1Công thức và phương pháp

Ưu tiên và ủy thác công việc

Phương pháp

  2. Áp dụng ABC (C-A-B khi ngừng tim), rồi Maslow: nhu cầu sinh lý trước an toàn, an toàn trước nhu cầu tâm lý.

  1. Bắt đầu với các biện pháp phòng ngừa chuẩn cho mọi bệnh nhân: vệ sinh tay, phương tiện bảo hộ cá nhân (PPE) cho phơi nhiễm dự kiến, xử lý vật sắc nhọn an toàn.

The pH is low (acidosis), the PaCO₂ is normal and the bicarbonate is low, so the cause is metabolic. Causes include diabetic ketoacidosis, sepsis and kidney failure.

Đáp số

C · Metabolic acidosis
2.Ưu tiên và ủy thác công việc

Đáp số: C · “Check his glucose and give the insulin according to the sliding scale.”

Vì sao đây là đáp án

Giving insulin is medication administration and needs a licensed nurse. Measuring glucose, hygiene and supervised walking are suitable tasks with clear reporting instructions.

Đáp số

C · “Check his glucose and give the insulin according to the sliding scale.”
3.Kiểm soát nhiễm khuẩn và an toàn người bệnh

Đáp số: D · Chlorhexidine-alcohol skin antisepsis and maximal sterile barrier precautions

Vì sao đây là đáp án

The insertion bundle includes hand hygiene, chlorhexidine-alcohol skin prep, maximal barrier precautions (cap, mask, sterile gown, sterile gloves and a full body drape) and avoiding the femoral site in adults. Routine replacement and routine antibiotics are not recommended.

Đáp số

D · Chlorhexidine-alcohol skin antisepsis and maximal sterile barrier precautions
4.Kiểm soát nhiễm khuẩn và an toàn người bệnh

Đáp số: B · A patient admitted overnight for observation who is now stable with normal tests

Vì sao đây là đáp án

In a surge, stable patients who could be safely cared for at home are discharged first. Patients who are unstable or need close monitoring or titrated infusions must stay.

Đáp số

B · A patient admitted overnight for observation who is now stable with normal tests
5.Giá trị xét nghiệm và điện giải

Đáp số: A · Absolute neutrophil count

Vì sao đây là đáp án

Clozapine can cause severe neutropenia (agranulocytosis), so the ANC is checked before starting and regularly during treatment. A sore throat, fever or other sign of infection must be reported at once.

Đáp số

A · Absolute neutrophil count
6.Giá trị xét nghiệm và điện giải

Đáp số: C · G6PD deficiency with acute hemolysis

Vì sao đây là đáp án

G6PD deficiency makes red cells vulnerable to oxidative damage. Fava beans, some drugs (such as primaquine and some sulfonamides) and infections can trigger acute hemolysis, with dark urine, jaundice and anemia. Triggers are avoided.

Đáp số

C · G6PD deficiency with acute hemolysis
7.Điều dưỡng lâm sàng

Đáp số: B · Constipation

Vì sao đây là đáp án

Opioids slow gut movement, and constipation persists with long-term use. A regular laxative is usually prescribed together with the opioid.

Đáp số

B · Constipation
8.Điều dưỡng lâm sàng

Đáp số: D · Report it as a near miss through the incident reporting system

Vì sao đây là đáp án

Near misses are reported so that causes can be studied and systems improved. A just, open culture encourages reporting without blame.

Đáp số

D · Report it as a near miss through the incident reporting system
9.Nội–ngoại khoa: tim mạch và hô hấp

Đáp số: D · One shock and well: contact the device clinic; repeated shocks or unwell: call emergency services

Vì sao đây là đáp án

A single shock with full recovery usually needs a call to the device team. Several shocks in a short time, or a shock with chest pain, breathlessness or collapse, needs emergency help. Patients should not drive themselves after a shock.

Đáp số

D · One shock and well: contact the device clinic; repeated shocks or unwell: call emergency services
10.Nội–ngoại khoa: tim mạch và hô hấp

Đáp số: A · Document and report it, as this is often normal in an athlete

Vì sao đây là đáp án

Endurance training increases vagal tone and stroke volume, so resting rates in the 40s can be normal. Without symptoms or haemodynamic compromise, no treatment is needed, though it is recorded and the team informed. Symptomatic bradycardia is treated.

Đáp số

A · Document and report it, as this is often normal in an athlete
11.Nội–ngoại khoa: thần kinh, nội tiết và thận

Đáp số: B · After about 6–8 weeks

Vì sao đây là đáp án

Levothyroxine has a half-life of about a week, so it takes several weeks to reach a new steady state. TSH is usually rechecked about 6–8 weeks after a dose change.

Đáp số

B · After about 6–8 weeks
12.Nội–ngoại khoa: thần kinh, nội tiết và thận

Đáp số: D · Secondary hypothyroidism

Vì sao đây là đáp án

When the pituitary cannot make enough TSH, the thyroid is not stimulated, so both TSH and free T4 are low. In primary thyroid failure, TSH is high. In secondary hypothyroidism, free T4, not TSH, is used to guide the dose.

Đáp số

D · Secondary hypothyroidism
13.Nội–ngoại khoa: tiêu hóa, quanh phẫu thuật và ung thư

Đáp số: B · Explaining what will happen and answering questions

Vì sao đây là đáp án

Clear information about what to expect, including pain control and recovery, reduces anxiety and can improve recovery. False reassurance does not address the patient's concerns.

Đáp số

B · Explaining what will happen and answering questions
