1Formules en methoden

Prioriteren en delegeren

Methode

3Oplossingen

1.Prioriteren en delegeren

Antwoord: B · Do not measure blood pressure on the left arm

Waarom dit het antwoord is

Blood pressure cuffs, tight bands and heavy loads can damage or block a fistula. The arm with the access is protected, and the nurse checks it for a thrill and bruit.

Antwoord

B · Do not measure blood pressure on the left arm
2.Prioriteren en delegeren

Antwoord: B · Thyroid storm

Waarom dit het antwoord is

Thyroid storm is a life-threatening surge of thyroid hormone, with fever, tachycardia and agitation or confusion. It needs urgent treatment; myxedema coma is the opposite extreme of hypothyroidism.

Antwoord

B · Thyroid storm
3.Infectiepreventie en patiëntveiligheid

Antwoord: B · The patient’s allergies, by checking the record and asking the patient

Waarom dit het antwoord is

Before any new drug, the nurse checks recorded allergies and asks the patient directly about past reactions. The patient is then watched closely after the first dose.

Antwoord

B · The patient’s allergies, by checking the record and asking the patient
4.Infectiepreventie en patiëntveiligheid

Antwoord: C · The source patient is tested per policy, and the nurse has baseline tests

Waarom dit het antwoord is

After first aid and reporting, the source patient is tested according to law and policy, and the nurse has baseline testing. A rapid risk assessment decides whether HIV prophylaxis is started while results are awaited.

Antwoord

C · The source patient is tested per policy, and the nurse has baseline tests
5.Laboratoriumwaarden en elektrolyten

Antwoord: D · It is always diluted and given by infusion pump, never as an IV push

Waarom dit het antwoord is

Concentrated potassium chloride given as an IV push can cause fatal cardiac arrest, so it is never pushed. It is diluted, mixed well and given by infusion pump at a rate set by protocol, often with cardiac monitoring at higher rates.

Antwoord

D · It is always diluted and given by infusion pump, never as an IV push
6.Laboratoriumwaarden en elektrolyten

Antwoord: C · G6PD deficiency with acute hemolysis

Waarom dit het antwoord is

G6PD deficiency makes red cells vulnerable to oxidative damage. Fava beans, some drugs (such as primaquine and some sulfonamides) and infections can trigger acute hemolysis, with dark urine, jaundice and anemia. Triggers are avoided.

Antwoord

C · G6PD deficiency with acute hemolysis
7.Klinische verpleegkunde

Antwoord: A · Reorientation, glasses and hearing aids in use, and good sleep and hydration

Waarom dit het antwoord is

Multicomponent prevention, including orientation, sensory aids, hydration, nutrition, mobility, sleep and avoiding unnecessary moves or drugs, reduces delirium.

Antwoord

A · Reorientation, glasses and hearing aids in use, and good sleep and hydration
8.Klinische verpleegkunde

Antwoord: C · APTT for heparin and INR for warfarin

Waarom dit het antwoord is

The APTT reflects the intrinsic pathway and is used to adjust heparin infusions. Warfarin is monitored by the INR, a standardised prothrombin time. Platelets are also checked on heparin to detect HIT.

Antwoord

C · APTT for heparin and INR for warfarin
9.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: C · Statins reduce further events whatever the starting cholesterol

Waarom dit het antwoord is

After ACS, high-intensity statin therapy reduces recurrent MI, stroke and death regardless of baseline cholesterol, partly by stabilising plaques. It is part of standard secondary prevention with antiplatelets, beta-blockers, ACE inhibitors and lifestyle change.

Antwoord

C · Statins reduce further events whatever the starting cholesterol
10.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: D · Cardiac tamponade from a clotted drain

Waarom dit het antwoord is

A sudden stop in drainage with falling blood pressure, tachycardia and rising venous pressure suggests blood is collecting around the heart because the drain is blocked. Tamponade after cardiac surgery is an emergency: the surgeon is called at once and the chest may need to be reopened.

Antwoord

D · Cardiac tamponade from a clotted drain
11.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: B · Limit close contact with children and pregnant women for a time

Waarom dit het antwoord is

After radioiodine, patients are advised to limit close and prolonged contact, especially with young children and pregnant women, for a period set by the department. It is contraindicated in pregnancy and breastfeeding.

Antwoord

B · Limit close contact with children and pregnant women for a time
12.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: A · "If she is unconscious, I will pour juice into her mouth."

Waarom dit het antwoord is

An unconscious person cannot swallow safely, so fluids in the mouth can be aspirated. The family should put the person in the recovery position, give glucagon if trained and call for help. Once awake and able to swallow, the person eats a longer-acting carbohydrate.

Antwoord

A · "If she is unconscious, I will pour juice into her mouth."
13.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: C · Blood glucose and signs of line infection

Waarom dit het antwoord is

PN can cause hyperglycaemia and electrolyte disturbance, and the central line is a route for bloodstream infection. Glucose, electrolytes, fluid balance, weight and temperature are monitored closely.

Antwoord

C · Blood glucose and signs of line infection
14.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: D · Withhold the next dose and report to the prescriber

Waarom dit het antwoord is

NSAIDs can cause acute kidney injury, particularly with hypovolaemia, older age or other nephrotoxic drugs. The nurse withholds and reports, and fluid status and other analgesia are reviewed.

Antwoord

D · Withhold the next dose and report to the prescriber
