assessing the access, making medication decisions and teaching require the registered nurse's knowledge and judgement.

Antwoord

B · Weighing the patient before they go to dialysis
13.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: A · They may need extra steroid cover to prevent an adrenal crisis

Waarom dit het antwoord is

Long-term steroids suppress the adrenal glands, so they cannot raise cortisol in response to surgical stress. Extra steroid cover may be prescribed, and steroids are not stopped suddenly. Healing may also be slower.

Antwoord

A · They may need extra steroid cover to prevent an adrenal crisis
14.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: B · An anastomotic leak

Waarom dit het antwoord is

Leakage at the join in the bowel causes peritonitis and sepsis, usually a few days after surgery. Faeculent drain fluid makes it very likely. It needs urgent surgical review, sepsis care and often return to theatre.

Antwoord

B · An anastomotic leak
