1Công thức và phương pháp

Ưu tiên và ủy thác công việc

Phương pháp

  2. Áp dụng ABC (C-A-B khi ngừng tim), rồi Maslow: nhu cầu sinh lý trước an toàn, an toàn trước nhu cầu tâm lý.

  1. Bắt đầu với các biện pháp phòng ngừa chuẩn cho mọi bệnh nhân: vệ sinh tay, phương tiện bảo hộ cá nhân (PPE) cho phơi nhiễm dự kiến, xử lý vật sắc nhọn an toàn.

Đáp số: C · Ensure the patient’s safety, document the findings and report as required

Vì sao đây là đáp án

Unexplained bruises of different ages and fear of a carer can suggest abuse. The nurse makes sure the patient is safe, documents objectively and reports according to law and policy.

Đáp số

C · Ensure the patient’s safety, document the findings and report as required
3.Kiểm soát nhiễm khuẩn và an toàn người bệnh

Đáp số: A · To remove items that could be used to harm self or others

Vì sao đây là đáp án

Belongings are searched with the patient, according to policy, for items such as sharp objects, cords, drugs and lighters. These are stored safely to reduce the risk of self-harm or violence.

Đáp số

A · To remove items that could be used to harm self or others
4.Kiểm soát nhiễm khuẩn và an toàn người bệnh

Đáp số: D · Lesions all at the same stage, most dense on the face and limbs

Vì sao đây là đáp án

In smallpox, lesions develop at the same stage in any one area and are densest on the face and extremities. In chickenpox, crops appear at different stages and are densest on the trunk.

Đáp số

D · Lesions all at the same stage, most dense on the face and limbs
5.Giá trị xét nghiệm và điện giải

Đáp số: B · About 140–180 mg/dL (7,8–10 mmol/L)

Vì sao đây là đáp án

For most critically ill patients, insulin is used to keep glucose around 140–180 mg/dL (7,8–10 mmol/L); UK guidance uses a similar range of about 6–10 mmol/L. Lower targets increase the risk of hypoglycemia, and persistently higher levels increase infection and other complications.

Đáp số

B · About 140–180 mg/dL (7,8–10 mmol/L)
6.Giá trị xét nghiệm và điện giải

Đáp số: C · "I must not eat anything for 12 hours before the HbA1c test"

Vì sao đây là đáp án

HbA1c reflects glucose attached to hemoglobin over about 2–3 months, so fasting is not needed. Lower HbA1c values are linked to fewer microvascular complications, and home glucose monitoring is still useful for day-to-day decisions.

Đáp số

C · "I must not eat anything for 12 hours before the HbA1c test"
7.Điều dưỡng lâm sàng

Đáp số: D · Avoid heat sources such as hot water bottles over the patch

Vì sao đây là đáp án

Heat increases absorption of fentanyl through the skin and can cause overdose. Old patches are removed and folded before disposal, and patches should not be cut unless the product allows it.

Đáp số

D · Avoid heat sources such as hot water bottles over the patch
8.Điều dưỡng lâm sàng

Đáp số: B · The forehead can still be wrinkled on the affected side

Vì sao đây là đáp án

In a stroke (upper motor neurone lesion), the forehead is usually spared because it has bilateral cortical supply. Bell's palsy (lower motor neurone) weakens the whole side, including the forehead.

Đáp số

B · The forehead can still be wrinkled on the affected side
9.Nội–ngoại khoa: tim mạch và hô hấp

Đáp số: A · Avoid BP cuffs and injections on the left arm where possible

Vì sao đây là đáp án

After axillary node removal, the arm's lymph drainage is reduced. Patients are advised to protect the skin, treat cuts promptly, avoid tight items and, where possible, avoid BP measurements, cannulation and injections on that arm. Gentle exercise helps lymph flow.

Đáp số

A · Avoid BP cuffs and injections on the left arm where possible
10.Nội–ngoại khoa: tim mạch và hô hấp

Đáp số: C · Low oxygen saturation is not explained by anxiety; PE must be excluded

Vì sao đây là đáp án

Panic-related hyperventilation causes respiratory alkalosis but normal oxygen saturation. Hypoxaemia with tachycardia in a woman taking oestrogen suggests PE. She needs oxygen, urgent medical assessment and investigation. Paper-bag rebreathing is unsafe and can worsen hypoxaemia.

Đáp số

C · Low oxygen saturation is not explained by anxiety; PE must be excluded
11.Nội–ngoại khoa: thần kinh, nội tiết và thận

Đáp số: A · To stop blood clots from blocking the catheter

Vì sao đây là đáp án

After TURP, the raw prostate bed bleeds, and clots can block the catheter and cause retention and bladder spasms. Continuous irrigation through a three-way catheter keeps the drainage flowing.

Đáp số

A · To stop blood clots from blocking the catheter
12.Nội–ngoại khoa: thần kinh, nội tiết và thận

Đáp số: C · "I will use a salt substitute instead of ordinary salt."

Vì sao đây là đáp án

Salt substitutes often contain potassium chloride, which can cause dangerous hyperkalaemia in CKD. Patients should reduce salt without using these products. Boiling vegetables and limiting high-potassium foods are correct.

Đáp số

C · "I will use a salt substitute instead of ordinary salt."
13.Nội–ngoại khoa: tiêu hóa, quanh phẫu thuật và ung thư

Đáp số: C · Emptying the pouch and measuring the output

Vì sao đây là đáp án

Đáp số

C · Emptying the pouch and measuring the output
14.Nội–ngoại khoa: tiêu hóa, quanh phẫu thuật và ung thư

Đáp số: D · Liver damage may still develop, and appears only days later

Vì sao đây là đáp án

Early after paracetamol overdose, patients often have no or few symptoms, but liver injury peaks after about 3–4 days. Treatment with acetylcysteine is guided by timed paracetamol levels and the history.

Đáp số

D · Liver damage may still develop, and appears only days later
