1Công thức và phương pháp

Ưu tiên và ủy thác công việc

Phương pháp

  2. Áp dụng ABC (C-A-B khi ngừng tim), rồi Maslow: nhu cầu sinh lý trước an toàn, an toàn trước nhu cầu tâm lý.

  1. Bắt đầu với các biện pháp phòng ngừa chuẩn cho mọi bệnh nhân: vệ sinh tay, phương tiện bảo hộ cá nhân (PPE) cho phơi nhiễm dự kiến, xử lý vật sắc nhọn an toàn.

If contrast causes kidney injury, metformin can build up and cause lactic acidosis. Protocols often hold metformin around the procedure, especially when kidney function is reduced.

Đáp số

C · Metformin
2.Ưu tiên và ủy thác công việc

Đáp số: A · A vasopressor, such as norepinephrine, to raise the pressure

Vì sao đây là đáp án

If hypotension continues after fluid resuscitation, a vasopressor such as norepinephrine is started to keep the MAP at least 65 mmHg. Diuretics and beta-blockers would worsen perfusion.

Đáp số

A · A vasopressor, such as norepinephrine, to raise the pressure
3.Kiểm soát nhiễm khuẩn và an toàn người bệnh

Đáp số: C · The patient’s identity is checked against the blood product and its paperwork at the bedside

Vì sao đây là đáp án

Wrong-patient transfusion can cause a fatal ABO reaction, so the final check is always done at the bedside, with positive patient identification against the product label, compatibility tag and order. Many US hospitals require two qualified people for this check, while UK guidance allows one trained person if positive identification is used. Local policy decides who checks.

Đáp số

C · The patient’s identity is checked against the blood product and its paperwork at the bedside
4.Kiểm soát nhiễm khuẩn và an toàn người bệnh

Đáp số: C · Use bariatric-rated equipment and enough trained staff

Vì sao đây là đáp án

Using equipment above its rated capacity can cause collapse and injury. Bariatric lifts, chairs and beds are used, with enough trained staff and a plan agreed in advance.

Đáp số

C · Use bariatric-rated equipment and enough trained staff
5.Giá trị xét nghiệm và điện giải

Đáp số: D · About 140–180 mg/dL (7,8–10 mmol/L)

Vì sao đây là đáp án

For most critically ill patients, insulin is used to keep glucose around 140–180 mg/dL (7,8–10 mmol/L). Lower targets increase the risk of hypoglycemia, and persistently higher levels increase infection and other complications.

Đáp số

D · About 140–180 mg/dL (7,8–10 mmol/L)
6.Giá trị xét nghiệm và điện giải

Đáp số: D · IV hydration, a uric acid-lowering drug and frequent electrolytes

Vì sao đây là đáp án

Prevention includes aggressive IV hydration, drugs such as allopurinol or rasburicase to lower uric acid, and frequent monitoring of potassium, phosphate, calcium, uric acid and creatinine. Tumor lysis usually begins within 1–3 days of starting treatment.Đã đối chiếu với: Uric Acid Test · MedlinePlus (NIH) (2026)

Đáp số

D · IV hydration, a uric acid-lowering drug and frequent electrolytes
7.Điều dưỡng lâm sàng

Đáp số: D · Dehydration and electrolyte loss from liquid output

Vì sao đây là đáp án

Ileostomy output is liquid because it bypasses the colon, which normally absorbs water and salts. Patients must drink enough and watch for signs of dehydration, especially in hot weather or illness.

Đáp số

D · Dehydration and electrolyte loss from liquid output
8.Điều dưỡng lâm sàng

Đáp số: A · To prevent "wrong blood in tube" errors that can cause fatal reactions

Vì sao đây là đáp án

A sample labelled with the wrong patient's details can lead to incompatible blood being issued. Labelling at the bedside after positive identification is a key transfusion safety step.

Đáp số

A · To prevent "wrong blood in tube" errors that can cause fatal reactions

Đáp số: C · The patient cannot breathe out and may suffocate

Vì sao đây là đáp án

A speaking valve opens on inspiration and closes on expiration, so exhaled air must pass around the tube and up through the larynx. If the cuff is inflated, air cannot escape, causing air trapping and respiratory arrest. The cuff must be fully deflated first.

Đáp số

C · The patient cannot breathe out and may suffocate
11.Nội–ngoại khoa: thần kinh, nội tiết và thận

Đáp số: D · An oral anticoagulant

Vì sao đây là đáp án

In AF the main cause of stroke is a clot from the left atrium. Oral anticoagulants such as a DOAC or warfarin prevent this far better than antiplatelets. The start date after a stroke depends on its size and bleeding risk.

Đáp số

D · An oral anticoagulant
12.Nội–ngoại khoa: thần kinh, nội tiết và thận

Đáp số: C · Lower the patient back towards lying, then raise them gradually later

Vì sao đây là đáp án

Loss of sympathetic tone causes orthostatic hypotension after high spinal injuries. The patient is reclined, and later raised gradually, often with compression stockings, an abdominal binder and sometimes medicines.

Đáp số

C · Lower the patient back towards lying, then raise them gradually later
13.Nội–ngoại khoa: tiêu hóa, quanh phẫu thuật và ung thư

Đáp số: C · A device that sends mild electrical pulses through skin pads

Vì sao đây là đáp án

TENS is thought to reduce pain partly through the gate-control mechanism. It is non-invasive and patient-controlled, but is avoided over certain areas and with some implanted devices.

Đáp số

C · A device that sends mild electrical pulses through skin pads
14.Nội–ngoại khoa: tiêu hóa, quanh phẫu thuật và ung thư

Đáp số: A · Prothrombin complex concentrate with IV vitamin K

Vì sao đây là đáp án

Major bleeding on warfarin is treated by stopping it and giving prothrombin complex concentrate for rapid reversal with IV vitamin K for a lasting effect. Protamine reverses heparin, not warfarin.

Đáp số

A · Prothrombin complex concentrate with IV vitamin K
