A woman who has had a caesarean asks when she can drive again. Which answer is most accurate?
A labouring woman has a temperature of 38.5 °C (101.3 °F), and the fetal baseline has risen from 140 to 170 bpm over the last hour. What is the most likely explanation?
A · When she can brake hard without pain
There is no fixed time. She can drive when she can brake suddenly, look over her shoulder and wear a seat belt comfortably. Many women need about 4 to 6 weeks, and insurance rules may apply.
D · Fetal tachycardia due to maternal fever or infection
Maternal fever, often from intra-amniotic infection, is a common cause of fetal tachycardia. The provider is informed, and treatment of fever and infection is expected. Opioids reduce variability rather than raise the rate.
R = 30 gtt/min
Infusion rates: a pump is set in mL/h = volume/time in hours. A gravity drip is counted in drops per minute: gtt/min = volume (mL) × drop factor (gtt/mL)/time (min). The drop factor is printed on the tubing package: macrodrip sets deliver 10, 15 or 20 gtt/mL (blood sets often 10 gtt/mL in the US, 15 in the UK) and microdrip sets 60 gtt/mL. Always use the drop factor the question or package gives.
使用微滴输液器(60 gtt/mL)时,gtt/min 等于 mL/h。
D = 10 mcg/min
Dose-based infusions: concentration = amount of drug in the bag/bag volume. Pump rate (mL/h) = dose per hour/concentration, and dose per hour = mL/h × concentration. Convert per-minute doses to per-hour (× 60) and keep the mass units the same (1 g = 1000 mg, 1 mg = 1000 mcg).
