A patient with mild dysphagia after a stroke is eating. Which technique helps make swallowing safer?
A · Constipation
Verapamil is a rate-limiting calcium channel blocker that slows AV conduction. Constipation is its most common side effect. It can also cause bradycardia and should not be combined with beta-blockers without specialist care because of the risk of heart block.
C · Take blood cultures and give IV antibiotics within the first hour
In sepsis with shock, blood cultures, IV antibiotics within the first hour, IV fluids, lactate measurement, oxygen if hypoxic and urine output monitoring are started together. Cultures should not delay antibiotics. Persistent hypotension may need vasopressors in critical care.
C · Drink plenty of fluid so the urine stays pale
High fluid intake is the most important measure for preventing stones. Dietary calcium is usually not restricted, because it binds oxalate in the gut. High-dose vitamin C can increase oxalate.
D · The other adrenal gland has been suppressed
Excess cortisol from the adenoma suppressed ACTH, so the remaining adrenal gland is shrunken and underactive. Steroid replacement is needed until it recovers, which can take months. Stopping too early risks adrenal crisis.
C · Hypomagnesaemia
Long-term PPIs are linked with low magnesium, C. difficile infection, fractures and B12 deficiency. The need for continuing treatment should be reviewed regularly.
B · Treat it as a possible burn, report it and complete an incident form
A blister under the return electrode suggests a diathermy burn. The nurse assesses and dresses it, informs the team, and reports it as an incident so that the cause can be investigated.
A · 5% glucose (D5W)
The glucose in 5% glucose is metabolised, leaving free water that dilutes the plasma sodium further. Postoperative patients often have raised ADH, so large volumes of hypotonic fluid can cause dangerous hyponatraemia.
C · High fat in the sample may interfere with some test results
Lipemia makes serum cloudy and can interfere with several laboratory methods, giving inaccurate results. The test may need to be repeated, often after a fast, according to laboratory advice.
D · Past infection that has resolved, with immunity
The vaccine contains only surface antigen, so vaccination produces anti-HBs alone. A positive anti-HBc shows past or current infection; with HBsAg negative and anti-HBs positive, the infection has cleared and the person is immune. Hepatitis Testing · MedlinePlus (NIH)
AGc = 14.75 mmol/L
A · Sitting upright with the chin tucked down while swallowing
Sitting upright and tucking the chin narrows the airway entrance and helps food pass into the esophagus. The nurse follows the speech therapist’s plan, which may include thickened fluids.
