1Formüller ve yöntemler

Önceliklendirme ve yetki devri

Yöntem

  1. Hiçbir şey yapılmazsa hastaya en çabuk neyin zarar vereceğini sorun.
  2. ABC’yi uygulayın (kalp durmasında C-A-B), sonra Maslow: fizyolojik gereksinimler güvenlikten, güvenlik psikolojik gereksinimlerden önce gelir.
  3. Seçenekleri karşılaştırın: akut kronikten, stabil olmayan stabilden, beklenmeyen beklenenden, sistemik lokalden önce gelir ve her bulguyu hastanın başlangıç durumuyla karşılaştırın.

3Çözeltiler

1.Önceliklendirme ve yetki devri

Cevap: D · Help the patient empty her bladder

Cevap neden bu

A uterus that is high and displaced to one side usually means a full bladder. A full bladder stops the uterus contracting well, so the nurse helps the patient void and reassesses.

Cevap

D · Help the patient empty her bladder
2.Önceliklendirme ve yetki devri

Cevap: A · Glasgow Koma Skalası skoru 15’ten 12’ye düşüyor

Cevap

A · Glasgow Koma Skalası skoru 15’ten 12’ye düşüyor
3.Enfeksiyon kontrolü ve hasta güvenliği

Cevap: D · A large toy with no small parts that can come off

Cevap neden bu

Small parts, marbles, magnets and uninflated or broken balloons are choking hazards for young children. Toys should be large, sturdy and suited to the child’s age.

Cevap

D · A large toy with no small parts that can come off
4.Enfeksiyon kontrolü ve hasta güvenliği

Cevap: B · Release the restraint at once and reposition the patient

Cevap neden bu

Patients have died from strangulation when vest restraints moved up to the neck. The nurse frees the patient immediately, checks breathing and repositions them, then reassesses whether the restraint is needed at all.

Cevap

B · Release the restraint at once and reposition the patient
5.Laboratuvar değerleri ve elektrolitler

Cevap: C · Absolute neutrophil count

Cevap neden bu

Clozapine can cause severe neutropenia (agranulocytosis), so the ANC is checked before starting and regularly during treatment. A sore throat, fever or other sign of infection must be reported at once.

Cevap

C · Absolute neutrophil count
6.Laboratuvar değerleri ve elektrolitler

Cevap: C · Hemophilia A (factor VIII deficiency)

Cevap neden bu

Hemophilia A is an X-linked lack of factor VIII, which belongs to the intrinsic pathway, so only the aPTT is prolonged. Bleeding into joints and muscles is typical. Vitamin K deficiency and DIC prolong the PT as well, and ITP lowers the platelets.Kontrol edilen kaynak: Hemophilia A · MedlinePlus (NIH) (2026); Partial Thromboplastin Time (PTT) Test · MedlinePlus (NIH)

Cevap

C · Hemophilia A (factor VIII deficiency)
7.Klinik hemşirelik

Cevap: B · Respiratory alkalosis from a low PaCO2

Cevap neden bu

Hyperventilation blows off CO2, raising the pH. Symptoms can include tingling of the lips and fingers and lightheadedness.

Cevap

B · Respiratory alkalosis from a low PaCO2
8.Klinik hemşirelik

Cevap: B · Intramuscular injection into the anterolateral thigh

Cevap neden bu

IM adrenaline into the outer thigh is absorbed quickly and is the first-line route. IV adrenaline is reserved for experienced specialists with full monitoring because of the risk of arrhythmias.

Cevap

B · Intramuscular injection into the anterolateral thigh
9.Dahili-cerrahi: kardiyovasküler ve solunum

Cevap: C · BP lying after rest, then again after 1 and 3 minutes standing

Cevap neden bu

Blood pressure is measured after the patient has been lying for about 5 minutes, then after 1 and 3 minutes of standing. A fall in systolic pressure of 20 mmHg or more, or diastolic of 10 mmHg or more, indicates orthostatic hypotension. Symptoms such as dizziness are recorded too.

Cevap

C · BP lying after rest, then again after 1 and 3 minutes standing
10.Dahili-cerrahi: kardiyovasküler ve solunum

Cevap: A · Stopping even today lowers carbon monoxide levels before surgery

Cevap neden bu

Carboxyhaemoglobin falls substantially within about 12–24 hours of stopping, improving oxygen delivery. Longer abstinence improves airway function, wound healing and reduces complications further. Any stop is beneficial, and support should be offered.

Cevap

A · Stopping even today lowers carbon monoxide levels before surgery
11.Dahili-cerrahi: nöroloji, endokrin ve böbrek

Cevap: B · Keep the needle in the skin for about 10 seconds

Cevap neden bu

Holding the needle in place for around 10 seconds (or as the manufacturer advises) lets the full dose be delivered. A new needle is used for each injection, and the site should not be massaged.

Cevap

B · Keep the needle in the skin for about 10 seconds
12.Dahili-cerrahi: nöroloji, endokrin ve böbrek

Cevap: C · A patient with Guillain–Barré syndrome whose vital capacity is falling

Cevap neden bu

A falling vital capacity and weak cough in Guillain–Barré syndrome warn of respiratory failure, which may need ventilation. The other patients have expected or less urgent problems.

Cevap

C · A patient with Guillain–Barré syndrome whose vital capacity is falling
13.Dahili-cerrahi: sindirim sistemi, perioperatif ve onkoloji

Cevap: C · Hourly urine output guides fluid resuscitation

Cevap neden bu

Fluid formulas give an initial estimate; the rate is then adjusted to urine output, typically around 0,5 mL/kg/h in adults, along with other signs of perfusion.

Cevap

C · Hourly urine output guides fluid resuscitation
14.Dahili-cerrahi: sindirim sistemi, perioperatif ve onkoloji

Cevap: C · Osteomyelitis

Cevap neden bu

A positive probe-to-bone test in a diabetic foot ulcer strongly suggests osteomyelitis. Imaging, bone samples, prolonged antibiotics and specialist foot team care are usually needed.

Cevap

C · Osteomyelitis
