1Formüller ve yöntemler

Önceliklendirme ve yetki devri

Yöntem

  1. Hiçbir şey yapılmazsa hastaya en çabuk neyin zarar vereceğini sorun.
  2. ABC’yi uygulayın (kalp durmasında C-A-B), sonra Maslow: fizyolojik gereksinimler güvenlikten, güvenlik psikolojik gereksinimlerden önce gelir.
  3. Seçenekleri karşılaştırın: akut kronikten, stabil olmayan stabilden, beklenmeyen beklenenden, sistemik lokalden önce gelir ve her bulguyu hastanın başlangıç durumuyla karşılaştırın.

3Çözeltiler

1.Önceliklendirme ve yetki devri

Cevap: B · Keeping the drainage bag below the level of the bladder

Cevap neden bu

Keeping the bag below the bladder prevents backflow, and the closed system is not broken. Catheters are removed as soon as they are no longer needed.

Cevap

B · Keeping the drainage bag below the level of the bladder
2.Önceliklendirme ve yetki devri

Cevap: D · Acute chest syndrome, which needs urgent escalation and oxygen

Cevap neden bu

Chest pain, fever and low oxygen in sickle cell disease suggest acute chest syndrome. It can worsen quickly, so oxygen, urgent review and prescribed treatment are needed.

Cevap

D · Acute chest syndrome, which needs urgent escalation and oxygen
3.Enfeksiyon kontrolü ve hasta güvenliği

Cevap: D · Hold it away from the uniform and place it straight into a linen bag

Cevap neden bu

Soiled linen is rolled up, held away from the uniform and placed directly into a laundry bag at the bedside. Shaking linen spreads organisms into the air, and the floor is a contaminated surface.

Cevap

D · Hold it away from the uniform and place it straight into a linen bag
4.Enfeksiyon kontrolü ve hasta güvenliği

Cevap: C · “We will put the rear-facing seat in the front so we can see the baby.”

Cevap neden bu

A rear-facing car seat must never be placed in front of an active airbag, which can strike the seat with great force. Infants ride rear-facing in the back seat, with a snug harness, on every trip.

Cevap

C · “We will put the rear-facing seat in the front so we can see the baby.”
5.Laboratuvar değerleri ve elektrolitler

Cevap: C · A fall in serum potassium

Cevap neden bu

Beta-2 agonists push potassium into cells and can cause hypokalemia, especially at high doses; they also raise blood glucose. For this reason nebulized albuterol is sometimes used as an extra treatment for hyperkalemia.

Cevap

C · A fall in serum potassium
6.Laboratuvar değerleri ve elektrolitler

Cevap: D · Primary (psychogenic) polydipsia

Cevap neden bu

In primary polydipsia, ADH is appropriately suppressed, so the kidneys make very dilute urine, but the intake exceeds what they can excrete. In SIADH the urine is inappropriately concentrated. Fluid restriction and close sodium monitoring are needed.Kontrol edilen kaynak: Osmolality urine test · MedlinePlus (NIH) (2025); Syndrome of inappropriate antidiuretic hormone secretion · MedlinePlus (NIH) (2025)

Cevap

D · Primary (psychogenic) polydipsia
7.Klinik hemşirelik

Cevap: A · Dehydration and electrolyte loss from liquid output

Cevap neden bu

Ileostomy output is liquid because it bypasses the colon, which normally absorbs water and salts. Patients must drink enough and watch for signs of dehydration, especially in hot weather or illness.

Cevap

A · Dehydration and electrolyte loss from liquid output
8.Klinik hemşirelik

Cevap: B · Cardiac tamponade

Cevap neden bu

Hypotension, raised JVP and muffled heart sounds (Beck's triad) suggest cardiac tamponade. Blood in the pericardial space compresses the heart and needs urgent drainage.

Cevap

B · Cardiac tamponade
9.Dahili-cerrahi: kardiyovasküler ve solunum

Cevap: D · Reassess heart rate, blood pressure, urine output and lung sounds

Cevap neden bu

A fluid challenge is followed by reassessment of perfusion (heart rate, BP, capillary refill, mental state, urine output) and signs of overload such as crackles, raised JVP or worsening breathlessness. The findings are reported to decide on further fluid, vasopressors or escalation.

Cevap

D · Reassess heart rate, blood pressure, urine output and lung sounds
10.Dahili-cerrahi: kardiyovasküler ve solunum

Cevap: A · A patient on day 1 after surgery who is newly restless, RR 30/min, SpO₂ 88% on 4 L/min

Cevap neden bu

New restlessness, tachypnoea and hypoxaemia despite oxygen suggest acute deterioration such as a PE, pneumonia or bleeding, so this patient is seen first. SpO₂ 89% is within target for a stable COPD patient, a mild fever on antibiotics is expected, and the asthma patient is well.

Cevap

A · A patient on day 1 after surgery who is newly restless, RR 30/min, SpO₂ 88% on 4 L/min
11.Dahili-cerrahi: nöroloji, endokrin ve böbrek

Cevap: C · It slows the decline in kidney function

Cevap neden bu

Hypertension is both a cause and a result of CKD. Controlling it, often with an ACE inhibitor or ARB when albuminuria is present, slows the loss of kidney function and reduces cardiovascular risk.

Cevap

C · It slows the decline in kidney function
12.Dahili-cerrahi: nöroloji, endokrin ve böbrek

Cevap: B · Autonomic dysfunction, which needs continuous cardiac monitoring

Cevap neden bu

Guillain–Barré syndrome can involve autonomic nerves, causing labile blood pressure and dangerous arrhythmias. Patients need continuous ECG monitoring and close observation, often in a high-dependency setting.

Cevap

B · Autonomic dysfunction, which needs continuous cardiac monitoring
13.Dahili-cerrahi: sindirim sistemi, perioperatif ve onkoloji

Cevap: D · They can reduce the absorption of many other drugs

Cevap neden bu

Antacids can bind or change the absorption of drugs such as some antibiotics (tetracyclines, quinolones), iron and levothyroxine. Doses are usually separated by a couple of hours.

Cevap

D · They can reduce the absorption of many other drugs
14.Dahili-cerrahi: sindirim sistemi, perioperatif ve onkoloji

Cevap: A · Olası omurilik basısı: üst basamağa bildirmek, reçete edildiği şekilde steroidler, acil MRG

Cevap

A · Olası omurilik basısı: üst basamağa bildirmek, reçete edildiği şekilde steroidler, acil MRG
