1Formules en methoden

Prioriteren en delegeren

Methode

3Oplossingen

1.Prioriteren en delegeren

Antwoord: B · Stop or slow the infusion and inform the provider

Waarom dit het antwoord is

Flushing of the upper body during vancomycin is usually an infusion reaction related to the rate. The nurse stops or slows the infusion, assesses for anaphylaxis and informs the provider.

Antwoord

B · Stop or slow the infusion and inform the provider
2.Prioriteren en delegeren

Antwoord: C · Atropine

Waarom dit het antwoord is

Atropine is the first-line drug for symptomatic bradycardia. Metoprolol, digoxin and diltiazem slow the heart and would make things worse; pacing may be needed if atropine does not work.

Antwoord

C · Atropine

Antwoord: B · A wrong-patient sample, so the test is repeated and the event reported

Waarom dit het antwoord is

A different blood group strongly suggests the sample came from another patient. A new sample is taken with careful bedside identification and labeling, and the event is reported as a serious near miss.

Antwoord

B · A wrong-patient sample, so the test is repeated and the event reported
5.Laboratoriumwaarden en elektrolyten

Antwoord: B · Group B Streptococcus

Waarom dit het antwoord is

Group B streptococci can colonize the vagina and rectum and can cause severe infection in newborns. Women who test positive receive IV antibiotics during labor to reduce transmission to the baby. The UK does not screen all women routinely; it uses a risk-based approach instead.

Antwoord

B · Group B Streptococcus
6.Laboratoriumwaarden en elektrolyten

Antwoord: A · Vitamin K deficiency

Waarom dit het antwoord is

Vitamin K comes from the diet and from gut bacteria. Poor intake plus antibiotics can cause vitamin K deficiency, which lowers factors II, VII, IX and X and prolongs the PT/INR. Vitamin K replacement corrects it.

Antwoord

A · Vitamin K deficiency
7.Klinische verpleegkunde

Antwoord: B · To prevent urine flowing back into the bladder

Waarom dit het antwoord is

Keeping the bag below the bladder allows drainage and prevents backflow, which reduces infection risk. The bag should also not touch the floor.

Antwoord

B · To prevent urine flowing back into the bladder
8.Klinische verpleegkunde

Antwoord: B · Strict hand hygiene by everyone who has contact with the patient

Waarom dit het antwoord is

Hand hygiene is the single most effective measure to protect neutropenic patients. Rectal procedures and IM injections are avoided because they can cause mucosal or tissue injury and bleeding.

Antwoord

B · Strict hand hygiene by everyone who has contact with the patient

Antwoord: C · Intravenous calcium gluconate or chloride

Waarom dit het antwoord is

Severe hyperkalaemia with ECG changes is an emergency. IV calcium stabilises the myocardium within minutes. Insulin with glucose and nebulised salbutamol shift potassium into cells, and the cause (here spironolactone and the ACE inhibitor) is stopped. Cardiac monitoring is essential.

Antwoord

C · Intravenous calcium gluconate or chloride
11.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: C · The injury is incomplete, which suggests a better chance of recovery

Waarom dit het antwoord is

Preserved sensation or movement in the lowest sacral segments (sacral sparing) means the injury is incomplete. Incomplete injuries have a much better chance of some functional recovery.

Antwoord

C · The injury is incomplete, which suggests a better chance of recovery
12.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: C · Repositioning the patient every two hours as set out in the care plan

Waarom dit het antwoord is

Repositioning to a set plan is a routine task that trained assistants can carry out. Swallow screening, assessing new symptoms and medication teaching need the nurse's clinical judgement and stay with the registered nurse.

Antwoord

C · Repositioning the patient every two hours as set out in the care plan
13.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: A · Do not use it; re-test later or arrange an X-ray as per policy

Waarom dit het antwoord is

A pH above 5,5 may mean the tube is in the lung or oesophagus, or the stomach acid is reduced by drugs or feed. Nothing is given until position is confirmed according to local policy.

Antwoord

A · Do not use it; re-test later or arrange an X-ray as per policy
14.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: B · It raises the risk of ketoacidosis, even with near-normal glucose

Waarom dit het antwoord is

SGLT2 inhibitors can cause euglycaemic diabetic ketoacidosis around fasting and surgery. They are usually paused before surgery as advised, and ketones are checked if the patient becomes unwell.

Antwoord

B · It raises the risk of ketoacidosis, even with near-normal glucose
