1Công thức và phương pháp

Ưu tiên và ủy thác công việc

Phương pháp

  2. Áp dụng ABC (C-A-B khi ngừng tim), rồi Maslow: nhu cầu sinh lý trước an toàn, an toàn trước nhu cầu tâm lý.

  1. Bắt đầu với các biện pháp phòng ngừa chuẩn cho mọi bệnh nhân: vệ sinh tay, phương tiện bảo hộ cá nhân (PPE) cho phơi nhiễm dự kiến, xử lý vật sắc nhọn an toàn.

Đáp số: C · Let it dry fully and do not touch the site again

Vì sao đây là đáp án

Chlorhexidine-alcohol must dry completely to kill organisms effectively. Touching the site after cleaning contaminates it, so the vein is located before prepping or with a sterile glove.

Đáp số

C · Let it dry fully and do not touch the site again
4.Kiểm soát nhiễm khuẩn và an toàn người bệnh

Đáp số: C · Explain the risks, agree on ways to reduce them, respect the decision and document

Vì sao đây là đáp án

A competent patient can accept a risk. The nurse explains the danger, offers measures such as a low bed, non-slip footwear, a clear path and a call light, respects the choice and documents the discussion.

Đáp số

C · Explain the risks, agree on ways to reduce them, respect the decision and document
5.Giá trị xét nghiệm và điện giải

Đáp số: B · At about 24–28 weeks of pregnancy

Vì sao đây là đáp án

Placental hormones increase insulin resistance in the second half of pregnancy, so US guidance recommends screening all women at 24–28 weeks, with earlier testing for those with risk factors. In the UK, NICE recommends an oral glucose tolerance test at 24–28 weeks only for women with risk factors.

Đáp số

B · At about 24–28 weeks of pregnancy
6.Giá trị xét nghiệm và điện giải

Đáp số: C · Iron overload, treated with iron chelation therapy

Vì sao đây là đáp án

The body has no active way to excrete iron, so repeated transfusions cause iron overload, which can damage the heart, liver and endocrine glands. A very high ferritin reflects this, and iron chelators are used to remove the excess.

Đáp số

C · Iron overload, treated with iron chelation therapy
7.Điều dưỡng lâm sàng

Đáp số: C · Giving prompt, effective analgesia

Vì sao đây là đáp án

A sickle cell crisis needs rapid analgesia, often opioids, within a short target time, plus hydration, oxygen if hypoxic, warmth and monitoring for complications. Cold can trigger more sickling.

Đáp số

C · Giving prompt, effective analgesia
8.Điều dưỡng lâm sàng

Đáp số: D · With an observational pain tool that scores facial expression, behaviour and body language

Vì sao đây là đáp án

Observational tools such as PAINAD or the Abbey Pain Scale help assess pain in people who cannot self-report. Family and carers can describe changes in usual behaviour.

Đáp số

D · With an observational pain tool that scores facial expression, behaviour and body language
9.Nội–ngoại khoa: tim mạch và hô hấp

Đáp số: C · It suggests tissues are short of oxygen and using anaerobic metabolism

Vì sao đây là đáp án

When tissues are poorly perfused, cells switch to anaerobic metabolism and produce lactate. A raised lactate is a marker of tissue hypoperfusion and severity in sepsis and shock. Repeated measurements show whether treatment is working.

Đáp số

C · It suggests tissues are short of oxygen and using anaerobic metabolism
10.Nội–ngoại khoa: tim mạch và hô hấp

Đáp số: D · Decompensated heart failure

Vì sao đây là đáp án

Basal crackles, raised JVP, oedema and a high natriuretic peptide point to heart failure rather than a COPD exacerbation alone. Distinguishing them matters, because treatment (diuretics and HF drugs versus bronchodilators and steroids) is different. Both may occur together.

Đáp số

D · Decompensated heart failure
11.Nội–ngoại khoa: thần kinh, nội tiết và thận

Đáp số: D · To help diagnose Cushing's syndrome

Vì sao đây là đáp án

A dose of dexamethasone normally suppresses ACTH and cortisol. In Cushing's syndrome, cortisol stays high. The test is part of the work-up for cortisol excess.

Đáp số

D · To help diagnose Cushing's syndrome
12.Nội–ngoại khoa: thần kinh, nội tiết và thận

Đáp số: A · Central cord syndrome

Vì sao đây là đáp án

Central cord syndrome often follows hyperextension of an arthritic neck. The central fibres, which serve the arms, are affected most, so the arms are weaker than the legs. It is the most common incomplete cord syndrome.

Đáp số

A · Central cord syndrome
13.Nội–ngoại khoa: tiêu hóa, quanh phẫu thuật và ung thư

Đáp số: B · It already contains paracetamol, so overdose could occur

Vì sao đây là đáp án

Co-codamol combines codeine and paracetamol. Giving extra paracetamol can exceed the maximum daily dose and cause liver damage, so all sources of paracetamol are counted.

Đáp số

B · It already contains paracetamol, so overdose could occur
14.Nội–ngoại khoa: tiêu hóa, quanh phẫu thuật và ung thư

Đáp số: C · Do not use the tube until its position is confirmed by policy

Vì sao đây là đáp án

A change in external length means the tube may have moved. Pushing it back blindly is unsafe. It is not used until placement is confirmed by pH or X-ray, or it is replaced.

Đáp số

C · Do not use the tube until its position is confirmed by policy
