1Formules en methoden

Prioriteren en delegeren

Methode

3Oplossingen

1.Prioriteren en delegeren

Antwoord: B · “I will press the button for her while she is asleep so she stays comfortable.”

Waarom dit het antwoord is

PCA is safe because a sleepy patient cannot press the button. If someone else presses it, the patient can receive too much opioid and develop respiratory depression.

Antwoord

B · “I will press the button for her while she is asleep so she stays comfortable.”
2.Prioriteren en delegeren

Antwoord: D · Hemoglobin 6,5 g/dL (65 g/L) in a patient with a GI bleed

Waarom dit het antwoord is

A hemoglobin of 6,5 g/dL (65 g/L) in a bleeding patient shows severe anemia and may need urgent transfusion. The other results are mildly abnormal or stable.

Antwoord

D · Hemoglobin 6,5 g/dL (65 g/L) in a patient with a GI bleed
3.Infectiepreventie en patiëntveiligheid

Antwoord: A · Below the level of the bladder and off the floor

Waarom dit het antwoord is

Keeping the bag below the bladder prevents backflow of urine into the bladder, and keeping it off the floor prevents contamination. Both reduce the risk of catheter-associated urinary tract infection.

Antwoord

A · Below the level of the bladder and off the floor

Antwoord: A · A low serum sodium

Waarom dit het antwoord is

Carbamazepine (and oxcarbazepine) can cause hyponatremia by increasing the effect of ADH, which can cause confusion, headache and seizures. The drug can also cause serious blood disorders, so the CBC is monitored.Gecontroleerd met: Syndrome of Inappropriate Antidiuretic Hormone Secretion · StatPearls (NCBI Bookshelf) (2023); Carbamazepine · StatPearls (NCBI Bookshelf) (2023)

Antwoord

A · A low serum sodium
6.Laboratoriumwaarden en elektrolyten

Antwoord: B · Severe C. difficile infection

Waarom dit het antwoord is

A white count of 15 × 10³/µL or more or a creatinine above 1,5 mg/dL marks severe C. difficile infection. These patients need prompt treatment, fluid replacement and close monitoring for complications such as toxic megacolon. Contact precautions with soap-and-water hand hygiene are used.

Antwoord

B · Severe C. difficile infection
7.Klinische verpleegkunde

8.Klinische verpleegkunde

Antwoord: B · Benzodiazepine sleeping tablets

Waarom dit het antwoord is

Sedatives, hypnotics, antipsychotics and drugs that lower blood pressure increase falls risk. A medication review is part of every falls assessment.

Antwoord

B · Benzodiazepine sleeping tablets
9.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: D · Salbutamol first, then the steroid after a short wait

Waarom dit het antwoord is

Using the bronchodilator first opens the airways so the inhaled corticosteroid can reach the smaller airways. A short wait between inhalers and a mouth rinse after the steroid are also taught.

Antwoord

D · Salbutamol first, then the steroid after a short wait
10.Medisch-chirurgisch: cardiovasculair en respiratoir

Antwoord: A · Acute stent thrombosis

Waarom dit het antwoord is

Acute stent thrombosis presents with recurrent ischaemic pain and ST elevation, and needs emergency return to the catheter laboratory. Missed antiplatelet doses are a major risk factor, which is why dual antiplatelet adherence is emphasised.

Antwoord

A · Acute stent thrombosis
11.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: C · Medication overuse headache

Waarom dit het antwoord is

Frequent use of painkillers or triptans can cause medication overuse headache. Treatment involves withdrawing the overused drug under guidance and starting a preventive treatment if needed.

Antwoord

C · Medication overuse headache
12.Medisch-chirurgisch: neurologie, endocrinologie en nieren

Antwoord: D · Give IV antibiotics at once and start sepsis treatment

Waarom dit het antwoord is

Meningococcal septicaemia can kill within hours. Antibiotics must not be delayed for a lumbar puncture or scan. Fluids and other sepsis care are started, and a lumbar puncture is often unsafe in a shocked patient.

Antwoord

D · Give IV antibiotics at once and start sepsis treatment

Antwoord

D · Hypokaliëmie
14.Medisch-chirurgisch: maag-darmstelsel, perioperatief en oncologie

Antwoord: C · It raises the risk of ketoacidosis, even with near-normal glucose

Waarom dit het antwoord is

SGLT2 inhibitors can cause euglycaemic diabetic ketoacidosis around fasting and surgery. They are usually paused before surgery as advised, and ketones are checked if the patient becomes unwell.

Antwoord

C · It raises the risk of ketoacidosis, even with near-normal glucose
