A patient with difficulty swallowing is prescribed an extended-release tablet. What should the nurse do?
ADissolve the tablet in a glass of hot water
BCut the tablet in half so it is easier to swallow
CAsk the prescriber or pharmacist for another form and not crush the tablet
DCrush the tablet finely and mix it into a spoonful of applesauce or yogurt
A patient asks for a pain tablet. The record shows none has been given, but the patient says, “The other nurse gave me one earlier.” What should the nurse do?
ACheck with the other nurse and the records before giving another dose
BGive the tablet, because the record shows none was given
CGive half a tablet now, to be safe, and the other half later if needed
DTell the patient they must be confused
When giving several medicines through a feeding tube, what is correct?
ACrush every tablet, including modified-release ones
BGive them without flushing to save time
CGive each medicine separately and flush with water between them
DMix them all together in the feed so they are given at the same time
A severely malnourished patient is about to start feeding. Which complication is a particular risk?
AVery high blood sodium in the first hour
BSudden weight gain of several kilograms of fat in the first week
CRefeeding syndrome with low phosphate, potassium and magnesium
DImmediate allergic reaction to the feed
AStaphylococcus
BEnterococcus короткими ланцюжками
CStreptococcus
DNeisseria
C · Ask the prescriber or pharmacist for another form and not crush the tablet
Crushing or splitting an extended-release product can release the whole dose at once and cause toxicity. The nurse asks for a suitable alternative, such as a liquid or immediate-release form.
C · Ask the prescriber or pharmacist for another form and not crush the tablet
A · Check with the other nurse and the records before giving another dose
Undocumented doses can lead to double dosing. The nurse checks with colleagues and the dispensing records, then gives the drug only once it is clear whether a dose was given.
A · Check with the other nurse and the records before giving another dose
C · Give each medicine separately and flush with water between them
Each drug is prepared and given separately, with water flushes before, between and after, to prevent tube blockage and drug interactions. A pharmacist advises on suitable forms.
C · Give each medicine separately and flush with water between them
C · Refeeding syndrome with low phosphate, potassium and magnesium
When feeding restarts after starvation, insulin release drives phosphate, potassium and magnesium into the cells. Low levels can cause heart failure and arrhythmias, so feeding starts slowly and bloods are monitored.
C · Refeeding syndrome with low phosphate, potassium and magnesium
A · Staphylococcus
