1Công thức và phương pháp

Ưu tiên và ủy thác công việc

Phương pháp

  2. Áp dụng ABC (C-A-B khi ngừng tim), rồi Maslow: nhu cầu sinh lý trước an toàn, an toàn trước nhu cầu tâm lý.

  1. Bắt đầu với các biện pháp phòng ngừa chuẩn cho mọi bệnh nhân: vệ sinh tay, phương tiện bảo hộ cá nhân (PPE) cho phơi nhiễm dự kiến, xử lý vật sắc nhọn an toàn.

B · Ensure the patient’s safety, document the findings and report as required
3.Kiểm soát nhiễm khuẩn và an toàn người bệnh

Đáp số: C · Use the oral syringe or dosing cup that comes with the medicine

Vì sao đây là đáp án

Household spoons vary widely in size and cause dosing errors. A marked oral syringe or dosing device in mL gives an accurate dose.

Đáp số

C · Use the oral syringe or dosing cup that comes with the medicine
4.Kiểm soát nhiễm khuẩn và an toàn người bệnh

Đáp số: C · Finding the system and process weaknesses that allowed the error

Vì sao đây là đáp án

A root cause analysis looks beyond the individual to the system factors, such as storage, labeling, staffing or workflow, that allowed the error. The aim is to change the system so the error cannot happen again.

Đáp số

C · Finding the system and process weaknesses that allowed the error
5.Giá trị xét nghiệm và điện giải

Đáp số: A · A low serum sodium

Vì sao đây là đáp án

Carbamazepine (and oxcarbazepine) can cause hyponatremia by increasing the effect of ADH, which can cause confusion, headache and seizures. The drug can also cause serious blood disorders, so the CBC is monitored.Đã đối chiếu với: Syndrome of Inappropriate Antidiuretic Hormone Secretion · StatPearls (NCBI Bookshelf) (2023); Carbamazepine · StatPearls (NCBI Bookshelf) (2023)

Đáp số

A · A low serum sodium
6.Giá trị xét nghiệm và điện giải

Đáp số: A · Folate deficiency, made worse by a direct alcohol effect

Vì sao đây là đáp án

Poor diet and alcohol reduce folate intake and absorption, causing macrocytic anemia, and alcohol itself also raises the MCV. Iron deficiency and thalassemia cause small cells. Folate is replaced after B12 deficiency is excluded, and thiamine is also commonly given.Đã đối chiếu với: Folate-deficiency anemia - Medical Encyclopedia · MedlinePlus (NIH) (2026)

Đáp số

A · Folate deficiency, made worse by a direct alcohol effect
7.Điều dưỡng lâm sàng

Đáp số: A · Non-rebreathe mask with reservoir bag at 15 L/min

A · Thyroid storm
9.Nội–ngoại khoa: tim mạch và hô hấp

Đáp số: D · To avoid delivering the shock during the T wave

Vì sao đây là đáp án

A shock delivered on the T wave (the vulnerable period) can trigger ventricular fibrillation. Synchronisation times the shock to the R wave. In ventricular fibrillation there is no organised QRS, so defibrillation is unsynchronised.

Đáp số

D · To avoid delivering the shock during the T wave
10.Nội–ngoại khoa: tim mạch và hô hấp

Đáp số: A · Treat the fever, pain and possible dehydration, then reassess

Vì sao đây là đáp án

Sinus tachycardia is usually a physiological response to fever, pain, hypovolaemia, anxiety, hypoxia or sepsis. Treating the cause (antipyretics, analgesia, fluids, oxygen if needed) is appropriate, while watching for sepsis. Rate-slowing drugs can be harmful by masking compensation.

Đáp số

A · Treat the fever, pain and possible dehydration, then reassess
11.Nội–ngoại khoa: thần kinh, nội tiết và thận

Đáp số: B · To match the natural cortisol rhythm and protect the stomach

Vì sao đây là đáp án

Morning dosing mimics the normal early-morning cortisol peak, causes less adrenal suppression and less sleep disturbance. Taking it with food reduces gastric irritation.

Đáp số

B · To match the natural cortisol rhythm and protect the stomach
12.Nội–ngoại khoa: thần kinh, nội tiết và thận

Đáp số: C · Central cord syndrome

Vì sao đây là đáp án

Central cord syndrome often follows hyperextension of an arthritic neck. The central fibres, which serve the arms, are affected most, so the arms are weaker than the legs. It is the most common incomplete cord syndrome.

Đáp số

C · Central cord syndrome
