1Công thức và phương pháp

Ưu tiên và ủy thác công việc

Phương pháp

  2. Áp dụng ABC (C-A-B khi ngừng tim), rồi Maslow: nhu cầu sinh lý trước an toàn, an toàn trước nhu cầu tâm lý.

  1. Bắt đầu với các biện pháp phòng ngừa chuẩn cho mọi bệnh nhân: vệ sinh tay, phương tiện bảo hộ cá nhân (PPE) cho phơi nhiễm dự kiến, xử lý vật sắc nhọn an toàn.

Đáp số: B · “I will use my steroid inhaler when I have a sudden attack.”

Vì sao đây là đáp án

Inhaled corticosteroids reduce inflammation over time and do not relieve an acute attack. Salbutamol (albuterol) is the quick-relief inhaler; rinsing after steroids helps prevent oral thrush.

Đáp số

B · “I will use my steroid inhaler when I have a sudden attack.”
2.Ưu tiên và ủy thác công việc

Đáp số: A · That the child is passing urine

Vì sao đây là đáp án

Potassium is not added to IV fluids until the child is passing urine, because poor kidney function can let potassium build up to dangerous levels.

Đáp số

A · That the child is passing urine
3.Kiểm soát nhiễm khuẩn và an toàn người bệnh

Đáp số: C · Whether the patient has any metal implants or devices, such as a pacemaker

Vì sao đây là đáp án

The MRI magnet can move or heat metal objects and can stop some implanted devices from working. Patients are screened for implants, pacemakers, metal fragments and metal items such as jewelry before entering the scanner room.

Đáp số

C · Whether the patient has any metal implants or devices, such as a pacemaker
4.Kiểm soát nhiễm khuẩn và an toàn người bệnh

Đáp số: B · Hold the dose and verify it with the prescriber and pharmacist

Vì sao đây là đáp án

Tenfold errors are common in pediatrics, usually from decimal or unit mistakes. The nurse holds the dose and clarifies it, and never changes the dose on their own.

Đáp số

B · Hold the dose and verify it with the prescriber and pharmacist
5.Giá trị xét nghiệm và điện giải

Đáp số: C · Magnesium

Vì sao đây là đáp án

Long-term proton pump inhibitor use can cause hypomagnesemia by reducing its absorption in the gut. Low magnesium causes tremor, cramps, hyperreflexia and arrhythmias, and it can also cause low potassium and low calcium.Đã đối chiếu với: Magnesium Blood Test · MedlinePlus (NIH) (2023); Proton pump inhibitors and hypomagnesemia: A meta-analysis of observational studies · PubMed Central (NIH/NLM)

Đáp số

C · Magnesium

Đáp số: A · Tiredness, weight gain and feeling cold

Vì sao đây là đáp án

Low thyroid hormone slows metabolism, causing fatigue, weight gain, cold intolerance, constipation, dry skin and bradycardia. Weight loss, heat intolerance and tremor suggest hyperthyroidism.

Đáp số

A · Tiredness, weight gain and feeling cold
8.Điều dưỡng lâm sàng

Đáp số: C · Neurogenic shock

Vì sao đây là đáp án

Neurogenic shock from cervical or high thoracic injury causes vasodilation and bradycardia because sympathetic outflow is lost. Hypovolaemic shock usually causes tachycardia and cool, pale skin.

Đáp số

C · Neurogenic shock
9.Nội–ngoại khoa: tim mạch và hô hấp

Đáp số: D · It reduces absorption of cholesterol from the gut

Vì sao đây là đáp án

Ezetimibe blocks the intestinal cholesterol transporter (NPC1L1), so less dietary and biliary cholesterol is absorbed and LDL falls. It is often added to a statin. Statins act by inhibiting HMG-CoA reductase in the liver.

Đáp số

D · It reduces absorption of cholesterol from the gut
10.Nội–ngoại khoa: tim mạch và hô hấp

Đáp số: A · Call for expert help and give oxygen to both the face and the stoma

Vì sao đây là đáp án

In the first days after a tracheostomy, the tract is immature and the tube can easily be misplaced into the tissues. Emergency guidance is to call for an airway expert, apply oxygen to face and stoma and assess breathing. Reinsertion is done only by trained staff.

Đáp số

A · Call for expert help and give oxygen to both the face and the stoma
11.Nội–ngoại khoa: thần kinh, nội tiết và thận

Đáp số: A · Venous thrombosis

Vì sao đây là đáp án

In nephrotic syndrome, anticoagulant proteins such as antithrombin are lost, and clotting factors made by the liver rise, so venous thrombosis is more likely. Infection and high cholesterol are other complications.

Đáp số

A · Venous thrombosis
12.Nội–ngoại khoa: thần kinh, nội tiết và thận

Đáp số: A · Ramipril, spironolactone and trimethoprim

Vì sao đây là đáp án

ACE inhibitors and spironolactone reduce aldosterone effect, and trimethoprim blocks potassium secretion in the distal tubule. Together, in CKD, they can cause dangerous hyperkalaemia. Loop diuretics, salbutamol and insulin lower potassium.

Đáp số

A · Ramipril, spironolactone and trimethoprim
13.Nội–ngoại khoa: tiêu hóa, quanh phẫu thuật và ung thư

Đáp số: D · Treatment before the main treatment, often to shrink the tumour

Vì sao đây là đáp án

Neoadjuvant chemotherapy or radiotherapy is given before surgery to shrink the tumour, which may make surgery easier or allow less extensive surgery. Adjuvant therapy follows surgery.

Đáp số

D · Treatment before the main treatment, often to shrink the tumour
14.Nội–ngoại khoa: tiêu hóa, quanh phẫu thuật và ung thư

Đáp số: B · Inform the surgeon so that the procedure can be explained again

Vì sao đây là đáp án

Valid consent requires understanding. The nurse advocates for the patient and asks the surgeon (who obtains consent) to explain again before surgery goes ahead. The conversation is documented.

Đáp số

B · Inform the surgeon so that the procedure can be explained again
